Problematically, even products labeled as \"fragrance free\" may contain covert fragrances. Patients and physicians alike should be aware that fragrances are a common cause of allergic contact dermatitis (ACD) and that patient education is critical to treatment success. Its pathogenesis is not entirely clear but is thought to involve neurogenic inflammation as well as aberrations in peripheral nerves and central pain mechanisms. It is believed that the same mechanism that causes hypersensitivity and pain in patients with fibromyalgia also predisposes them to pruritus. Due to the variability in manifestation, hypertrophic lichen planus, pemphigoid nodularis, and neurotic excoriations tend to mimic PN. The pathophysiology of PN is believed to be due to interactions in the neural and immune pathways causing the release of proinflammatory and pruritogenic cytokines. Dermatologic and systemic conditions, including atopic dermatitis and chronic kidney disease, accompany PN diagnoses. Patients with moderate to severe or recalcitrant PN may benefit from dupilumab, the first medication approved by the US Food and Drug Administration (FDA) to treat PN. We performed a systematic search of the literature for cases of epidermal tumors arising on skin graft donor sites in patients undergoing autologous skin graft surgery. Our results included the demographic and clinical characteristics of these cases. {"title":"Erythema Nodosum Triggered by a Bite From a Copperhead Snake.","authors":"Jack Newcomer, Chandler Jansen, Dirk M Elston","doi":"10.12788/cutis.1074","DOIUrl":"10.12788/cutis.1074","url":null,"abstract":"<p><p>Erythema nodosum (EN) is a form of septal panniculitis that can occur secondary to multiple etiologies including inflammatory disorders, infections, or medications. We describe a 42-year-old woman who developed tender, erythematous, subcutaneous nodules persisting for 4 months following a copperhead snakebite. The diagnosis of EN was confirmed via histopathology, and the patient was treated with colchicine. Physicians should be aware of possible etiologies of EN to evaluate patients who present with new-onset tender subcutaneous nodules.
