We surveyed US dermatology residency leadership and faculty to evaluate experiences with and opinions of the existing system. Overall, respondents expressed broad support for tiered preference signaling while identifying opportunities to improve consistency, equity, and transparency. Areas for potential refinement included guidance for special applicant groups, interpretation of signals, and the role of geographic preference. In this study, we aimed to identify factors associated with a higher follower count among American Board of Dermatology-certified dermatologists on Instagram and TikTok. We also summarized high-engagement content themes. Using platform analytics, we short-listed board-certified dermatologist (BCD) accounts with 100,000 followers or more and abstracted activity metrics. Fame was defined as 100,000 followers or more on both platforms. Group differences were assessed using independent-sample t tests, Mann-Whitney U tests, and χ<sup>2</sup> tests, as appropriate; correlations were assessed using Spearman rank correlation (P<.05). Sixty-five BCDs met the inclusion criteria, contributing 129 accounts (65 Instagram; 64 TikTok); 37 (56.9%) met the definition of fame described above. On Instagram, famous BCDs had more total posts (1900 vs 1100; P=.010), more YouTube subscribers (680,000 vs 48,000; P<.001), and larger TikTok followings (1,600,000 vs 110,000; P<.001). On TikTok, famous BCDs had more total posts (1300 vs 324; P<.001), greater research output (1.4 vs 0.8 articles/y; P=.045), larger Instagram followings (690,000 vs 140,000; P<.001), and more YouTube subscribers (680,000 vs 48,000; P<.001). TikTok followers correlated with total TikTok posts (r=0.807; P<.001), and Instagram followers (r=0.471; P<.001) were inversely correlated with age and years in practice. High-engagement themes centered on diagnoses/skin care and procedures, often via treatment tips and duet/reaction formats. This column reviews the American Medical Association (AMA)/Specialty Society Relative Value Scale Update Committee (RUC), the expert panel that recommends physician work and practice-expense values to the Centers for Medicare & Medicaid Services (CMS). We describe the RUC's composition, dermatology's representation through the American Academy of Dermatology (AAD), and the pathway by which a code is valued or revalued. Central to that pathway is the RUC survey, through which practicing physicians supply the time and intensity estimates that anchor each recommendation; we outline what the survey asks and offer practical guidance for completing one. We describe a simple intraoperative adjunctive technique: a focal blister that may form at a recent biopsy site during slow intradermal local anesthetic infiltration. This finding may reflect transient immaturity of anchoring fibrils during early wound remodeling and appears to occur most often within the first few weeks after biopsy. Although it requires no extra equipment and can help confirm a carefully chosen presumptive site, it should be used only as an adjunct to standard evaluation, not as proof or a reason to inject multiple sites. This article provides a practical framework for developing effective prompts for dermatology applications by outlining 5 core components: persona, task, context, format, and constraints. It also describes strategies to optimize interactions with AI, including iterative prompting, meta-prompting, voice-to-text input, and requesting clarifying questions before AI generates a response. Hispanic and Latino patients, many with darker skin tones, may face unique challenges in the presentation, recognition, and long-term effects. In our literature review, reported toxicities included dermatitis, vitiligo, rash, maculopapular exanthema, panniculitis, nail toxicity, and other hypersensitivity reactions. Radiotherapy toxicities were most consistently reported, with rates of moderate to severe dermatitis similar to prior research, but patients with skin of color often experienced more persistent pigmentary changes and long-term symptoms. Immune checkpoint inhibitor toxicities were reported less consistently, and limited representation prevented meaningful assessment of differences by ethnicity.
