It is therefore important to identify factors that positively or negatively influence the birth experience. The study objective was to determine the social-demographic variables associated with intrapartum care experience at a tertiary Hospital in Kenya. This cross-sectional descriptive design employed systematic sampling to recruit 109 mothers. An established research instrument with local cultural adaptations was used. Independent variables were; parity, marital status, occupation, monthly income and education level. Dependent variables were effective communication; respect, care and dignity and emotional support. Almost half of the mothers were aged between 20-24 years, over two thirds (71.6%) were married, with almost half (48.6) attaining primary education and over a third (37.6%), secondary. Nearly half (45%) were self-employed, (52%) earned US$ 50-99.9 per month. Marital status (rs=- 0.192, p=0.046) was significantly associated with midwives explanations, perceived to be in understandable terms. An inverse significant rs=-0.192, p=0.045 correlation, was found between being employed and explanation of procedures being performed. There was an inverse significant (rs=-0.216, p=0.024) correlation between being employed and genuine interest displayed by midwives. An inverse significant (rs=-0.202, p=0.036) correlation, was found between marital status and mothers being asked for consent before procedures. Majority reported being treated with respect, were accorded privacy, and had consent requested before procedures. Many reported they were left alone (88.1%), and almost all mothers were \"scolded\" by their midwives during child birth (97.2%). The study revea led that various sociodemographic factors influence mothers’ experience of care during the intrapartum period. Midwives need to be cognizant of this in order to improve quality of maternal care in Kenya.","PeriodicalId":111627,"journal":{"name":"International Journal of Midwifery and Nursing Practice","volume":"9 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130592521","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is one of the most commonly preferred obstetric operation during delivery since 1920 and practised routinely in most of the developing countries till today. Restricted practice refers to episiotomy reserved for situations where there is clear indication to perform the procedure. World Health Organisation (2018) recommends restricted practice of episiotomy. Voluminous literature highlights extensive disagreement about the necessity and benefits of the procedure. Even though the pendulum has swung back the practice of liberal episiotomy is still higher than the expected rate. Role of decision ends up in the midwives/ obstetrician’s hands who conduct the delivery. Limited researchers have examined the motive behind practicing liberal episiotomy. In this context a qualitative study was conducted to address the issues from midwives/obstetrician’s own perception regarding restricted practice of episiotomy. Materials and methods: This is a phenomenological approach qualitative study using the thematic analysis. The participants included 15 midwives and 8 obstetricians with minimum experience of one year selected by purposive sampling and attended semi structured interview. Results: The themes emerged after data analysis are Negative impact on self and maternal morbidity and Inadequate readiness. Conclusion: The subjective opinion of midwives/obstetricians could identify reasons behind the reluctance to give up the age old practice and threw light in to the gap that exists in practice and scientific evidence. Conscious effort may be taken by the midwives /obstetricians to adopt the best evidence based protocol and deliver quality care.","PeriodicalId":111627,"journal":{"name":"International Journal of Midwifery and Nursing Practice","volume":"66 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133826383","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Once obstetric fistula occurs, it requires reconstructive surgery as fistulas usually cannot heal by themselves. However, most afflicted women do not seek fistula repair services. Objective : To explore barriers in seeking fistula repair services among women with obstetric fistula. Methods : This was a qualitative study. The study was conducted between October, 2017 and September, 2018. The study setting was Muchinga, Luapula, Eastern and Southern provinces of Zambia. A purposive sample of 16 participants was recruited and interviewed in their homes. In-depth interviews were conducted. Data was analyzed manually using thematic content analysis. Results : The age of the participants ranged from 19 - 87 years. Four thematic areas were identified that included individual, household, community and health systems barriers. Conclusion : Women either delayed in seeking fistula repair services or did not seek the service at all. Strategies such as addressing barriers at individual, family, community and health facility levels must be implemented in order to improve access to fistula surgical repair services.","PeriodicalId":111627,"journal":{"name":"International Journal of Midwifery and Nursing Practice","volume":"17 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126704242","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There is a popular acknowledgement of the importance of establishing patient safety culture in healthcare organizations. Hospitals with a positive patient safety culture are transparent and fair with staff when incidents occur, learn from mistakes, and rather than blaming individuals, look at what went wrong in the system. Health care providers are willing to report the errors but, due to poor reporting system and culture of blame and shame, there exists struggle of disclosure of adverse events. Objective: To investigate the influence of patient safety culture on incident reporting behavior among health care professionals in public hospitals in Addis Ababa, central Ethiopia. Methods: Institution based cross-sectional study was conducted from March 15-20, 2017 at public hospitals in Addis Ababa. A total of 697health professionals were selected by simple random sampling method. Self-administered questionnaire was distributed to collect the data. A multivariate linear regression model was fitted. Then the effect of the socio-demographic variables and patient safety culture dimensions on the dependent variable “incident reporting behavior” was assessed using multiple linear regression analysis. Results: Among the participants, 20.4% never reported an incident, 13.1% reported rarely, 19.9% reported sometimes. Only 30.4 % reported incidents al ways. Feedback about error (β=0.136, p=0.008), management support for safety (β=0.28, p< 0.001), Non-punitive response to error, Supervisor/manager expectation and actions promoting patient safety (β=0.356, p< 0.001) and communication openness (β=0.170, p< 0.001) were the most predictive dimensions of patient safety culture for the incident reporting. Conclusions: Incident reporting behavior among health care professionals was very low. To increase the incident reporting behavior, placing priority on improving event reporting feedback mechanisms, communication regarding systems and process, giving priority by top-level hospital leadership and non-punitive response to errors.","PeriodicalId":111627,"journal":{"name":"International Journal of Midwifery and Nursing Practice","volume":"223 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127182467","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
