Lifestyle interventions are effective; however, barriers interfere with in-person program participation. This two-period randomized cross-over pilot study evaluated the feasibility and acceptability of a virtual lifestyle intervention in n = 19 individuals with schizophrenia. Secondary analyses explored outcome changes between intervention and control groups. Results indicated modest retention (58%) and intervention engagement (67%) for Period 1 (weeks 1-11) with especially low rates in Period 2 (weeks 12-22; retention: 37%, intervention engagement: 50%). Of participants with satisfaction data (n = 6), nearly all participants reported that the intervention was helpful and easy to follow. Secondary outcome changes during Period 1 favored the intervention group on nutritional knowledge, physical activity, mental health, physical health, exercise motivation, self-esteem, and weight/body mass index (Cohen's ds: 0.35-2.09). Due to attrition and sparsity of data, neither a cross-over design analysis nor examination of outcome changes during Period 2 was conducted. Although satisfaction results and outcome changes from Period 1 were encouraging, the low rates of retention and intervention engagement in Period 2 raise feasibility concerns. Intervention development research is needed to determine whether modifications to this virtual lifestyle intervention would enhance feasibility and acceptability for people with schizophrenia.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148976018","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While pharmacological and psychotherapeutic interventions remain central to treatment, peer support has emerged as a promising complementary approach that fosters agency, psychosocial recovery, and affective connection. This study explored the perceived effectiveness of Peer Support Groups (also known as Peer Support Circles or CAPs in Spanish)-a pioneering, community-based intervention in Chile for individuals diagnosed with bipolar disorder (BD). The research examined participants' experiences, focusing on relational benefits, challenges, and group dynamics. A qualitative design was used. In-depth semi-structured interviews were conducted with 17 participants (7 peer supporters and 10 peer participants). Data were analyzed using Constructivist Grounded Theory (CGT) and intersubjective validation was performed within the research team. Three main analytical categories were identified: (1) Benefits of participating in the CAPs, (2) Conditions that enable CAPs' effective functioning, and (3) Challenges and limitations CAPs face. The central phenomenon that emerged was the CAPs' capacity to lower the barrier of isolation commonly experienced by people with BD through relational safety and peer-driven meaning-making. CAPs operate not only as spaces for support and psychoeducation but also as platforms for identity reconstruction and community reintegration. Findings highlight the value of peer-led models for advancing recovery-oriented care and the importance of institutional support to sustain them and maximize their impact.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148966728","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Individuals from ethnically minoritised backgrounds are disproportionately affected by early discontinuation; however, less is known about how these populations understand and experience this process. This review aimed to explore the lived experiences and perceived reasons for psychotherapy discontinuation among adults from ethnically minoritised backgrounds. A systematic review and qualitative synthesis were conducted. Six electronic databases were searched from 1983 to February 2026. Eligible studies included qualitative and mixed-methods designs where qualitative findings could be extracted. After screening, 13 studies involving 286 participants were included. Study quality was assessed using the Critical Appraisal Skills Programme qualitative checklist. Data were synthesised using narrative synthesis. Six interconnected themes were identified: (1) perspectives and expectations of therapy; (2) cultural and societal influences; (3) practical and accessibility-related barriers; (4) the therapeutic relationship; (5) systemic and service-level barriers; and (6) autonomy and control in treatment engagement. Across studies, psychotherapy discontinuation was characterised as a multidimensional process shaped by interacting individual, cultural, relational, and structural factors. Discontinuation was not solely attributable to lack of motivation but was strongly influenced by structural inequalities, cultural incongruence in care, ruptures in the therapeutic relationship, and limited patient autonomy. Findings suggest that improving engagement and reducing psychotherapy discontinuation among ethnically minoritised populations requires interventions that extend beyond service provision alone. Culturally responsive practice, stronger therapeutic alliances, shared decision-making, and flexible models of care delivery may help address the complex barriers to sustained engagement in psychotherapy.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148944413","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Yet prevention, screening, and treatment services for mental health and for substance use often are delivered separately, creating missed opportunities for effective integrated, co-occurring competent care. This Fresh Focus Policy and Practice commentary makes the case for a more intentional and systematic recognition of mental health drivers of substance use initiation and escalation to substance use disorder and of substance use drivers of mental health challenges and progression to mental health disorders, particularly among young people. A targeted, non-systematic review of relevant literature illustrates that the interplay between mental health and substance use often is recognized only in hindsight by people in treatment - and by those who care for them, which may limit the provision of appropriate interventions and the prevention of co-occurring disorders. An integrated approach to prevention, early identification, and intervention for both substance use and mental health challenges in youth is grounded in research, practical, and cost-effective, yet its implementation continues to lag behind the evidence. This commentary highlights concrete opportunities for prevention professionals, educators, clinicians, and policymakers to advance earlier, more integrated, co-occurring competent care.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148896693","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Mental disorders and psychotropic medications can cause sexual dysfunctions. Despite its clinical relevance, sexual health is often not routinely assessed or discussed in psychiatric assessment and treatment, possibly due to insufficient training, stigma, and clinicians' uncertainty. This pilot study examined how sexuality is addressed in psychiatric treatment and how relevant the topic is perceived, integrating patient and clinician perspectives. A cross-sectional, anonymous online survey was conducted between February and July 2025 in two German academic psychiatric hospitals. Psychiatric patients and mental-health professionals completed questionnaires tailored to their perspectives. Patients completed measures of sexual functioning, well-being, and communication about sexuality, whereas mental-health professionals completed measures of attitudes, clinical practice, and professional training. Data were analysed using descriptive and inferential statistics. A total of 231 patients and 194 clinicians participated. Nearly all patients reported at least one sexual dysfunction (100% women, 97% men), while only 18.7% reported being sexually satisfied. Although most patients (62.0%) considered sexuality an important topic, only 12.5% reported that it had been addressed. Clinicians reported addressing sexuality more frequently (psychotherapists: 44.0%; specialists: 62.1%). Among physicians, confidence and years of professional experience predicted actively discussing sexual issues (p = .013). More than 80% of clinicians reported insufficient coverage of sexual health during their education. Sexual health remains a neglected domain in psychiatric care despite being recognized as important by both patients and clinicians. Strengthening clinicians' confidence through targeted training, routine screening, and institutional support may help normalize the assessment of sexual health as part of holistic, patient-centred psychiatric care.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148879451","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, culturally grounded frameworks describing how therapeutic relationships support recovery in Japan remain limited. This study explored the perspectives of Japanese community mental health service users to identify relational components that facilitate recovery and develop a culturally responsive conceptual framework. Using a phenomenological-qualitative design, semi-structured interviews were conducted with ten service users receiving community-based care. The data were analyzed through reflexive thematic analysis. Seven components were identified and organized into a two-stage structure. The listening, individuality, friendliness, and empowerment (LIFE) stage describes relational processes that build psychological safety through respectful communication, personal identity recognition, approachable interpersonal interaction, and self-determined decision-making support. The awareness, commitment, and exposure (ACE) stage captures subsequent processes that promote recovery-oriented action, including gaining new perspectives, setting and pursuing personally meaningful goals, and engaging in social roles and group interactions. Together, the LIFE-ACE framework illustrates the transformation of therapeutic relationships from trust-building to fostering social participation. It aligns with recovery principles while reflecting Japanese cultural values such as interpersonal harmony, empathy, and relational subtlety. The findings highlight the importance of approachable communication, recognition of individuality, and supportive opportunities for social engagement in cultivating recovery-oriented practice. The framework offers practical guidance for mental health professionals seeking to create culturally responsive therapeutic relationships that enhance psychological safety, autonomy, and meaningful participation in community life.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148872889","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aims to evaluate the feasibility of a post-discharge telephone contact pilot program designed to assist caregivers of youths experiencing mental health emergencies in navigating these resources. The pilot initiative was retrospectively evaluated after being integrated into routine clinical care across inpatient, ambulatory, and emergency settings within a children's hospital in Baltimore, Maryland, from August 2022 to August 2023. The primary outcomes evaluated included: (1) the successful contact of caregivers, (2) the establishment of an initial mental health appointment for their child after a mental health emergency, and (3) the solicitation of additional assistance. Over the course of one year, a dedicated social worker, contributing 10 h per week, contacted nearly 1,000 caregivers. The program successfully reached 55% of caregivers with only one follow-up attempt and provided additional assistance to 21% of those engaged. This pilot program demonstrated the feasibility of multilingual caregiver outreach across demographic groups using existing clinical infrastructure to help families navigate post-discharge mental health care. The study indicates that such a program can be successfully implemented within existing infrastructure using minimal additional resources for outreach while identifying ongoing caregiver support needs following pediatric mental health emergencies.
","PeriodicalId":10654,"journal":{"name":"Community Mental Health Journal","volume":" ","pages":""},"PeriodicalIF":2.1,"publicationDate":"2026-08-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148863775","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, less is known about how peer support workers experience supervision in substance use settings. This study explored the experiences of supervision among peer support workers employed in substance use services. A qualitative design using a hybrid inductive-deductive analytic approach was used with 12 peer support workers from substance use services. Participants took part in qualitative interviews. Interviews were conducted online and data were analysed using thematic analysis. Four themes were developed: supervision as a space for emotional containment; supervision as role clarification; negotiating peer identity within professional systems; and the need for peer-informed and organisationally embedded supervision. Participants valued supervision when it provided safety, reflection, boundaries and recognition of the distinctiveness of lived experience work. However, supervision was experienced as less helpful where it was overly managerial, clinically framed or disconnected from the realities of peer work in substance use settings. This study contributes to the emerging literature on the peer support workforce by focusing specifically on supervision in substance use services.
