We sought the correlation of ESR with the total and differential leucocyte counts in a series of sick and healthy subjects (n=238). \u0000Results: The ESR values of sick patients were higher alongside a lower mean leukocyte count for this group. In both groups the leukocyte counts correlated negatively with ESR values (P healthy subjects (n=238). \u0000Conclusion: The presence of organic infections can lead to overall reduction in the number of leucocyte. The presence of some endemic or organic diseases conditions may the underlying aetiological factor in the so called ethnic leucopaenia of persons of African origin. The measurements were done using the mandibulometer according to accepted definitions of the different perameters. \u0000Results: For the right mandible, the mean mandibular angle was found to be 120.41\u0000°±6.29, mean mandibular length 9.27cm±0.650 and mean mandibular ramus height 5.58cm±0.555. For the left mandible, the correspponding mean values were 120.03°±5.800 for the mandibular angle, 8.83cm±0.830 for the mandibular length and 5.81cm±0.451 for the mandibular ramus height. The Angle/Height ratio for the right mandible was found to be 21.58 and 13.01 respectively, while the corresponding values for the left mandible were found to be 20.66 and 13.50 respectively. These differences were found not to be statistically significant (p \u0000Conclusion: Our findings suggest that Nigerians of Eastern origin apparently exhibit a short ramus and a narrow mandibular angle. The implication of this is in the ease of laryngoscopy and intubation for the Eastern Nigerian will need further investigation. The object of this study therefore is to identify trauma death pattern in our environment with emphasis on time of death with the view to proffer efficient trauma care strategies. \u0000Methods: The medical records of patients admitted acutely through the Accident and Emergency Department of our institution and who subsequently died between 1998 and 2000 were reviewed. Such information as their age, sex, aetiology of injury, primary diagnosis on admission, interval between injury and presentation, and time to death, and the cause of death as recorded in the duplicate copy of death certificates were collected and analyzed. \u0000Results: There were 85 trauma related deaths, fifty-one males and thirty-four females. The male to female ratio is 1.5:1. The age range is 6-78 years with a mean of 36-/+2.5 years. The most common aetiological factor was road traffic accidents 43(50.59%). This was followed by burns35 (41.17%). Of the 85 patients, burns were the most common diagnoses on admission 46 (54.12%). Fifteen (17.65%) cases sustained injuries to multiple organ systems injuries while open fractures accounted for 12(14.12%) of the deaths. The time to death showed two peaks. The first peak accounted for 12(14.12%) of the deaths. The time to death showed two peaks occurred within the first 96 hours du to uncompensated shock, respiratory failure and acute renalshot down while the second peak was observed after the twelfth day due to sepsis and multiple organ failure. \u0000Conclusion: Trauma is common cause of death. In our setting patients that arrived the hospital alive run the risk of of dying from complications ranging from uncompensated shock in the immediate post injury period to multiple organ failure later. These deaths are largely preventable if a well-funded comprehensive trauma system manned by skilled personnel is put in place. Each perameter was measured several times with a vernier sliding caliper until three consistent values were obtained. \u0000Results: The results obtained indicate a sexual dimorphism, with a significantly higher value of all the perameters in males compared to females (p \u0000Conclusion: The mean values of these perameters for Nigerians reported in the present study will provide baseline data for anthropologists, functional orthodontics, cosmetic plastic surgeons, and the otolaryngologist in Nigeria. Dermoid apidermoid cysts represent about 0.01% of all oral cysts with an incidence of 1.6 to 6.9% in the head and neck region. The can present at birth and remain undiagnosed until they present cosmetic problems to the patient in adult life. They may also present in adult life with vaying degrees of problems. \u0000Reason for reporting: This case is reported because it was first diagnosed when the patient was 6 months old but was not treated and presented in the clinic when it was causing aesthetic problems when the patient was 20 years old. \u0000Case Report: The patient was a 20-year old male with a sub mental swelling. The swelling, which measured 6.5cmx5cm clinically, was reported by the sonologist to be 6cmx3.9cm. It was doughy in consistency and did nit interfere with swallowing. It was neither visible nor palpable intra-orally. A sub mental vertical incision was made and the cyst was located on the mylohoid muscle and was excised. The histology confirmed dermoid cyst. No recurrence of the cyst has occurred after 18 months of follow up. \u0000Conclusion: Dermoid Cysts are sometimes difficult to diagnose in an infant or new born because they may be small and symptomless. An appropriate referral to and follow up by the oral and maxillofacial surgeon to remove them early is advocated. Relevant literatures on the subject are presented. \u0000Results: There were neonates with neonatal sepsis. Initial attempt to secure peripheral veins failed and umbilical catheterization was done without bleaching. Both developed peripheral gangrene between 4-9 days post catheterization. \u0000Conclusion: We reaffirm earlier observations that umbilical catheterization beyond 48 hours is associated with high risk of peripheral gangrene. Ilechukwu","doi":"10.4314/OJM.V16I3.29082","DOIUrl":"https://doi.org/10.4314/OJM.V16I3.29082","url":null,"abstract":"Objective: It has been postulated that offering bland diets to infants could habituate to food refusal during early childhood. \u0000To investigate the complementary feeding pattern in Nigerian preschool children and a possible association with their current feeding habits, a cross-sectional study of two hundred (200) children was done in Enugu, eastern Nigeria. \u0000Method: Their mothers were interviewed with a structured questionnaire. \u0000Results: Irrespective of socio-economic status, majority of the children were commenced on complementary foods between 4 and 6 months of life, which mainly consisted of poorly fortified home-prepared cereals. A higher percentage (90%) of the children who exhibited food-refusal habit at the time of study were given cereals not fortified with sweeteners, compared to about 75% of their counterparts without this habit who were similarly fed. The difference between the two groups was however not statistically significant (x 0.795, p > 0.05). \u0000Conclusion: Although a valid association has not been established between complementary feeding pattern and food refusal, a longitudinal study may be required to establish any causal relationship. Ezepue, Nwabueze O Magulike","doi":"10.4314/OJM.V16I3.29081","DOIUrl":"https://doi.org/10.4314/OJM.V16I3.29081","url":null,"abstract":"Objective: To determine the magnitude and pattern of ocular disorders and blindness among leprosy patients, presenting at three leprosy clinics in South —Eastern Nigeria. \u0000Methodology: All the in- patients, as well as the out- patients that presented to the 3 leprosy clinics during the 2- month period of the study were examined. Altogether, 171 patients were studied. All data were entered into the computer and analyzed using the SPSS software package. \u0000Results: Ocular examinations revealed that 60.2% of the patients had leprotic lesions. Other findings were cataract 24.6%; pterygium 24.6%; refractive errors 21.6%; glaucoma 12.3%; age- related macular degeneration 4.6%; presumed toxoplasmosis 1.2%; optic atrophy 1.2% and squint 0.6%.A total of 10.5% of patients were blind and 39.8% visually impaired. Cataract accounted for \u000055.6% blindness. \u0000Conclusion: It is concluded that non- leprotic lesions, particularly cataract were responsible for most of the blindness. We recommend that ophthalmic surgeons should organize regular and periodic surgical outreaches to leprosy centers with the aim of dealing with non- leprotic causes of avoidable blindness in such centres. Ogbonnaya","doi":"10.4314/OJM.V16I3.29079","DOIUrl":"https://doi.org/10.4314/OJM.V16I3.29079","url":null,"abstract":"Objective: To analyze the specialty choice of residents in the University of Nigeria Teaching Hospital Enugu from 1989 - 1999, and to assess the ratio of community physicians to the clinical specialties of residents. \u0000Method: The record of admissions into the residency training programme in the University of Nigeria Teaching Hospital Enugu was analyzed. Simple percentages and proportions of residents in the different specialties were computed and compared with one another and with community medicine. \u0000Result: The ratio of community medicine to the other clinical departments of Surgery, Internal medicine, Obstetrics & Gynaecology and Paediatrics is 1:7. When compared individually, each of these four were at a ratio of about 2:1 to community medicine. \u0000Conclusion: This study and other studies from other parts of the nation show that Nigeria is producing more core clinicians than community health physicians. As the nation's health policy is anchored on Primary Health Care, Community Health Physicians may be better suited to implement the nation's health policy. It would therefore be beneficial to increase the number of community Health Physicians produced by training institutions in Nigeria. The result can have significant impact on the patient. This study was therefore undertaken to review the use of anticoagulation in embolic stroke in the setting of atrial fibrillation at the UNTH Enugu. \u0000Method: Records of patients managed for embolic stroke at the University of Nigeria Teaching Hospital Enugu in a total study period of 20 years (1980 - 1999) were retrospectively studied. Data was collected from patients' case records using a proforma that had been specifically designed for the study. \u0000Results: Twenty three patients with embolic stroke were reviewed. Nine patients had atrial fibrillation as risk factor of which six patients received anticoagulation for secondary prophylaxis of embolic stroke. A greater fraction of requiring males received anticoagulation than that in requiring females. All the patients that were anticoagulated were lost to follow-up. \u0000Conclusion: This study shows a considerable willingness in physicians to commence anticoagulation in the setting of embolic stroke with concurrent atrial fibrillation. Efforts should be made to enhance physician confidence and attitude towards appropriate anticoagulation where required. Measures to encourage adequate follow-up of patients are required.
