Xiao, Xiaopei Xue, Zhi-zhang Xu, Bao-yu Zhao, Wenping Wang","doi":"10.3760/CMA.J.ISSN.1004-4477.2020.01.012","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1004-4477.2020.01.012","url":null,"abstract":"Objective \u0000To investigate the current basic situation of ultrasound machines of ultrasound departments in Shanghai medical institutions, and to provide the basis for making management policy in order to promote medical quality. \u0000 \u0000 \u0000Methods \u0000Questionnaire surveys about ultrasound machines and service ability including producing countries of ultrasound machines, the number of ultrasound machines, using years of ultrasound machines, yearly workload and the yearly number of patients that each ultrasound machine accepted were made in 2013 and 2018, respectively. Statistical results were compared between the two surveys. \u0000 \u0000 \u0000Results \u0000Compared with 2013, the share of imported ultrasound machines declined in 2018 (82% vs 91%). Compared with 2013, the number of ultrasound machines in Shanghai medical institutions had increased by 31% in 2018 (2 123 vs 1 617). The occupancy rate of ultrasound machines in tertiary hospitals was the highest (tertiary hospitals 40%, secondary hospitals 25%, primary grade hospitals 20%, and private hospitals 15%). Compared with 2013, the proportion of ultrasound machines that have been used for more than 10 years increased (9% vs 4%), the yearly workload of ultrasound examination had increased by 46% (19.82 million person-time vs 13.59 million person-time). Tertiary hospitals bored the highest proportion of the workload (tertiary hospitals 51%, secondary hospitals 35%, primary grade hospitals 4%, and private hospitals 10%). Currently, the number of ultrasound machines per 10, 000 people in Shanghai was 1.14. The yearly number of patients that each ultrasound machine accepted had increased by 11% (9300 person-time vs 8400 person-time in 2018). \u0000 \u0000 \u0000Conclusions \u0000The scale of ultrasound departments in Shanghai medical institutions has been developing. Brand localization of ultrasound machines is improving. However, the renewal ability of ultrasound machines still needs to be improved. The workload of ultrasound department is getting heavier. Thirty-three healthy volunteers were selected as control group at the same period. Two-dimensional dynamic images in apical two chamber, three chamber and four chamber views were collected by two-dimensional speckle-tracking. Using the off-line EchoPAC software, a tracing analysis was conducted and the blood pressure was entered to obtain left ventricular global longitudinal strain (GLS), global MW index (GWI), global constructive work (GCW), global wasted work (GWW) and MW efficiency (GWE). The differences of GLS and MW parameters (GWI, GCW, GWW, GWE) were compared between each groups, and Pearson correlation was used to analyze the correlation between MW parameters and LVEF. \u0000 \u0000 \u0000Results \u0000Compared with the control group and HFpEF group, the left atrial dimension(LAD), left ventricular end-diastolic dimension(LVDd), left ventricular end-diastolic volume(LVEDV), left ventricular end-systolic volume(LVESV) in HFrEF group increased, while LVEF decreased significantly(P<0.05). Compared with the control group, LAD in HFpEF group increased, the vaules of GLS, GWI, GCW were obviously lower in HFpEF group and HFrEF group(all P<0.05), GWW was higher and GWE was lower in HFrEF group(all P<0.05). GLS, GWI, GCW, GWE decreased and GWW increased in HFrEF group than those in HFpEF group, and all the differences were statistically significant(all P<0.05). In CHF group, GWW was negatively correlated with LVEF (r=-0.521, P<0.001) while positive correlations between GWI, GCW, GWE and LVEF were discovered (r=0.846, 0.807, 0.788; all P<0.001). \u0000 \u0000 \u0000Conclusions \u0000PSL can effectively evaluate the left ventricular MW in CHF patients. It has a certain correlation with LVEF and can reflect left ventricular systolic function. According to the numbers of cardiovascular risk factors, the patients were divided into group A (without cardiovascular risk factors, 31 cases), group B (single cardiovascular risk factor, 20 cases), and group C (multiple cardiovascular risk factors, 13 cases). All patients underwent echocardiography before and after epirubicin respectively. Global longitudinal strain, circumferential strain, radial strain (GLS, GCS, GRS), endocardial and epicardial layers of GLS and GCS (endoGLS, epiGLS, endoGCS, epiGCS) were obtained by EchoPAC software analysis. The differences of GLS, EndoGLS, EpiGLS, GCS, EndoGCS, EpiGCS and GRS before and after chemotheraphy were calculated as ΔGLS, ΔEndoGLS, ΔEpiGLS, ΔGCS, ΔEndoGCS, ΔEpiGCS and ΔGRS. \u0000 \u0000 \u0000Results \u0000Compared with before chemotherapy, GLS, EndoGLS, EpiGLS, GCS, EndoGCS, EpiGCS and GRS were significantly reduced in group A, B and C after chemotherapy (all P<0.05). ΔEndoGLS and ΔEndoGCS showed an increasing trend in groups A, B and C, with statistical significance (all P<0.05). Correlation analysis showed that after chemotherapy with epirubicin, the numbers of cardiovascular risk factors were significantly correlated with strain parameters, and the correlation coefficient of EndoGLS was relatively higher (r=-0.582, P<0.001). \u0000 \u0000 \u0000Conclusions \u0000Layer-specific imaging can reliably detect the influences of cardiovascular risk factors on the left ventricular wall motion function in breast cancer patients treated with epirubicin. TRUS was performed within one week before pathological examination. The sonographic features were summarized, ultrasonic typing was performed according to the lesion size and growth pattern, and the accuracies of preoperative ultrasound diagnosis and tumor staging were analyzed. \u0000 \u0000 \u0000Results \u0000Among the 22 lesions, 63.64%(14/22) were with a distance of ≤3 cm between the anal verge and the lower margin of tumor, 68.18%(15/22) with regular shape, 81.82%(18/22) with clear boundary, and 63.64%(14/22) with Adler blood flow grading from Ⅱ to Ⅲ. The ultrasonic manifestations were graded and classified before UAE. The intraoperative and postoperative complications, time of β-human chorionic gonadotrophin declined to normal limits were evaluated, and the formation of hematoma and hematoma absorption time were followed up by ultrasound. \u0000 \u0000 \u0000Results \u0000UAE was performed successfully in all patients. The ultrasound-guided scar pregnancy was successfully completed. The uterus was successfully retained in all patients.Among the 42 patients, 18 cases were 3 scores group, and 24 cases were 4 scores group according to the ultrasonic scoring system. Two cases were type Ⅰgroup, 25 cases were type Ⅱ group, and 15 cases were type Ⅲ group according to the ultrasonic classification. The amount of intraoperative complications, postoperative mass formation and absorption time were significantly different between 3 scores group and 4 scores group (P 0.05). \u0000 \u0000 \u0000Conclusions \u0000Ultrasonography has clinical application value in evaluating the prognosis of CSP treated with UAE. All patients underwent conventional two-dimensional echocardiography (2DE), tissue Doppler imaging (TDI), and 3D-STE. The following parameters were acquired and compared between the groups: interventricular septal end-diastolic thickness (IVSD), left ventricular posterior wall end-diastolic thickness (LVPWD), left ventricular mass(LVM), left ventricular mass index (LVMI), left atrial volume (LAV), and left ventricular end-systolic volume(LVESV), left ventricular end-diastolic volume (LVEDV), left ventricular outflow tract pressure gradient (LVOT-PG), left ventricular outflow tract velocity time integral (LVOT-VTI), isovolumic relaxation time (IVRT), global strain (GS), global longitudinal strain (GLS), global radial strain (GRS), global circumferential strain (GCS). Conventional 2DE examinations were performed. The volume datasets were analyzed offline using the new automatic image processing software SonoVCADheart. The diameters of AO and PA were measured by 2DE and SonoVCADheart, respectively. Pearson correlation analysis was used to evaluate the correlation between the two methods. The consistency of the two methods was verified by Bland-Altman analysis, and he reliability of SonoVCADheart was assessed using the intraclass correlation coefficient (ICC). \u0000 \u0000 \u0000Results \u0000Fetal ventricular outflow views were successfully obtained using SonoVCADheart in 73(91.2%) of 80 fetuses. There were good correlations between the two methods for measuring the diameters of AO and PA (r=0.953, 0.971; all P<0.001). The 95% agreement limits of AO and PA were (-0.669, 0.568)mm and (-0.632, 0.580)mm, respectively. Among 60 male Sprague Dawley (SD) rats, 50 rats were randomly selected and were induced to be DM models by streptozotocin(STZ) through intraperitoneal injecting, the other 10 rats as control group. Then DM rats were randomly divided into 4 groups: DM model group, MaFGF solution group, MaFGF-nlip group and MaFGF-nlip+ UTMD group. After the successful induction of DM model, the intervention was performed twice a week.After 12 weeks of intervention, all rats underwent conventional echocardiography and velocity vector imaging (VVI). Left ventricular ejection fraction (LVEF) and left ventricular fraction shortening(LVFS) were measured by conventional echocardiography. The mean peak systolic radial velocity (Vs), radial strain (Sr) and radial strain rate (SRr) of six walls at the papillary muscle level were measured in left ventricular short-axis view by VVI. At last, myocardial tissue of all rats were stained with Sirius red to evaluate myocardial interstitial fibrosis. The level of myocardial apoptosis was evaluated by TUNEL staining, and the changes of myocardial ultrastructure were observed by transmission electron microscopy. \u0000 \u0000 \u0000Results \u0000The prepared MaFGF-nlip were more rounded, evenly dispersed, and of good stability and high encapsulation efficiency. Twelve weeks later after intervention, LVEF, LVFS, Vs, Sr and SRr in the DM model group were significantly lower than those in the control group (all P<0.05). LVEF, LVFS, Vs, Sr and SRr in the MaFGF-nlip+ UTMD group were significantly higher than those of the DM model group and other intervention groups (all P<0.05). The results of Sirius red staining and Tunel staining showed that CVF and AI in the DM model group were significantly higher than those in the control group (all P<0.05). For MaFGF-nlip+ UTMD group, CVF and AI were significantly decreased compared with the DM model group and other intervention groups(all P<0.05). After characterization of the nanoparticles, photoacoustic imaging, magnetic resonance imaging and ultrasound imaging after phasetransition with magnetic-thermo were performed in vitro, and the results were analyzed by a software. \u0000 \u0000 \u0000Results \u0000PFH-HIONS was successfully prepared with uniform particle size of (537.3±24.8)nm. PFH-HIONS could apparently enhance photoacoustic imaging and magnetic resonance imaging in vitro. In an alternating magnetic field, it could significantly increase the temperature, which promotes phasetransion of the PFH to produce microbubbles, thereby enhancing ultrasound imaging. Furthermore, as the concentration increased, the imaging intensity was enhanced, and the differences in imaging intensity between different concentration groups were statistically significant (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000The PFH-HIONS can enhance the multimodal imaging including ultrasound, photoacoustic and magnetic resonance, and it also has obvious magnetic heating performance. After gray and color Doppler ultrasound, the shear wave velocity(SWV) values of target muscle and control muscle in the same side of affected limb were measured by VTIQ technique. Maximum velocity(Vmax) and minimum velocity(Vmin) were measured, average velocity(Vmean) and standard deviation(SD) were calculated, and compared between the two groups. SWV difference and ratio between target muscle and control muscle were calculated, and creatine kinase(CK) was detected in the same day. \u0000 \u0000 \u0000Results \u0000Part of PM patients had abnormal ultrasonographic manifestations of full muscle shape (31.25%), increased or decreased echo of muscle bundles (56.25%), blurred muscle texture (25.00%) and increased blood flow signals (37.50%). There were significant differences in Vmin, SD, and Vmax difference, Vmin difference, Vmean difference, and Vmax ratio, Vmin ratio, and Vmean ratio between PM group and healthy control group (all P<0.01). The ROC curve analysis showed that the diagnostic efficiency of Vmin was lower than those of Vmin difference and Vmean difference, Vmin ratio and Vmean ratio with very close diagnostic efficiencies of the latter four parameters.
