{"title":"Endoscopic Resection of Ampullary Neoplasm","authors":"K Ito","doi":"10.1016/S2212-0971(13)70203-6","DOIUrl":"10.1016/S2212-0971(13)70203-6","url":null,"abstract":"<div><p>The indications for endoscopic papillectomy (EP) are as follows: (1) no infiltration of tumor into the bile duct or pancreatic duct; (2) adenoma or cancer confined to the mucosa; (3) no distant metastasis. EP is performed with a duodenoscope by snaring with a snare, 15–25 mm in size, from the cephalic to the caudal side. Without local injection into the submucosal layer, EP is performed using the 120 W, Endocut® mode, followed by bile and pancreatic ducts stenting after the performance of biliary/pancreatic sphincterotomy. Argon plasma coagulation was used for hemostasis. To reduce colorectal cancer mortality, however, not only is early detection of colorectal cancer required but also adequate depth diagnosis is needed. Current endoscopes provide high-resolution imaging that produces clear, vivid features of detected lesions. In particular, when combined with image enhancement, high-magnification endoscopy can provide a detailed analysis of the morphological architecture of pit pattern and the capillary pattern in a simple and quick manner. Characteristic colonoscopic findings obtained by a combination of conventional colonoscopy, magnifying chromoendoscopy, and narrow-band imaging (NBI) with magnification are useful for determining the depth of invasion of early stage colorectal cancers, an essential factor in selecting a treatment modality. Klatskin tumors are mostly presented by jaundice. In cases of advanced inoperable tumors, a palliative photodynamic therapy (PDT) is possible. The aim of a PDT is a selective destruction of tumor tissue caused by the activation of photosensitizers by laser light. After the application of photosensitizers, they accumulate in the tumor and photochemistry reactions start by the influence of laser light. This leads to tumor necrosis. A common photosensitizer for cholangiocellular carcinomas is photofrin. After a PDT, it is necessary to perform light protection for nearly 2 months.</p><p>The authors present a case of a patient who underwent percutaneous transhepatic cholangioscopy and palliative PDT of a cholangiocellular carcinoma (Klatskin tumor). It is equally effective whether sphincterotomy is performed or not. We demonstrate the case of a young woman with severe liver trauma due to a car crash. Endoscopic retrograde cholangiography revealed multiple intrahepatic biliary leaks that were successfully treated with endoscopic stenting of the common bile duct.
