{"title":"Post-manipulative stroke: a study of two cases","authors":"Neil Osborne BSc, DC, FCC, Haymo Thiel DC, MSc (Ortho), FCCS(C), DipMedEd, FCC, William Kusiar DC","doi":"10.1016/S1466-2108(00)80054-6","DOIUrl":"10.1016/S1466-2108(00)80054-6","url":null,"abstract":"<div><p>Spinal manipulative therapy to the cervical, and especially the upper cervical, spinehas been linked to stroke of the posterior circulation affecting the cerebellum and brain stem.</p><p>Two cases are presented in which chiropractors manipulated patients who presented with neck pain and headaches, resulting in post-manipulative stroke in both cases. Analysis of the cases and review of the relevant literature demonstrate that these scenarios remain both extremely rare and unpredictable. {"title":"The importance of psychosocial aspects in clinical assessment","authors":"Jan Krir DC, MSc, FCC (Ortho)","doi":"10.1016/S1466-2108(00)80026-1","DOIUrl":"10.1016/S1466-2108(00)80026-1","url":null,"abstract":"<div><p>The biopsychosocial concept emphasises human illness rather than disease, Waddell and others have argued for a biopsychosocial approach to low back pain, encouraging patients to develop coping strategies in contrast to the traditional medical model which allows patients to take on a sick role. It has been suggested that patients should be assessed early on for psychosocial ‘yellow flags’ which increase the prognostic risk of chronicity. Assessment may be made using clinical assessments or questionnaires. The advantages and disadvantages of both are discussed as well as the practicality of using both in tandem. Both objective findings and subjective reporting should be investigated, in order to develop a complete overview of the patient's condition. {"title":"Radiological evaluation of a known leg length inequality in Klippel-Trenaunay syndrome","authors":"George Carruthers BSc, DC, FCC","doi":"10.1016/S1466-2108(00)80064-9","DOIUrl":"10.1016/S1466-2108(00)80064-9","url":null,"abstract":"<div><p>Klippel-Trenaunay syndrome (KTS) is a rare congenital disorder which consists of a triad of symptoms. These include a port wine stain of cutaneous capillary malformations, hypertrophy of tissue on the affected side and venous and lymphatic malformations and abnormalities. The musculoskeletal involvement is an important manifestation of this disease which affects normal patients function. Each case is unique with a multitude of variations being perceived in patients with KTS. Radiologically demonstrable bone change is found in the vast majority of cases, 74% of which affects the lower limb. At present there is no known cure for Klippel-Trenaunay syndrome.
