{"title":"Prevalence of obesity in asthma and its relations with asthma severity and control","authors":"Gabriele Carra Forte , Denis Maltz Grutcki , Samuel Millán Menegotto , Rosemary Petrik Pereira , Paulo de Tarso Roth Dalcin","doi":"10.1016/S2255-4823(13)70525-3","DOIUrl":"https://doi.org/10.1016/S2255-4823(13)70525-3","url":null,"abstract":"<div><h3>Objective</h3><p>To determine the prevalence of obesity in asthmatic patients attending at an out-patient clinic, and to investigate its relationships with asthma severity and level of asthma control.</p></div><div><h3>Methods</h3><p>In a cross-sectional study we recruited patients aged 11 years and older with con-firmed asthma diagnosis from the outpatient asthma clinic of Hospital de Clínicas de Porto Alegre, Brazil. They underwent an evaluation by a general questionnaire, an asthma control questionnaire and by pulmonary function tests. Nutritional status was classified by body mass index (BMI).</p></div><div><h3>Results</h3><p>272 patients were included in the study. Mean age was 51.1<!--> <!-->±<!--> <!-->16.5 years and there were 206 (74.9%) female patients. Mean BMI was 27.5<!--> <!-->±<!--> <!-->5.3<!--> <!-->kg/m<sup>2</sup>, and 96 (35.3%) patients were classified as normal weight, 97 (35.7%) as overweight and 79 (29%) as obesity.There was a significant higher proportion of female than male patients (34.3% vs. 13.2%, p<!--> <!-->=<!--> <!-->0.002) in the obesity group. There were no significant differences with respect to asthma control (p<!--> <!-->=<!--> <!-->0.741) and severity classification (p<!--> <!-->=<!--> <!-->0.506). The FEV<sub>1</sub>% predicted was significantly higher in the obese than in the non-obese group (73.7% vs. 67.2%, p<!--> <!-->=<!--> <!-->0.037). Logistic regression analysis identified sex (OR<!--> <!-->=<!--> <!-->3.84, p<!--> <!-->=<!--> <!-->0.002) as an independent factor associated with obesity.</p></div><div><h3>Conclusions</h3><p>This study showed a high prevalence of obesity in asthmatic patients. Obese andnon-obese subjects were similar in regard to asthma severity and level of asthma control. Selected articles were read in entirety, including in the review those that met the established criteria.</p></div><div><h3>Results</h3><p>The prevalence of use among medical students reached 16%, with no gender dif- ference. Most students began using the drug after entering the university, and the reasons cited to justify it are related to enhancing academic performance.</p></div><div><h3>Conclusion</h3><p>There is no evidence in the literature that the use of methylphenidate is benefi- cial in terms of memory or learning. {"title":"Impact of early enteral nutrition therapy on morbimortality reduction in a Pediatric Intensive Care Unit: A systematic review","authors":"Fernanda Marchetto da Silva , Ana Carolina Gouvea Bermudes , Ivie Reis Maneschy , Graziela de Araújo Costa Zanatta , Rubens Feferbaum , Werther Brunow de Carvalho , Uenis Tannuri , Artur Figueiredo Delgado","doi":"10.1016/S2255-4823(13)70520-4","DOIUrl":"https://doi.org/10.1016/S2255-4823(13)70520-4","url":null,"abstract":"<div><h3>Objective</h3><p>To assess the impact of early introduction of enteral nutrition therapy on morbidity and mortality reduction in a pediatric intensive care unit.</p></div><div><h3>Methods</h3><p>A search was performed in the literature of the last 10 years in English, with the target population of individuals aged 1 month to 18 years admitted to pediatric intensive care units in the PubMed, Lilacs and Embase databases using the keywords: Critical Care, Nutritional Support and Nutrition Disorders or Malnutrition.</p></div><div><h3>Results</h3><p>Despite advances in the quality of clinical care, the prevalence of malnutrition in hospitalized children remains unchanged in the last 20 years (15-30%) and has implications for the time of admission, course of illness and morbidity. Malnutrition is common and is often poorly recognized and therefore, untreated. Nutritional therapy is an essential part in the treatment of severely ill pediatric patients who have hypercatabolic protein state, which can be minimized with an effective nutritional treatment plan. In this study, we reviewed publications which have shown that there is still a paucity of randomized and controlled studies with good statistical treatment in relation to enteral nutritional therapy with outcomes related to morbidity and mortality. Beck Depression Inven-tory (BDI) and Migraine Disability Assessment (MIDAS) were used to evaluate depressive symptoms and headache-related disability, respectively. Depression was considered if BDI ≥ 15.</p></div><div><h3>Results</h3><p>A thousand and thirteen young adults were included in the study. A clear relation-ship was observed between headache and depression among the participants. Multivariate analyses demonstrated that nausea or vomiting related to headache and higher headache-related disability scores were independent factors associated with depression.
