Rational and accurate establishment of the diagnosis enables early </span>treatment that reduces the occurrence and severity of acute and chronic complications.</p><p>Conservative treatment of this disease has reached a turning point with the introduction of novel direct oral anticoagulants<span>. According to present guidelines, standard treatment with low molecular heparines such as Fondaparinux<span> and vitamine K antagonists still remain a clinically effective therapeutic option not only in the acute phase, but also in the secondary and long term prophylaxis of the disease. {"title":"New classification of congenital vascular malformations (CVMs)","authors":"Byung-Boong Lee","doi":"10.1016/j.rvm.2015.06.001","DOIUrl":"10.1016/j.rvm.2015.06.001","url":null,"abstract":"<div><p>The congenital vascular malformation<span> (CVM) remains a most difficult and confusing diagnostic and therapeutic clinical entity due to the wide range of clinical presentations, degree of severity, location, unpredictable clinical course and erratic response to treatment with high recurrence by its nature/pathogenesis.</span></p><p>Despite tremendous efforts to control the CVMs through centuries, it has remained a significant challenge to clinicians and the CVM is often viewed as an enigma among the vascular disorders. Besides, old definition of the CVMs based on only the clinical findings and also the classification with name-based eponyms through the last century added more confusion and failed to provide proper information necessary for its advanced management.</p><p>Through last two decades new classifications have been established to meet its mandate for the contemporary management: Hamburg Classification and ISSVA Classification. Both classifications were established based on new advanced knowledge of the anatomy<span>, embryology and pathophysiology<span> involved to the CVM pathogenesis as birth defects<span> affecting entire vascular system: artery, vein, and lymphatics. The role of old classification became obsolete, no longer needed for contemporary management of the CVMs.</span></span></span></p><p>Hamburg Classification is now able to provide safer clinical guide based on embryological characteristics of the CVM lesions depending upon the stage of embryogenesis<span> when the developmental arrest occurs: ‘extratruncular’ type for the lesions originated from the ‘early’ classified with the potential for growth and proliferation to cause the recurrence and subsequent morbidity while ‘truncular’ type for the lesion from the’‘late’ stage with more serious hemodynamic impacts.</span></p><p>ISSVA Classification also provides a clear guideline for the differentiation between the vascular tumor, known as hemangioma, and vascular malformation which was further subgrouped based on the flow characteristics.</p><p>Therefore, correct understanding on these two new classifications are the prerequisite for proper management of every CVM lesion and correct identification and confirmation of each CVM lesion on this embryological subtype is extremely crucial for contemporary management.</p></div>","PeriodicalId":101091,"journal":{"name":"Reviews in Vascular Medicine","volume":"3 3","pages":"Pages 1-5"},"PeriodicalIF":0.0,"publicationDate":"2015-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.rvm.2015.06.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76519200","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Nevertheless, there exists only low evidence for the use of compression therapy in patients with uncomplicated varicose veins even though there has been reported a high recommendation. Especially the duration of time remains unclear. The article gives an overview about the actual literature. Overall it seems that a compression therapy after phlebolgical interventions has a benefit. Nevertheless, the evidence is very low and the postoperative use is based on experience. Therefore, the objective of this is to identify factors associated with AAA growth and suggested a therapeutic regimen for their inhibition.</p></div><div><h3>Material and methods</h3><p>An electronic search of Medline from 1966 to January 2015 in English and adult subjected using the following mesh terms: abdominal aortic aneurysm growth and/or risk factors and/or genetic predisposition, abdominal aortic aneurysm and/or sac expansion and/or risk factors was performed. After application of inclusion criterion 21 articles were retrieved and discussed in this review.</p></div><div><h3>Conclusion</h3><p>Antibiotics, statins, anti-inflammatory agents, selenium, vitamin D<span><span> (25-hydroxyvitamin D), ace-inhibitors, smoking cessation and well-controlled </span>COPD inhibit AAA growth. Eklof","doi":"10.1016/j.rvm.2015.09.004","DOIUrl":"https://doi.org/10.1016/j.rvm.2015.09.004","url":null,"abstract":"<div><h3>Aim</h3><p><span>The aim of this review is to try to explain the controversy by critical analysis of published randomized controlled trials<span> on the value of elastic compression stockings in the treatment of acute </span></span>deep vein thrombosis in prevention of post-thrombotic syndrome.</p></div><div><h3>Methods</h3><p>A research was made through Medline and Embase<span> databases to identify relevant original articles, with the following keywords: post-thrombotic syndrome, deep venous thrombosis, venous thromboembolism, compression stockings, prevention and compliance.</span></p></div><div><h3>Results</h3><p>We identified six randomized controlled trials. Brandjes et al. (1997) <span>[2]</span>, Ginsberg et al. (2001) <span>[3]</span>, Partsch et al. (2004) <span>[4]</span>, Prandoni et al. (2004) <span>[5]</span>, Aschwanden et al. (2008) <span>[6]</span>, Kahn et al. (2014) <span>[11]</span>. Their conclusions and value are analyzed.</p></div><div><h3>Conclusion</h3><p>Prescription of elastic compression stockings for the prevention of post-thrombotic syndrome is now in doubt including long-term compression treatment. Two major issues remain: – We need randomized control trials with long-term outcome to determine if compression is effective to prevent post-thrombotic syndrome according to the precise location of the initial deep venous thrombosis. – Compliance is also a major issue. No update based on robust data has been published previously. The management of this pathology is not codified. We wanted to summarize the current knowledge on diagnostic and therapeutic features of these iatrogenic vascular injuries.</span></span></p></div><div><h3>Methods</h3><p>Medline and EMBASE were queried for publications using the descriptors «artery pseudoaneurysm» and «orthopedic surgery». Articles referenced were read selectively and this review included 121 articles. Patient demographics, clinical presentations, diagnostic methods and treatment were reviewed.</p></div><div><h3>Results</h3><p>A total of 131 cases of arterial PSAs was analyzed. There were 82 men with an average age of 50 years old. The commonest clinical presentation was painful swelling (58%). Most patients underwent an arteriography<span><span> (64%). Arterial injuries<span><span> were frequently caused by knee arthroplasty (25%) and </span>knee arthroscopy (24%). The most common vascular injuries included geniculate (27%) and </span></span>popliteal arteries<span> (26%). Surgical (53%) and endovascular (40%) procedures were the commonest therapeutic modalities. Postoperative courses were uneventful in 129 patients (98%).</span></span></p></div><div><h3>Conclusion</h3><p><span>Geniculate and popliteal artery PSAs are the most frequent vascular complications of orthopedic surgery. They must be evoked when faced with a painful swelling and/or pulsatile mass. Arteriography is an ideal option for confirming the diagnosis. It is often performed with embolization to treat small branches PSAs or with stenting graft for those arising from </span>arterial trunks. The present article presents a review of acute leg swelling according to clinical criteria, and then describes the typical external appearance and the corresponding duplex images with their characteristics. Possible causes for (generally unilateral) acute leg swelling requiring same-day treatment include: thrombosis, which should be diagnosed and treated as quickly as possible; ruptured </span>Baker's cyst; haematoma, e.g. after a contusion or torn muscle. In the case of a post-operative </span></span>lymphoedema (especially after </span>revascularisation<span> surgery), swelling may occur within a few days. Oedemas after radiation or tumour surgery on the other hand, may not appear until months to years after treatment, but then present an acute state. Oedemas associated with fractures may form gradually. Erysipela develops over a few days with hyperthermia and reddening in the leg.
