Therefore, our aim was to explore patterns of hospital admissions diagnosed with anaphylaxis in France from 2012 to 2021, across different age groups, using the national French hospital database and to evaluate potential determinants to propose phenotypes at-risk, with a particular view to severe cases.</p></div><div><h3>Methods</h3><p>This is a descriptive study, using data from the French Program for the Medicalization of Information Systems, from 2012 to 2021. We included all patients hospitalized for anaphylaxis using International Classification of Diseases (ICD)-10 codes listed as a primary diagnosis. Potential risk factors were selected according to corresponding ICD codes, available as secondary data of hospitalization. Severe anaphylaxis (SA) was defined based on the need of admissions in intensive care units. To explore the patterns and characteristics, we employed cluster analysis using the clustering large applications algorithm. Charlson comorbidity index (CCI) was used to evaluate comorbidities.</p></div><div><h3>Results</h3><p>During the 9-year study period, 78,453 patients had 86,654 admissions due to anaphylaxis for all-causes. Most of the cases were non-severe anaphylaxis (93.7%), 5.9% were SA, and 0.4% evolved to fatal anaphylaxis (FA). Rate of admissions for SA was 0.08/100,000/year. Seasonal trend of cases of anaphylaxis due to Hymenoptera venoms were shown. Increased severity and risk of death were significantly higher in the elderly. Food-induced anaphylaxis is more frequent in childhood, but SA occurred at adolescence and early adulthood. The CCI was higher in SA and FA. By applying the clustering algorithm CLARA, we identified 3 clusters distributed according to the severity.</p></div><div><h3>Conclusion</h3><p>More than providing a snap shot of potential risk factors for anaphylaxis in France, we were able to suggest phenotypes of anaphylaxis of different severities and to propose actions based on our findings. Based on new insights from genetic studies and increased knowledge regarding pathophysiology, JAK1 and downstream STAT6 have been characterized as the main therapeutic target pathway in allergies. As a consequence, oral JAK1/2 (baricitinib) and specific JAK1 inhibitors (abrocitinib, upaticinib) have received approval in atopic dermatitis and a dual inhibitor targeting JAKs and SYK, another important tyrosine kinase implicated in allergies, is in development (gusacitinib). Moreover, JAK inhibitors have demonstrated their efficacy in animal models for food allergies and in humans for asthma. In atopic dermatitis, JAK inhibitors are effective as early as 1-week post-administration in reducing eczema disease activity, pruritus, skin pain, and by improving sleep and quality of life. In 2023, a warning label was placed on JAK inhibitors based on an excess risk for serious heart-related events and cancer reported in rheumatoid arthritis (RA) patients undergoing JAK inhibitor treatment. While previous studies assessed trends in searches for some allergic conditions, very few studies reported search patterns for asthma and asthma-related topics.</p></div><div><h3>Methods</h3><p>We took monthly Canadian GTr data for asthma-related keywords such as ‘asthma’, ‘shortness of breath’, ‘wheeze’, ‘asthma emergency’, ‘asthma hospital’, and ‘asthma medication’ between January 1, 2014, and December 31, 2023. We assessed the trend of yearly relative search volume (RSV) for the past 10 years. We also assessed the trend by influenza and wildfire seasons.</p></div><div><h3>Results</h3><p>We observed a marginal increase in searches for some asthma-related topics such as ‘asthma’ ‘shortness of breath’, and ‘wheeze’ in Canada between 2014 and 2023 but not other topics such as ‘asthma emergency’, ‘asthma hospital’, and ‘asthma medication’. More than 80% of respondents had correct answers regarding definition, triggers, risk factors and management of anaphylaxis. The rates of correct answers regarding the storage conditions, the need for an emergency kit including adrenaline in pharmacy authorized to perform vaccination were high (at around 90%). Regarding the provision of AAI in secondary schools and the authorization for school nurses to prescribe AAI, 37 (52%) and 45 (63%) pharmacists had correct answers, respectively.</p><p>Among the 22 community pharmacists, 19 (86%) had at least one AAI in their first emergency kit and 7 (32%) had a trainer for demonstration in their office. Seven (10%) pharmacists had previously experienced anaphylaxis. The main barriers to patient training were: lack of AAI trainer (n = 47, 65%), lack of self-training/knowledge about AAI use (n = 34, 48%). Dietary avoidance of eggs may be very troublesome and cause an important impairment in the quality of life of patients and caregivers. Most patients acquire a natural spontaneous tolerance, and, by the age of 6, 70%–80% of them may eat cooked egg.</p><p>The diagnosis of hen’s egg allergy requires a thorough anamnesis that should be integrated with the results of the skin prick tests with extracts and/or fresh foods, serum specific IgE (sIgE), including molecular components, and, whenever available, basophil activation test (BAT). Nowadays, oral food challenges remain the gold standard for the diagnosis, as no cut-off values for skin prick test or sIgE are considered as universally valid.</p><p>Egg avoidance and education on the proper treatment in case of allergic reactions are essential cornerstones in patients’ management. In those children not outgrowing natural tolerance and suffering from persistent forms, oral immunotherapy is a helpful approach to induce tolerance or, at least, to increase the reactive threshold.
