Anthropometric data, metabolic (glucose, glycated hemoglobin, insulin, insulin resistance, total cholesterol and fractions, triglycerides) measures, fetal biometry, and visceral and subcutaneous adiposity in the binomial (pregnant woman/fetus) were evaluated at the 16<sup>th</sup>, 28<sup>th</sup> and 36<sup>th</sup> gestational weeks by ultrasonography. Pearson's correlation coefficient and multiple linear regression were used, with a significance level of 5%.</p></div><div><h3>Results</h3><p>At the 16<sup>th</sup> week, the mean age of the pregnant women was 26.6<!--> <!-->±<!--> <!-->5.8 years and mean weight was 62.7<!--> <!-->±<!--> <!-->11.5<!--> <!-->kg; 47.0% had normal weight, 28.3% were overweight, 13.3% were underweight, and 11.2% were obese. At 36 weeks, 44.1% had inadequate gestational weight gain, 32.5% had adequate gestational weight gain, and 23.3% had excessive gestational weight gain. Fetal visceral adiposity at week 36 showed a positive correlation with maternal variables: weight (<em>r</em> <!-->=<!--> <!-->0.15) and body mass index (<em>r</em> <!-->=<!--> <!-->0.21) at the 16<sup>th</sup>; with weight (<em>r</em> <!-->=<!--> <!-->0.19), body mass index (<em>r</em> <!-->=<!--> <!-->0.24), and gestational weight gain (<em>r</em> <!-->=<!--> <!-->0.21) at the 28<sup>th</sup>; and with weight (<em>r</em> <!-->=<!--> <!-->0.22), body mass index (<em>r</em> <!-->=<!--> <!-->0.26), and gestational weight gain (<em>r</em> <!-->=<!--> <!-->0.21) at the 36<sup>th</sup> week. After multiple linear regression, adiposity at the 28<sup>th</sup> week remained associated with fetal variables: abdominal circumference (<em>p</em> <!-->&lt;<!--> <!-->0.0001), head circumference (<em>p</em> <!-->=<!--> <!-->0.01), area (<em>p</em> <!-->&lt;<!--> <!-->0.0001), and thigh circumference (<em>p</em> <!-->&lt;<!--> <!-->0.001). At the 36<sup>th</sup> week, adiposity remained associated with the abdominal circumference of the 28<sup>th</sup> (<em>p</em> <!-->=<!--> <!-->0.02) and 36<sup>th</sup> weeks (<em>p</em> <!-->&lt;<!--> <!-->0.001).</p></div><div><h3>Conclusion</h3><p>Adiposity was positively correlated with the measurements of the pregnant woman. Based on perinatal questionnaires, anthropometric measurements of newborns and death certificates were analyzed to obtain the prevalence rate, neonatal mortality, and risk factors (maternal age, income and type of delivery) for very low birth weight.</p></div><div><h3>Results</h3><p>A total of 19,625 newborns were included in the study. In the years 1982, 1993, 2004, and 2015, there were, respectively, 5909, 5232, 4226, and 4258 births. The prevalence of very low birth weight was, respectively, 1.1% (<em>n</em> <!-->=<!--> <!-->64), 0.9% (<em>n</em> <!-->=<!--> <!-->46), 1.4% (<em>n</em> <!-->=<!--> <!-->61), and 1.3% (<em>n</em> <!-->=<!--> <!-->54). There was no statistical evidence of an increasing trend over time (<em>p</em> <!-->=<!--> <!-->0.11). Among the risk factors, family income in the three poorest quintiles was associated with prevalence rates that were approximately twice as high as in the richest quintile (<em>p</em> <!-->=<!--> <!-->0.003). Mortality per 1000 live births for neonates weighing &lt;1500<!--> <!-->g decreased from 688 to 259 per thousand from 1982 to 2015 (<em>p</em> <!-->&lt;<!--> <!-->0.001), but still represented 61% of neonatal deaths in the latter year.</p></div><div><h3>Conclusion</h3><p>Although mortality in very low birth weight decreased by more than 60% in recent years, this group still contributes with more than half of neonatal deaths. Review articles, case reports, book chapters, master's theses, and doctoral dissertations were included.</p></div><div><h3>Summary of findings</h3><p>Congenital heart diseases are among the most common type of birth defects, afflicting up to 1% of the liveborn. Traditionally, the etiology was defined as a multifactorial model, with both genetic and external contribution, and the genetic role was less recognized. The sample was randomly estimated in stages, including children older than 30 days and younger than 10 years of age, of both sexes, excluding syndromic children and those in whom it was impossible to directly measure anthropometry. Weight, height, and arm circumference were measured, as well as the <em>Z</em>‐scores of the anthropometric indices weight‐for‐age, height‐for‐age, weight‐for‐height, body mass index for age, and arm circumference for age, classified according to the reference curves of the World Health Organization. After the tool was applied, its accuracy tests were performed in comparison with the anthropometric data, with the evaluation of sensitivity, specificity, and positive and negative predictive values.</p></div><div><h3>Results</h3><p>A total of 271 children were evaluated, 56.46% males and 41.70% younger than 2 years of age. The prevalence rates of malnutrition, nutritional risk assessed by anthropometric measurements, and nutritional risk assessed by the tool were 12.18%, 33.95%, and 78.60%, respectively. All articles of major relevance were blind‐reviewed by both authors according to the PRISMA statement, looking for possible bias and limitations or the quality of the articles.</p></div><div><h3>Summary of the findings</h3><p>Through the search criterion applied, 417 articles were found, and their abstracts evaluated. A total of 69 papers were thoroughly studied. Articles about propofol use in children are increasing, including in neonates, with the majority being cohort studies and clinical trials in two main scenarios: upper digestive endoscopy and magnetic resonance imaging. A huge list of adverse events has been published, but most articles considered them of low risk.</p></div><div><h3>Conclusions</h3><p>Propofol is a hypnotic drug with a safe profile of efficacy and adverse events. Indeed, when administered by non‐anesthesiologists, quick access to emergency care must be provided, especially in airway events. When triggers were detected in the chart, reviewers investigated the chart in depth to decide whether an event occurred. Consensus about the occurrence of the event was obtained in meeting with a healthcare team. Events were classified by harm category and drugs were classified according to the Anatomical Therapeutic Chemical Classification. Patients who had suffered were compared to those who had not experienced events using the chi‐squared test and the Mann–Whitney <em>U</em> test.</p></div><div><h3>Results</h3><p>A total of 62 adverse events were found, and 18.8% of the patients had at least one event. Adverse events rates were 25.83 per 100 admissions, 20.27 per 1000 patient‐days, 25.94 per 1000 drugs, and 2.12 per 1000 drug doses. All events found were classified as temporary harm, and cardiovascular drugs were most frequently related to events. Groups of patients with and without event were segregated (<em>p</em> <!-->&lt;<!--> <!-->0.05) by the length of stay, number of drugs, and drug doses.</p></div><div><h3>Conclusion</h3><p>The use of triggers demonstrated its utility in a pediatric setting by identifying harm. Adverse events rates were found to be higher than those of previous studies, but the harm rate was lower than other studies. {"title":"Abnormal thymic stromal lymphopoietin expression in the gastrointestinal mucosa of patients with eosinophilic gastroenteritis","authors":"Hongmei Guo,&nbsp;Xiaolan Ji,&nbsp;Guang Yang,&nbsp;Yu Jin","doi":"10.1016/j.jpedp.2019.03.002","DOIUrl":"https://doi.org/10.1016/j.jpedp.2019.03.002","url":null,"abstract":"<div><h3>Objective</h3><p>To investigate the differential expression of the thymic stromal lymphopoietin isoforms, short and long, and discern their biological implications under eosinophilic gastroenteritis.</p></div><div><h3>Methods</h3><p>The expression of thymic stromal lymphopoietin and its two isoforms in tissues was assessed by quantitative RT‐PCR in healthy controls (n<!--> <!-->=<!--> <!-->24) and patients with eosinophilic gastroenteritis (n<!--> <!-->=<!--> <!-->17).</p></div><div><h3>Results</h3><p>Thymic stromal lymphopoietin mRNA was significantly reduced in eosinophilic gastroenteritis when compared with healthy controls (p &lt;<!--> <!-->0.0001). A significantly lower amount of short thymic stromal lymphopoietin mRNA was observed in eosinophilic gastroenteritis when compared with controls (p &lt;<!--> <!-->0.05), while a significantly higher amount of long thymic stromal lymphopoietin mRNA was observed in eosinophilic gastroenteritis when compared with controls (p &lt;<!--> <!-->0.05).
