Based on the capacity, this study allocated 2018 nursing human resource allocation of internal medicine, and compared the production capacity scores of internal medicine capacity, the medical service efficiency, medical service safety, nursing quality, satisfaction of nurses with nursing human resource allocation and patients' satisfaction with nursing care before and after implementation. A total of 267 patients with femtosecond laser assisted cataract surgery from March 2018 to September 2018 were in control group; a total of 319 patients with femtosecond laser assisted cataract surgery from October 2018 to March 2019 were in observation group. Patients of control group carried out routine nursing for cataract day surgery. On the basis of that in control group, observation group implemented feedforward control management. We compared the day's surgery cancellation rate and patient satisfaction of between two groups. \u0000 \u0000 \u0000Results \u0000On the same day, the surgery cancellation rates of patients of observation group and control group were 4.1% (13/319) and 14.2% (38/267) with a statistical difference (χ2=17.615, P<0.01) . The pass rate of the instrument inspection and the quality indicators of the disinfection supply room before and after modification were compared. \u0000 \u0000 \u0000Results \u0000The pass rates of instrument inspection and Bowie-Dictest in the disinfection supply room after modification were better than those before modification (P<0.01) . Semi-structured interview was carried out with the method of phenomenology in qualitative research. Colaizzi 7-step analysis was used in data analysis to conclude themes. \u0000 \u0000 \u0000Results \u0000By interview data analysis, a total of 5 themes that had influences on pain control were concluded: activity-related pain and pain management after removing patient-controlled analgesia (PCA) ; adverse reaction related to pain management; recognition of patients and their family members on pain or pain treatment; information support for pain treatment; influence of non-medical service on patient satisfaction with pain control. \u0000 \u0000 \u0000Conclusions \u0000We should explore the operation mode of acute pain service organization suitable for our country. The conventional group was given conventional nursing intervention while the ERAS group was given ERAS-based nursing intervention. The extubation time, ambulation time, hospital stay, Harris score, quality of life (SF-36) score and hospitalization medical expenses in the two groups were compared. \u0000 \u0000 \u0000Results \u0000The extubation time, ambulation time and hospital stay in the ERAS group were respectively (30.2±10.4) min, (1.2±0.6) d and (10.7±3.2) d, all significantly shorter than those in the conventional group, the differences were statistically significant (t=12.975, 14.349, 9.586; P<0.01) . The Harris score of patients in the ERAS group was (87.2±5.5) points and that in the conventional group was (81.6±6.3) points, the difference between the two groups was statistically significant (t=8.858, P<0.01) . The general health of SF-36 scale of patients in the ERAS group in the ERAS group was (81.35±5.81) points and that in the conventional group was (74.17±5.16) points, the difference between the two groups was statistically significant (t=12.223, P<0.01) . The laboratory fees, bed fees, nursing expenses, medicine expenses and total hospitalization expenses of the ERAS group were all lower than those in the conventional group, the differences were statistically significant (P<0.01) . \u0000 \u0000 \u0000Conclusions \u0000ERAS-based nursing intervention has a good clinical effect on the rehabilitation of patients after hip replacement. All of patients were divided into control group and observation group with the method of random number table, 45 cases in each group. Control group carried out cognitive behavior intervention, while observation group implemented the TENS combined with cognitive behavior intervention. Of which, 5 dental chair units (DCU) were supplied with water after centralized treatment of municipal water and 2 dental chair units were supplied with independent water storage tanks. From the first day of clinical use, water samples of three-way syringes and high-speed dental handpieces and mouthwashes were continuously collected for bacterial culture and the number of colonies was counted. \u0000 \u0000 \u0000Results \u0000A total of 343 water samples were collected. There was a statistically significant difference in the number of colonies in the water samples from the outlet of three-way syringes and high-speed dental handpieces after the waterlines was rinsed for 2 min before treatment (P 0.05) . Waterline washing could significantly reduce the number of colonies in the waterlines (P 0.05) . Patients in the conventional management group received routine discharge guidance, while patients in the web-based self-management group received comprehensive web-based self-management based on routine nursing, including education webpage, WeChat group, and SMS and phone alerts based on the telecommunications network. The incidence of RVT in the two groups was collected in the outpatient clinic 6 months after discharge. The Self-Efficacy Scale and Health Promoting Lifestyle Profile (HPLP-II) were used to evaluate the self-management efficacy and health behavior of the two groups on the day of discharge and 6 months after discharge. \u0000 \u0000 \u0000Results \u0000One cases of RVT occurred in the web-based self-management group (1 case was lost to follow-up) , and 8 cases of RVT occurred in the conventional management group (4 cases were lost to follow-up) . The incidence of RVT in the web-based self-management group was 1.64%, lower than 13.33% in the conventional management group, and the differences were statistically significant (χ2=6.008; P=0.014) . The scores of two dimensions as well as the total self-efficacy scores increased in both groups 6 months after discharge, and those of the web-based self-management group were higher than those of the conventional management group, and the differences were statistically significant (P< 0.01) . Compared with the day of discharge, the scores in different HPLP-II dimensions increased in both groups 6 months post discharge, and the differences were statistically significant (P< 0.01) . {"title":"Application of action research practice in the out-of-hospital health management of patients with rectal cancer stoma","authors":"Yuhua Chen, Guiqing Yu, Bing Li, Zhan Song","doi":"10.3760/CMA.J.CN115682-20190929-03534","DOIUrl":"https://doi.org/10.3760/CMA.J.CN115682-20190929-03534","url":null,"abstract":"Objective \u0000To explore application values of action research practice in the out-of-hospital health management of patients with rectal cancer stoma. \u0000 \u0000 \u0000Methods \u0000According to the convenience sampling method, 46 patients with rectal cancer stoma who were admitted to Nanyang Central Hospital of Henan Province from March 2017 to March 2018 before the implementation of the action research practice were selected as the control group and 46 patients with rectal cancer stoma who were admitted from April 2018 to April 2019 after the implementation of the action research practice were selected as the observation group. The control group was given routine nursing after surgery while the observation group was given action research practice nursing after surgery. Hamilton Anxiety Scale (HAMA) , Hamilton Depression Scale (HAMD) , Exercise of Self-Care Agency Scale (ESCA) and MOS 36-Item Short-Form Health Survey (SF-36) were used before and after implementation to compare the implementation effects between the two groups. \u0000 \u0000 \u0000Results \u0000After nursing, scores of HAMA and HAMD in the observation group were respectively (15.65±10.31) points and (17.41±9.25) points, both lower than those in the control group and the differences were statistically significant (t=2.181, 2.384; P<0.05) . After nursing, the total score of ESCA of the observation group and the control group were respectively (114.96±20.45) points and (100.28±16.80) points and the difference was statistically significant (t=3.762, P<0.05) .
