The compliance with a GFD is dependent on multiple factors, which include, possibly, the quality of life (QoL).</p></div><div><h3>Objective</h3><p>To evaluate the perception of health status and QoL in a sample of Portuguese celiac patients, relating it to the compliance of the GFD.</p></div><div><h3>Material and methods</h3><p>Transversal study which data collection was based on a structured questionnaire prepared to be self-applied and filled in online. The QoL was evaluated through Short-Form health survey 36 (SF-36). Categorical variables were described through proportions and continuous variables were described through mean and standard deviation or median and interquartile range.</p></div><div><h3>Results</h3><p>The study included 195 Portuguese individuals, with a median age of 32 years. About 67% of respondents believed to enjoy very good or good health. The highest mean scores were obtained for the domains “physical functioning” and “role-physical” and the lowest scores for the domains “vitality” and “general health”. Women had significantly lower scores for the domains “bodily pain”, vitality” and “mental health”. There was no association found between the perception of QoL and compliance with the GFD. Participants with more recent diagnosis had lower scores in all domains of the SF-36.</p></div><div><h3>Conclusions</h3><p>The celiac respondents rated their general health status and QoL in better levels than the Portuguese population in general. Some case reports and small series describe the association between celiac disease and others autoimmune disorders, mainly type 1 diabetes mellitus, autoimmune thyroiditis, autoimmune hepatitis and Sjögren syndrome. In the literature, there are 12 reports of an association between celiac disease e immune thrombocytopenic purpura. We report a case of a 24‐years‐old woman with celiac disease since childhood, which developed immune thrombocytopenic purpura with the reintroduction of a gluten diet. {"title":"One day of upper gastrointestinal endoscopy in a southern European country","authors":"Miguel Areia, Mário Dinis-Ribeiro, Sociedade Portuguesa de Endoscopia Digestiva (SPED)","doi":"10.1016/j.jpg.2013.05.004","DOIUrl":"https://doi.org/10.1016/j.jpg.2013.05.004","url":null,"abstract":"<div><h3>Introduction</h3><p>Upper gastrointestinal (UGI) endoscopic outcomes are seldom described.</p></div><div><h3>Objectives</h3><p>To assess UGI endoscopy performance in all Portugal's National Health Service hospitals and assess the prevalence of premalignant gastric lesions.</p></div><div><h3>Methods</h3><p>One randomly assigned day, cross-sectional study of UGI endoscopies.</p></div><div><h3>Results</h3><p>28% of the 43 hospitals invited actually participated in the study, reporting a total of 123 UGI endoscopies. Exams were conducted on an outpatient basis in 84% of cases and 78% required no sedation. The commonest indications were presence or suspicion of GI bleeding (20%), abdominal pain or dyspepsia (18%) or reflux (12%). Histological diagnosis of atrophy was found in 19% of cases (95% CI 8–30%), extensive atrophy or intestinal metaplasia in corpus in 15% (5–25%) and positivity for <em>Helicobacter pylori</em> in 38% (23–53%). When comparing first-time vs. repeat UGI endoscopies, no differences were found in atrophy (22% vs. 14%, <em>p</em> <!-->=<!--> <!-->0.49) and <em>H. pylori</em> (44% vs. 30%, <em>p</em> <!-->=<!--> <!-->0.36) nor did age<!--> <!--><<!--> <!-->vs.<!--> <!-->≥<!--> <!-->50 years was relevant (11% vs. 21%, <em>p</em> <!-->=<!--> <!-->0.51 and 63% vs. 31%, <em>p</em> <!-->=<!--> <!-->0.10, respectively).</p></div><div><h3>Conclusions</h3><p>Most UGI endoscopies carried out in Portugal are safely performed on an outpatient basis without anaesthesia and 15% of patients have extensive atrophy or intestinal metaplasia in the corpus that should be scheduled for endoscopic surveillance according to recent guidelines.
