It develops in intestinal intra-epithelial T-cells and is commonly associated with celiac disease (CD).</p><p>The authors report a patient presenting with abdominal discomfort, weight loss, intolerance to gluten, positive gliadin antibodies and mesenteric lymph node enlargement. The aim of this study was to evaluate the role of the wireless capsule enteroscopy (WCE) in the management of obscure digestive bleeding (ODB) in patients with chronic renal failure (CRF) undergoing haemodialysis, in comparison to a group of patients with creatinine clearance (CrCl) >60 mL/min.</p></div><div><h3>Material and methods</h3><p>This prospective cohort study included 90 patients with ODB: 12 patients with CrCl <30mL/min undergoing haemodialysis and 78 patients with CrCl >60 mL/min, all referred for WCE in a single institution in a 12 month-period. Estimated CrCL using Cockcroft-Gault formula was determined in the day of WCE for all patients. Patient's demographic data and clinical characteristics, WCE findings and outcome (including specific therapy, transfusion requirements and hospital admissions due to recurrent bleeding or severe anemia) were assessed.</p></div><div><h3>Results</h3><p>Patients’ age and the clinical presentation of ODB (occult/overt) were similar in the 2 groups. There were no significant differences in gastric emptying and small bowel transit times, or in the ratio of incomplete/inconclusive exams. In patients undergoing haemodialysis, there were fewer normal WCE procedures (17% versus 46%, respectively, p=0,031) and a higher prevalence of small bowel angiodysplasias (58% versus 23%, respectively, p=0,011). By logistic regression analysis, CRF in haemodyalisis was found to be the only predictive factor for angiodysplasias in WCE (p=0,017, 95% CI 0,061-0,758). Endo-SPONGE (B-Braun Medical<sup>®</sup>), an endoluminal vacuum therapy minimally invasive, was shown to be an effective alternative in the treatment of colorectal anastomotic leakage with local infection in the minor pelvis. However, stent migration is still a common complication. We report a case of a 66 years old that had an esophageal inoperable cancer of distal esophagus. The patient had total dysphagia and a significant weight lost. A covert self-expanding metal stent was placed and he stayed well during almost 6 months. After that period, the patient restarted having dysphagia and went back to the hospital. Chest radiography was performed and no image of the stent was observed. An abdominal radiography was made and observed the image of the stent in the right iliac fossa. The patient did not complain a defecation problems or abdominal pain. Another covert stent was placed in the distal esophagus. We present the case of a 64-year-old male with steatorrhea and weight loss (17% total body weight). Colonoscopy detected presence of oil droplets in stools. Esophagogastroduodenoscopy revealed a fistulous orifice with mucous drainage in the duodenal bulb and a bulging ampulla of Vater, possibly by external compression; Abdominal CT showed the presence of a cystic lesion in the pancreatic head, 5.8<!--> <!-->cm in diameter, significant dilation of wirsung and slight dilation of common bile duct. EUS revealed a multicystic lesion with irregular contours in the pancreatic head, suggestive of IPMN. EUS guided fine needle citologic examination confirmed IPMN. Abdominal MRI detected fistulization to the duodenum and stomach. Pancreatoduodenectomy was performed and a pathological diagnosis of mucinous adenocarcinoma in IPMN was obtained. There has been no evidence of recurrence during 14 months of follow-up. Most patients diagnosed with IPMN are assymptomatic, steatorrhea is the less frequent form of presentation.
