It is desirable that endoscopists have knowledge of the basic pharmacology of antithrombotic agents and their indications, as well as the risk of thrombosis after discontinuation of therapy and the risk of bleeding due to endoscopic procedures. The importance of this information lies in optimising the outcomes of thromboembolism and bleeding. The aim of the study was to review the indications for oesophago-gastro-duodenoscopy, adherence to the ASGE guidelines in Mexico Hospital Center, and their usefulness.</p></div><div><h3>Material and methods</h3><p>A retrospective cohort study was conducted on all cases of patients who underwent an oesophago-gastro-duodenoscopy for diagnostic purposes in the Gastrointestinal Endoscopy service of a hospital of the center of Mexico during January 2006 to December 2010. Demographic data, speciality of the referral physician and the, description of the procedure were recorded, along with the endoscopic diagnosis. Sensitivity, specificity, predictive values and likelihood ratios were calculated, using a standard endoscopic diagnosis as a reference.</p></div><div><h3>Results</h3><p>A total of 3,467 oesophago-gastro-duodenoscopies were reviewed. According to the ASGE guidelines, only 36.6% of them were considered adequate, with 6.4% of the all endoscopies being requested by gastroenterologists. The main reasons for referral were oesophageal reflux disease, gastrointestinal bleeding, and dyspepsia. More than half (54.8%) of the referrals with inadequate indication showed some relevant endoscopic finding. The sensitivity and specificity were 37% and 64%, respectively, as a diagnostic test for the referral diagnosis. In multiple regression analysis, the factors most associated with a relevant diagnosis, were age (greater than 40 years), be referred for specialist medical and referral diagnosis.</p></div><div><h3>Conclusions</h3><p>The adherence to ASGE guidelines was low, probably due to the open access system. Its aetiology is unknown. At present it is considered to be pre-malignant. The case is presented of a 27-year-old patient who was admitted for abdominal pain, loss of weight, and vomiting. This disease is diagnosed using endoscopy and biopsy of stomach tissue. Although gastric varices bleed less frequently than oesophageal varices, when this occurs, it is associated with a 45% mortality rate. Obstructive methods using with tissue adhesives are the treatment of choice for gastric varices.</p></div><div><h3>Objectives</h3><p>To determine the efficacy and safety of cyanoacrylate injection in gastric varices and its impact on survival.</p></div><div><h3>Material and Methods</h3><p>This was a retrospective analysis of patients with gastric varices treated with cyanoacrylate injection between January 2005 and May 2013 at the Endoscopy Unit of Hospital General de Mexico.</p></div><div><h3>Results</h3><p>A total of 333 patients were included, with male predominance (55%) and a mean age of 53.44<!--> <!-->±<!--> <!-->14.9 years. Alcohol was the most common cause of cirrhosis (46.5%), followed by chronic viral hepatitis (24.6%) and cryptogenic (28.8%). The Child-Pugh grade was A in 66 patients (19.81%), B in 126 (37.83%) and C in 141 (42.34%). The mean cyanoacrylate sessions until eradication were 1.83<!--> <!-->±<!--> <!-->0.574, with a mean 2.07<!--> <!-->±<!--> <!-->0.703 cyanoacrylate doses per session. Complications observed included splenic vein thrombosis (0.3%) and pleuritis (0.3%). Both were managed conservatively. Variceal eradication was achieved in 309 patients (92.8%). The survival rate was high, with 281 patients remaining alive (84.4%). Causes of death were associated to liver failure (6.3%) and infections (2.1%).
