Because of improved medical and surgical treatments, they are less indicated than they used to be for poliomyelitis and paediatric orthopaedic foot wearing, but new indications emerge, such as the diabetic foot. The improvement of techniques and materials have completely changed the old image of these orthopaedic devices that are no longer heavy and uncomfortable but much more functional and aesthetic. Prescription is based on a precise diagnosis of the pathology and its functional consequences, and on a good relationship between the patient, the practitioner and the pedorthist. Orthopaedic shoes provide multiple solutions for correcting, stabilising or unloading the foot, but the selection of this or that kind remains based on personal experience rather than on scientific evidence. Limping could be classified in two groups: a painful or antalgic gait is caracterized by a decreased stance phase on the affected side. The trunk shift gait or Trendelenburg gait is caused by a decreased strength of the hip abductor muscles or by hip joint instability. Other modifications of gait patterns could be caused by neurologic pathologies that should not be misdiagnosed. Excepted when very severe, limb-length discrepancy is not causing limping in children. Excepted traumatism, most of the etiologies are specifically pediatric. Some mandate an urgent diagnosis and treatment as osteoarticular infections, or bone tumors, because delay in treatment could leave severe after-effects. A precise physical examination should dictate the proper imaging and biologic studies. In children, the diagnosis of tendonitis should not be kept without radiographic examination. One should remember that some hip pathologies could present as knee pain. Some pathologies never occur before a given age (for instance no hip transient synovitis before age 3), when other diagnoses should be systematically sought after (slipped capital epiphysis in teenagers). In association with other investigation techniques, it helps establishing both diagnosis and adequate therapy, and to evaluate subsequent improvement. Compensating insufficiently reliable subjective joint evaluation by objective and quantified amplitude assessments is mandatory. Numerous techniques exist and each therapist has to select the most adequate method, depending on both the pathological problem and the available means. Clinical symptoms are contingent on response patterns of the immune system. Leprosy has a preference for attacking the skin and certain peripheral nerves. «Leprotic foot » is the result of lesions in the nervous system. Clinical symptoms are stereotypic in that they stem from a common cause. But different stages of evolution can affect the appearance and the seriousness of the disease. Hypoaesthesia in the sole of the foot leads to chronic foot ulcers. Palaralysis of leg and foot muscles can lead to unnatural foot positions which increase the risk of ulcers. Nervous acropathy, infections due to <em>Mycobacterium leprae</em> bacillus and ulcers infected by other bacteria, associate to break down bone and joint tissue. There development leads to diseaded induced mutilations. Deterioration can continue independently of recovery from the leprotic infection when nerve damage is irreversible. Disease induced mutilation due to leprosy concerns more than three millions people in the world (with half of this number afflicted with foot lesions) and constitutes a serious physical, psychological and social handicap. Nowadays the losely-linked prosthesis are the most frequently fitted ; these prosthesis have a good range of motion but they need a good quality rotator cuff. Rehabilitation is fundamental to achieve a good functional result. It begins during the immobilization phase to obtain a satisfactory passive range of motion, especially through scapulo-humeral mobilization. The following phase, at progressive splint removal, is centred on active mobilization, made easier in the hot bath. Finally, in the last phase, active exercises provide muscle tonicity and concrete functional gestures integrated by the occupational therapist.
