Among them, the important bodies with motor competence such as the cortex, the extra-pyramidal system, the cerebellum and the spinal cord. To distinguish central motricity from peripheral motricity, the physiology of the neuromuscular system is described. In terms of hodological structures and organization, a dynamic view of both the unitary neuronal physiology and cortical-sub-cortical networks is presented, following an ascending Jacksonian order from the spinal cord to the cortex. A basal ancestral species, <em>Australopithecus afarensis</em>, exhibits a well-preserved postcranium that permits reconstruction of important events in the evolution of our locomotor skeleton. When compared to those of living apes and humans, it provides insights into the origin and design of the modern human frame. Evolutionary aspects of the human hip and thigh are reviewed, including the unsusual corticotrabecular structure of the human proximal femur, and our markedly elongated lower limb. Evaluation has become an essential point in terms of health policy, and a daily concern for those professionals responsible for developing the quality of care. The concept of evaluation has evolved also, and has a specific role separate from the research field, which should be more developed. Research results are used to establish shared referentials. Human relationships are present throughout the rehabilitation therapy; therefore, evaluation is based on criteria that take into account both subjective and objective aspects of the patient's, therapists', and institution's projects. The political will of developing evaluation within the health system has resulted, in qualitative terms, in the creation of the ANAES (Agence nationale d'accréditation et d'évaluation en santé), a national agency for health evaluation, and in quantitative terms in the creation of the PMSI (Programme de médicalisation du système d'information) a program for the development of a medical information system; social services as well as patients and practitioners participate in evaluation practices. It describes the functional anatomy and biomechanics of the knee including normal movement, and factors which may destabilize the knee, and predispose it to injury. The controversial “Q” angle is discussed, as is its relationship to patellofemoral pain. The importance of balance musculature to the integrity of the knee is emphasized as are the potential consequences of weak, or overly tight, muscles. Examination of the knee including patient history, standard orthopedic testing, observation of posture and gait, and palpation are summarized. Treatment protocols for the acute phase, recovery, and maintenance phases of treatment are described.
