In comparison with patients using alternative immunoglobulin heavy-chain variable (IGHV) genes, patients with IgHV1-69 CLLs more often presented at advanced stage, lacked somatic hypermutation (unmutated cases, 87% vs. 35%; P = .00001), and expressed unfavorable biologic characteristics. In 12 patients (26%), common amino acid motifs within the heavy-chain third complementarity-determining region were identified, allowing assignment to previously reported stereotyped subsets. In our study, treatment-free survival of patients with unmutated IGVH1-69 did not differ significantly from that of patients expressing unmutated alternative IGHV genes. Recent data have suggested an increasing incidence in immunocompetent individuals, with a peak of incidence between 60 and 70 years of age. Patients with PCNSL present mostly with symptoms of increased intracranial pressure. The clinical management of these patients remains controversial, and the optimal treatment for patients with PCNSL has not yet been defined. Surgery, even if macroscopically radical, does not improve survival because of the multifocal and infiltrative nature of PCNSL; furthermore, the deep location of most of these tumors makes patients susceptible to serious and irreversible neurologic sequelae. Corticosteroids have a specific role in the treatment of patients with PCNSL, whose disease is sensitive to them as a chemotherapeutic agent. PCNSL is an extremely radiation-sensitive neoplasm; whole-brain radiation therapy plus corticosteroids was the first modality of treatment for patients with this neoplasm until 10 years ago, with a low cure rate and a high local recurrence rate. PCNSL is also a chemosensitive neoplasm; while the optimal choice, sequence, and combination of appropriate agents for efficacious treatment of patients with PCNSL has yet to be determined. An essential component of therapy must include an adequate drug delivery behind a normal blood-brain barrier. Methotrexate is the agent with the most proven activity in PCNSL. Kyphoplasty has also increasingly been used to treat vertebral osteolyses caused by multiple myeloma (MM).</p></div><div><h3>Patients and Methods</h3><p>In our cohort of 76 patients with MM with a total of 190 vertebral fractures treated with kyphoplasty, we performed a 30-day postoperative analysis of cement leakage, neurologic symptoms, pulmonary embolism, and infections.</p></div><div><h3>Results</h3><p>Painful osteolytic or fractured vertebrae or even imminent vertebral instability caused by osteolyses were seen as indications for kyphoplasty. Jesús Nambo ,&nbsp;Sergio Cleto ,&nbsp;Natividad Neri ,&nbsp;Judith Huerta-Guzmán","doi":"10.3816/CLM.2009.n.075","DOIUrl":"10.3816/CLM.2009.n.075","url":null,"abstract":"<div><h3>Background</h3><p>Treatment of primary testicular lymphoma (PTL) remains unsatisfactory even in patients with good prognosis, as &lt; 30% of patients are alive at 3 years.</p></div><div><h3>Patients and Methods</h3><p>We began a phase II study to assess efficacy and toxicity of a dose-dense cyclophosphamide/epirubicin/vincristine/prednisone (CEOP14) regimen with rituximab (CEOP14R) in 38 previously untreated patients with PTL with early-stage (I or II) and low-risk disease, followed by adjuvant radiation therapy and central nervous system prophylaxis.</p></div><div><h3>Results</h3><p>Complete response was 86% (similar to historical controls), but improvement in outcome was observed; with actuarial curves at 5 years, event-free survival was 70%, and overall survival was 66%. Toxicity was mild, and the regimen was well tolerated.</p></div><div><h3>Conclusion</h3><p>The addition of rituximab to dose-dense chemotherapy improves outcome in this setting of patients who previously had been considered to have the poorest prognosis. The tumor manifests as patches, plaques, tumors, or erythroderma and can secondarily involve lymph nodes, peripheral blood, and visceral organs. In advanced tumor stage, chemotherapy is a second-line approach, which is generally not considered curative. Initially, most patients profit from this treatment, but observed remissions usually do not exceed several months. Because of possible immunosuppressive effects in vulnerable patients, the overall benefit of chemotherapy itself is not unequivocal in cutaneous T-cell lymphoma. We report 3 patients whose tumor-stage mycosis fungoides was not sufficiently controlled by several preceding systemic therapies, including liposome-encapsulated doxorubicin. Multiple bone amyloidomas are rare as the initial presenting feature of myeloma. Solitary bone amyloidomas share common features with those of patients who have solitary plasmacytomas and progression to disseminated myeloma is common. We report a case of an elderly man who presented with extensive amyloid deposition in multiple plasmacytoma sites as well as evidence of amyloid in a fat pad aspirate but with none of the usual organ damage associated with systemic amyloidosis. This presentation is similar to a subset of patients said to have macrofocal myeloma. These patients are typically aged &lt; 40 years, have no bone marrow involvement, and have a good prognosis. A minority of patients have clinical and biologic features associated with a very high risk of relapse. For the remaining patients, no clear prognostic factors can be identified at diagnosis. The degree of treatment response is likely to be an informative predictor of outcome for these patients. Modern assays to detect AML cells that are undetectable by conventional morphologic techniques, ie, minimal residual disease (MRD), can potentially improve measurements of treatment response. It is plausible that modifications to treatment based on the results of these assays will improve clinical management and ultimately increase cure rates. Established MRD assays for AML are based on either polymerase chain reaction amplification of genetic abnormalities or flow cytometric detection of abnormal immunophenotypes. Residual disease and treatment response can be measured by these assays in a manner that is much more sensitive and objective than that afforded by conventional morphologic examination. The expanding use of MRD testing is beginning to change the definitions of treatment response and of remission. With current regimens, the majority of patients achieve a complete remission, and their survival curves are similar to those for appropriate age-matched individuals without the disease. At the same time, new technologies are allowing better understanding of the molecular mechanisms responsible for the pathogenesis of this and other indolent lymphoid neoplasms. Several studies using modern techniques with different sensitivities have demonstrated the persistence of minimal residual disease (MRD) after therapy with nucleoside analogues in majority of patients. However, it is not clear whether such MRD would invariably lead to leukemia recurrence or what level of MRD can predict relapse. The role of monoclonal antibodies, naked or conjugated with toxins, in the management of HCL and their ability to eradicate MRD is under investigation.
