The prevalence of type<!--> <!-->2 diabetes in Spain is 13.8% (6% undiagnosed), while the incidence of type<!--> <!-->1 diabetes is 12-14 cases per100,000 inhabitants/year. DM consumes 8.2% of total health expenditure in Spain.</p><p>DM is a paradigmatic example of chronic disease that affects all ages, and requires continuous medical care to: (i)<!--> <!-->optimize metabolic control; (ii)<!--> <!-->prevent and treat acute complications and associated chronic complications; (iii)<!--> <!-->control cardiovascular risk factors (CVRF); and (iv)<!--> <!-->provide therapeutic education related to self-management for patients, relatives and caregivers. For quality of care, scientific societies advocate quality standards of therapeutic patient education programs, as well as interdisciplinary team work in which the diabetes expert/specialist nurse has an important role, especially that of educator.</p><p>Both the curricular and professional profiles of diabetes expert/specialist nurses in different countries are described. The Spanish Diabetes Society (SED) supports the academic profile of diabetes expert/specialist nurses via Master or post-graduate degrees and the need for continuing education. The competence profile is based on 5 domains: (I) Pathophysiology and Epidemiology of Diabetes Clinical Guidelines. (II) Culturally competent care throughout life. (III) Teaching and learning skills. (IV) Education for self-management. However, there is emerging evidence to suggest that retinal </span></span>neurodegeneration is an early event in the pathogenesis of DR, which participates in the development of microvascular abnormalities. Therefore, the study of the underlying mechanisms leading to neurodegeneration and the identification of the mediators linking neurodegeneration and microangiopathy will be essential for the development of new therapeutic strategies in the early stages of DR. In this review the mechanisms involved in neurodegeneration, as well as the link between neurodegeneration and microangiopathy have been updated. Transition to adult units takes place during adolescence in an especially critical period of life with changes, both in psychological and physiological aspects that increase the risk of onset and progression of chronic complications related to T1DM.</span></p><p>Adverse outcomes that may affect the health of these patients can appear if transition is not done properly. Previous studies have shown that planned and structured transition programs are required, including the participation of the individual, the family, and the health service. The best time to make the transition is when they are mature enough to be almost capable of managing their T1DM. The majority of patients do not reach this stage before the age of 16-18<!--> <!-->years.</p><p>There should be coordination between professionals of paediatric and adult health care in the planning of this transition. Group education programs, the use of new technologies, and the approach to psychosocial aspects are suggested in order to improve adherence and follow-up during this period.
