Lei, Liangping Li","doi":"10.3760/CMA.J.ISSN.0254-1432.2019.05.010","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.0254-1432.2019.05.010","url":null,"abstract":"Objective \u0000To investigate early kidney injury in the patients with non-alcoholic fatty liver disease (NAFLD) without hypertension, diabetes and history of kidney diseases, as so to provide evidence for preventing early kidney injury in patients with NAFLD. \u0000 \u0000 \u0000Methods \u0000From December 2014 to January 2016, 169 subjects visiting Sichuan Provincial People′s Hospital were recruited. Among them, 104 cases were in NAFLD group, 31 cases were in simple obesity group (overweight or obesity), and 34 subjects were in the healthy control group. The general data, biochemical indexes, metabolic indexes, biochemical indexes of kidney, and early kidney injury makers, including serum β2-microglobulin, urinary albumin and creatinine ratio (ACR) and estimated glomerular filtration rate (eGFR), were detected. Least significant difference-t test, chi-square test and Spearman correlation analysis were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000Compared with simple obesity group, alanine aminotransferase (ALT), aspartate aminotransferase (AST), γ-glutamyl transpeptadase (GGT), fasting insulin level, homeostasis model assessment-insulin resistance (HOMA-IR), serum creatinine, ACR and β2 microglobulin were higher in NAFLD group ((21.13±8.14) U/L vs. (66.20±44.34) U/L, (24.80±9.57) U/L vs. (49.78±25.41) U/L, (19.26±7.88) U/L vs. (66.53±56.34) U/L, (7.03±1.52) mU/L vs. (9.55±5.41) mU/L, 1.22±0.38 vs. 2.23±2.01, (62.90±10.01) μmol/L vs. (71.75±10.80) μmol/L, (4.41±1.16) μg/mg vs. (13.76±9.56) μg/mg, (1.46±0.26) mg/L vs. (2.01±0.53) mg/L); however the eGFR was lower (112.46±11.90) mL·min-1·(1.73 m2)-1 vs. (101.09±17.17) mL·min-1·(1.73 m2)-1). The differences were statistically significant (t=9.825, 8.250, 8.288, 4.229, 4.121, 4.007, 9.732, 7.792 and -3.443, all P 0.05). Compared with healthy control group, the renal injury indexes serum creatinine, ACR and β2 microglobulin of NAFLD group were higher((58.78±7.77) μmol/L vs. (71.75±10.80) μmol/L, (1.01±0.32) μg/mg vs. (13.76±9.56) μg/mg, (1.12±0.15) mg/L vs. (2.01±0.53) mg/L), and the eGFR was lower ((115.10±12.59) mL·min-1·(1.73 m2)-1 vs. (101.09±17.17) mL·min-1·(1.73 m2)-1). The differences were statistically significant (t=7.621, 13.591, 15.126 and -5.120, all P<0.01). Compared with healthy control group, the renal injury indexes ACR and β2 microglobulin of simple obesity group were higher ((1.01±0.32) μg/mg vs. (4.41±1.16) μg/mg, (1.12±0.15) mg/L vs. (1.46±0.26) mg/L), and the differences were statistically significant (t=9.732 and 7.792, both P<0.01). According to therapy the patients were divided into two groups: the radiotherapy group (30 cases) treated with radioactive 125I seeds alone and the combination treatment group (27 cases) treated with combination of radioactive 125I seeds and cetuximab. The tumor size, pain relief and adverse reactions were observed in both groups. Chi-square test were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000After treatment for six months, the total efficacy rate and local control rate of combination treatment group were 54.2% (13/24) and 87.5% (21/24), respectively; and which were higher than those of radiotherapy group (17.9%, 5/28 and 39.3%, 11/28), and the differences were statistically significant (χ2=15.01 and 2.55, both P 0.05). After treatment for six months, the rates of adverse reactions of radiotherapy group and combination treatment group were 46.7% (14/30) and 63.0% (17/27), respectively, there was no statistically significant difference between the two groups (P>0.05). According to inclusion and exclusion criteria, a total of 3 312 individuals were enrolled and divided into NAFLD group (913 cases) and non-NAFLD group (2 399 cases). The serum lipid levels were compared between the two groups. Receiver operating characteristic (ROC) curve was performed to evaluate the value of HDL-C in the diagnosis of NAFLD. The binary logistic regression models were established based on HDL-C level.The differences in liver function indexes were compared among the research objects with different HDL-C levels.T test and Mann-Whitney U test were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000The serum levels of total cholesterol, triglyceride and low density lipoprotein-cholesterol (LDL-C) of NAFLD group were all higher than those of non-NAFLD group ((5.24±0.92) mmol/L vs.(4.98±0.92) mmol/L, (1.95±1.41) mmol/L vs.(1.13±0.68) mmol/L, (3.31±0.84) mmol/L vs.(3.09±0.84) mmol/L), and the differences were statistically significant (t=-7.29, -22.38 and -6.84, all P<0.01). However the serum HDL-C level of NAFLD group was lower than that of non-NAFLD group((1.30±0.33) mmol/L vs.(1.64±0.40) mmol/L), and the difference was statistically significant (t=24.93, P<0.01). The incidence of hypercholesterolemia, hypertriglyceridemia, hypo-high-density lipoprotein cholesterolemia and hyper-low-density lipoprotein cholesterolemia of NAFLD group was 48.0%(438/913), 44.8%(409/913), 31.0%(283/913) and 82.8%(756/913), respectively, which were significantly higher than that of non-NAFLD group (36.8%, 882/2 399; 13.2%, 317/2 399; 10.5%, 251/2 399; 71.8%, 1 723/2 399), and the differences were statistically significant (χ2=34.65, 385.43, 206.18 and 42.37, all P<0.01). Using the cut-off values of HDL-C≤1.66 mmol/L in female and≤1.33 mmol/L in male, the area under curve (AUC) values for NAFLD diagnosis were 0.720(95% confidence interval (CI) 0.693 to 0.747) and 0.708 (95%CI 0.679 to 0.737), respectively, the sensitivity was 79.1% and 76.6%, and the specificity was 55.0%and 54.6%. The results of binary logistic regression models based on HDL-C level indicated that prevalence of NAFLD in female with low HDL-C was 4.584 times (95%CI 3.530 to 5.940, P<0.01) higher than that in female with high HDL-C; the prevalence of NAFLD in male with low HDL-C was 3.898 times (95%CI 3.020 to 5.030, P<0.01) higher than that of male with high HDL-C. Clinical data of IBD patients with diarrhea were collected. Clostridium difficile toxins A and B of stool specimens of all included patients were detected with enzyme immunoassay. T test, chi-square test and logistic regression analysis were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000Among 169 IBD patients with diarrhea, there were 137 adults and 32 children. The positive rate of CDI of IBD patients with diarrhea (9.5%, 16/169) was higher than that of non-IBD patients with diarrhea (1.1%, 2/184), and the difference was statistically significant (χ2=12.785, P 0.05). \u0000 \u0000 \u0000Conclusions \u0000The incidence of CDI in IBD patients with diarrhea increases. The risk factors include history of previous hospitalization before admission and antibiotic usage. Blood samples and colonic mucosa specimens of UC group and healthy control group were collected. DNA methyltransferase (DNMT) activity, methylation level of CpG locus in IL-6 promoter region and expression level of IL-6 were detected. Chi-square test and student′s t test were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000The methylation level and IL-6 expression level examination were completed in 24 and 30 cases of UC group, respectively; which were detected in 21 and 20 cases of healthy control group, respectively. The activity of DNMT of UC group was significantly lower than that of healthy control group ((16.48±6.17) OD·h-1·mg-1 vs. (62.48±33.88) OD·h-1·mg-1), and the difference was statistically significant (t=3.707, P<0.05). The ratios of methylation, partial methylation and non-methylation of UC group were 31.2% (15/48), 50.0% (24/48) and 18.8% (9/48), respectively, and those of healthy control group were 58.3%(35/60), 21.7%(13/60) and 20.0%(12/60), and the difference between the two groups was statistically significant (χ2=10.495, P<0.05). The positive expression rate of IL-6 in intestinal mucosa of UC group was higher than that of healthy control group (100.0%, 21/21 vs. 25.0%, 5/20), and the difference was statistically significant (χ2=24.837, P<0.05). UC mice models were established by dextran sulfate sodium (DSS). Forty-eight BALB/c mice were divided into 2.5% DSS group, 5.0% DSS group and 7.5% DSS group and control group. The colon tissues of UC patients and UC mice models were pathologically scored. The expression of NLRP3, cysteinyl aspartate specific proteinase 1(caspase-1) and apoptosis-associated speck-like protein containing CARD (ASC) at mRNA level in colon tissues of UC patients and UC mice models were determined by real time fluorescence quantitative polymerase chain reaction (PCR). The expression of NLRP3, caspase-1 and ASC at protein level in colon tissues of UC patients and UC mice models were detected by Western blotting. One-way analysis of variance and SNK-t test were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000The histopathological scores of colon tissues of UC patients at remission phase, at mild activity phase, at moderate activity phase, at severe activity phase and healthy controls were 2.37±0.46, 4.84±1.29, 6.82±0.96, 9.42±1.13 and 1.23±0.55, respectively; the differences were statistically significant (F=67.68, P<0.01). The higher the degree of inflammation, the higher the pathological score, and the differences were statistically significant (all P<0.05). The pathological scores of colon tissues of mice in the 2.5% DSS group, 5.0% DSS group and 7.5% DSS group were 4.54±0.74, 6.02±1.00 and 8.43±1.46, respectively; the higher the dose of DSS, the higher the pathological score, and the differences were statistically significant (all P<0.05). The expression of NLRP3 at mRNA level of UC at remission phase, mild activity phase, moderate activity phase, severe activity phase and healthy controls were 1.15±0.10, 1.49±0.13, 2.00±0.25, 2.05±0.33 and 0.61±0.09, respectively; the expression of caspase-1 at mRNA level were 1.13±0.08, 1.51±0.19, 2.10±0.23, 2.88±0.33 and 0.61±0.11, respectively; the expression of ASC at mRNA level were 1.12±0.08, 1.88±0.33, 2.53±0.22, 3.20±0.24 and 0.59±0.12, respectively; the differences between groups were statistically significant (F=108.43, 63.25 and 105.25, all P<0.01). Chen","doi":"10.3760/CMA.J.ISSN.0254-1432.2019.04.001","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.0254-1432.2019.04.001","url":null,"abstract":"Objective \u0000To analyze the relationship between body type, age, gender, esophageal motility function, lower esophageal sphincter pressure (LESP) and clinical classification and type of reflux contents of patients with gastroesophageal reflux disease (GERD). \u0000 \u0000 \u0000Methods \u0000From September 2015 to July 2016, at Beijing Tongren Hospital of China Capital Medical University, the results of 24-hour esophageal impedance-pH monitoring of 141 patients with GERD were retrospectively analyzed. The differences of reflux contents were compared in patients with GERD among different body type, age, gender, esophageal motility function, LESP and clinical classification. Mann-Whitney test was performed for comparison between groups. \u0000 \u0000 \u0000Results \u0000Acid reflux was more common in obese patients (body mass index more than 23.9 kg/m2) compared with patients with normal body type (body mass index from 18.5 to 23.9 kg/m2) (18.5, 7.0 to 45.3 vs. 10.0, 2.0 to 32.0), and the difference was statistically significant (Z=-2.320, P=0.020). Patients under 65 years old had more numbers of weak acid reflux, non-acid reflux and gas reflux than patients over 65 years old (58.5, 32.8 to 95.0 vs. 40.0, 24.0 to 71.0; 19.5, 6.0 to 47.5 vs. 8.0, 3.0 to 19.0; 46.0, 23.8 to 79.3 vs. 35.0, 11.0 to 56.0), and the differences were statistically significant (Z=-2.690, -3.286 and -2.091, all P 0.05). Patients with reflux esophagitis(RE)and /or Barrett′s esophagus (BE) had more weak acid reflux, non-acid reflux and gas reflux compared with patients with non-erosive reflux disease (NERD) (61.0, 31.3 to 102.5 vs. 44.0, 24.5 to 66.5; 18.0, 8.0 to 36.5 vs. 8.0, 2.0 to 22.0; 49.5, 27.5 to 86.5 vs. 26.0, 11.0 to 47.0), and the differences were statistically significant (Z=-2.585, -2.942 and -3.278, all P<0.05). Mei, Han-ming Peng, Yuan Gao, Jiaqing Chen","doi":"10.3760/CMA.J.ISSN.0254-1432.2019.04.004","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.0254-1432.2019.04.004","url":null,"abstract":"Objective \u0000To explore the role of interleukin (IL)-38 in inhibiting inflammatory bowel disease (IBD) in children and to investigate the potential molecular mechanisms. \u0000 \u0000 \u0000Methods \u0000From January 2014 to October 2017, 67 patients with ulcerative colitis (UC) and 115 patients with Crohn′s disease (CD) admitted to Wuhan Children′s Hospital were recruited, and 40 individuals with normal endoscopic findings were selected as control. Serum levels of IL-38 of IBD patients and healthy control were determined by enzyme-linked immunosorbent assay (ELISA). Immunohistochemical staining (IHC) was used to detect the expression level of IL-38, nuclear factor κB (NF-κB), phosphorylated signal transduction and activator of transcription 3 (p-STAT3), C-reaction protein (CRP) and erythrocyte sedimentation rate (ESR) in the intestinal mucosa of IBD patients and healthy controls. The extent of disease, therapeutic agents and disease activity scores (Mayo score system for UC patients, Crohn′s disease activity index (CDAI) for CD patients) were evaluated. IL-38-C57BL/6 transgenic mice model was established, and dextran sulfate sodium was used to induce IBD mice model. The intestinal inflammation levels were compared between the wild type IBD mice and IL-38 transgenic IBD mice. The levels of IL-38, NF-κB and p-STAT3 in intestinal mucosa of mice of different groups were determined by IHC. The ratio of CD4+ IL-17+ T helper (Th) 17 cells in peripheral blood of mice of different groups was detected by flow cytometry. Independent sample t test, chi square test and Pearson correlation were performed for statistical analysis. \u0000 \u0000 \u0000Results \u0000The results of ELISA showed that the serum levels of IL-38 of UC and CD patients were (6.1±1.9) ng/L and (9.8±2.1) ng/L, respectively, which both were lower than that of healthy controls ((16.4±2.7) ng/L), and the differences were statistically significant (t=23.107 and 15.853, both P<0.05). The results of IHC indicated that the levels of IL-38 in the intestinal mucosal tissues of UC and CD patients were 0.04±0.01 and 0.03±0.01, respectively, which were both lower than that of healthy controls (0.18±0.02), and the differences were statistically significant (t=48.186 and 69.443, both P<0.05). The levels of NF-κB and p-STAT3 of UC and CD patients were 0.150±0.030, 0.160±0.040 and 0.130±0.030, 0.110±0.010, which were all higher than those of healthy controls (0.020±0.003 and 0.010±0.002), and the differences were statistically significant (tUC=27.273 and 23.078, tCD=23.657 and 62.684; all P<0.05). The number of patients with disease at active phase, CRP level, ESR and disease activity scores of UC and CD patients with low IL-38 expression were all significantly higher than those of patients with high IL-38 expression (χUC2=11.552, tUC=7.118, 8.991 and 7.086; χCD2=5.675, tCD=9.559, 9.358 and 11.268; all P<0.05).
