{"title":"[Pharmacologic prevention of postoperative cystitis].","authors":"J Zikmund","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>For prevention of postoperative cystitis the authors tested currently used drugs which influence the adherence of bacteria to the urothelium, nitrofurantoin (FurantoinR), 3 x 1 tabl. per day and trimetroprim with clotrimoxazole (BiseptolR), one tablet in the evening before operation. They found that Furantoin reduced the frequency of the inflammation in patients subjected to abdominal operations from 12.3% in the control group to 2.3% and in those subjected to vaginal operations from 46.1% to 9.6%. The disadvantage of the drug is that it must be taken every day and that it is poorly tolerated by the patients. Biseptol is excreted more slowly and therefore 1 tablet before operation blocks the development for a maximum of 48 hours. In patients with abdominal operations the frequency of inflammation was similarly as in controls, 12%, in patients with vaginal operations the number of inflammations declined to 24.5%. In the group of 16 women with Rh incompatibility whose pregnancy terminated by delivery of a normal neonate, in 22% the SP1 levels were higher than 1.5 x the median value for the given gestation week (MoM). In 17 pregnancies where the foetus suffered from haemolytic disease the SP1 levels were higher than 1.5 MoM in 45% of the examined foetuses. Two of these fetuses were aborted as a result of early prenatal diagnostic procedures: one after transcervical CVS, one after early amniocentesis. In three of these fetuses intraamniotic bleeding originating from the umbilical vein near the umbilical insertion produced cords of clotted blood and blood clots adhering to the surface of the fetus. Subsequently, the clots were organized by invading mesenchymal fibroblasts from fetal skin. The cords bent around extremities produced amputations (TLD) and constricted the umbilical vessels, if bent around the umbilical cord. The organizing surface blood clots located predominantly in the folds of the embryo, or fetus, influenced the growth of underlying tissues, producing oblique facial clefts, microphthalmia or micrognathia. EIAB represents a serious complication of invasive procedures related to prenatal diagnosis and must be regarded as the cause of malformations, such as TLD, or ADAM sequence.
