However,\u0000the evidence is controversial about immediate or conventional use.\u0000Objective: Assess the impact of immediate NIV after extubation on\u0000oxygenation and functional capacity of patients undergoing to CABG.\u0000Methods: Randomized clinical trial. Patients were assessed before and\u0000after surgery using the Functional Independence Measure(FIM), six-minute\u0000walk test(6MWT) and peripheral muscle strength(MRC). On the first day\u0000after the surgery, two groups formed immediate NIV(NIVI) and\u0000conventional NIV(NIVC). Hemogasometry was collected before and after\u0000NIV. Complication rates were also assessed. NIVI performed ventilation\u0000after one hour of orotracheal extubation, at NIVC performed NIV on the\u0000first postoperative day, 24 hours after extubation. After discharge, the\u0000above variables were reevaluated. Results: 79 patients were evaluated,\u000046(58.22%) men, mean age 65±9 years. NIVI reduced the reintubation\u0000rate, only 1 (3%) compared to NIVC with 5 (12%) patients, p=0.01. In\u0000the post-Intervention the inspired oxygen fraction (FiO2) was 0.43±0.07\u0000in the conventional group and 0.30±0.10 in the intervention group,\u0000p=0.01. The post-intervention PaO2/FiO2 ratio was 191±45 and NIVI\u0000266±29(p <0.001) and one day later in the NIVC it was 210±39\u0000and NIVI 279±37(p <0.001). ABSTRACT Introduction Bioelectrical impedance analysis (BIA) can be used to estimate FatFree Mass Index (FFMI). However, the use of directly measured BIA variables, such as phase angle (PhA), has gained attention. The frequency of low FFMI and PhA and its associations with exercise capacity and healthrelated quality of life (HRQL) in patients with idiopathic pulmonary fibrosis (IPF) have been scarcely studied. Objectives To investigate the frequency of low FFMI and PhA and their associations with exercise capacity and HRQL in patients with IPF. Methods Patients underwent assessment of lung function, body composition, exercise capacity by the 6 min walk distance (6MWD), and HRQL by the Medical Outcomes Study ShortForm 36item Questionnaire (SF36). Patients were classified as presenting normal or low PhA or FFMI, accordingly to the 10th percentiles of agesexbody mass index (BMI)specific reference values. Results 98 patients (84 males, age: 68±8 years, forced vital capacity: 64%±18%predicted) were included. 24 patients presented low PhA. They were characterised by worse lung function, exercise capacity and HRQL compared with patients with normal PhA. 10 patients presented low FFMI, but despite differences in body composition, no differences were found between these patients and patients with normal FFMI. In a single regression analysis, age, lung function and body composition variables (except FFMI) were related to 6MWD and SF36 Physical Summary Score (R2=0.06–0.36, p<0.05). None of the variables were related to SF36 Mental Summary Score. Conclusion Onefourth of the patients with IPF with normal to obese BMI present abnormally low PhA. However, studies in COPD have yielded mixed results, possibly because the contextual factors that characterise the use of mobile technology in COPD are not well understood. This study compared intrapersonal characteristics of patients with COPD who use mobile applications (apps) with those who do not. Patients with COPD were eligible if they used smartphones. They were surveyed about their use of mobile apps (any app beyond calls/texts apps), and specifically, apps for PA promotion and for COPD management, performed a 4.5m gait speed test and used an accelerometer (ActiGraph GT3X+) for 7 days. Chi-square, Mann-Whitney U and T-tests were used to compare sociodemographic (age, sex, marital status, education, occupation), health-related (FEV1pp, gait speed, CAT, mMRC, 6MWT) and PA (step count; time in sedentary, light, moderate and vigorous PA) characteristics between patients using and not using mobile apps. A total of 54 participants were enrolled (72% male; 67±8yrs; FEV1 48±18pp). 31/54 (57%) used mobile apps; from these, 15 (48%) used apps for PA promotion, 4 (13%) used apps for COPD management and 4 (13%) used both. Participants using mobile apps walked at a higher speed (Median [M] 1.49 [1.33-1.72] vs. M 1.31 [0.98-1.54]m/s; p=0.021) and spent more time in vigorous PA (M 0.49 [0.14-1.45] vs. M 0.12 [0.07-0.84]min/day; p=0.025) than those not using apps. No other differences were found. Patients with COPD using apps presented higher functionality and PA behaviours than those not using them.
