It is also an uncommon first manifestation of leprosy. In this case, due to the absence of classic signs and symptoms, such as skin lesions, hypoesthesia, and loss of sensation, the swelling was clinically diagnosed as a peripheral nerve sheath tumour on initial presentation. The authors have described an unusual presentation of pure neuritic leprosy that showcased nerve abscess. Clinical presentation was swelling over the medial aspect of the right arm and ulnar aspect of the right forearm, and clawing of the fourth and fifth digits of the right hand, with no signs of inflammation present on the skin surrounding the swelling. Hansen’s disease with pure nerve involvement could not be considered as a differential diagnosis solely based on clinical examination, therefore posing a diagnostic challenge. On ultrasonography, the right ulnar nerve showed diffuse intermittent thickening and a heterogeneously hypoechoic fusiform area with internal echoes at the site of swelling indicating a nerve abscess. These led the authors to the diagnosis of leprosy with perineuronal abscess. Histopathology confirmed the diagnosis by revealing granulomas and acid-fast Hansen’s bacilli. The patient showed dramatic response to surgery, multidrug therapy, and systemic corticosteroids.","PeriodicalId":93747,"journal":{"name":"EMJ. More\u0000recently, Internet-linked point-to-point virtual private networks (VPN) mean that\u0000departments and hospitals that have access to such systems can request and\u0000send images via these links. Cloud-based systems have been replacing these in\u0000some institutions, with images held both locally and uploaded to a remote centre.\u0000An update to these systems is being trialled in Austria by Andreas Nuener and\u0000Christian Stark, based at the University Hospital, Innsbruck, who here discuss\u0000the development of such. Utilising a Siemens Healthineers (Erlanger, Germany)\u0000eHealth Solutions technology platform, they are planning the automatic registration\u0000(provision) of clinical images, such as from sonography, computed radiography, MRI,\u0000or CT, in Austria’s nationwide electronic health record (EHR) system, Elektronische\u0000Gesundheitsakte (ELGA). A small trial of this system demonstrated that any eligible\u0000healthcare professional who needs access to the images can do so instantly. Astrid\u0000Grams, Medical University of Innsbruck, Austria, illustrated the use of this system via\u0000the case scenario of a patient who has had an ischaemic stroke and is being treated\u0000at a remote hospital, with the treating physician querying whether or not they need\u0000to be transferred to the University Hospital, Innsbruck, for more specialist treatment.","PeriodicalId":93747,"journal":{"name":"EMJ.
