{"title":"The comparison of three selective serotonin reuptake inhibitors for adolescent depression","authors":"Oguz Guvenmez, Serkan Güneş","doi":"10.5455/IM.24457","DOIUrl":"https://doi.org/10.5455/IM.24457","url":null,"abstract":"Background: Major depression is a common psychiatric disorder among children, adolescents, and adults that affects the daily functioning. In this study, we aim to determine the efficacy and tolerability of three medications that frequently use in the treatment of adolescent depression. Methods: A total of 432 adolescents with documented DSM-5 major depression diagnosis was included to the study. Clinical characteristics of the sample were retrospectively obtained from the medical records and structured psychiatric interviews. Improvement was assessed with Children's Depression Inventory (CDI) and Clinical Global Impression-Improvement Scale (CGI-I) and side effects were examined with the adverse effect scale developed by the authors. Results: Mean age of the patients was 14.11±1.75. 34.1% of adolescents were male and 65.9% were female. 38.0% of the patients were using fluoxetine, 43.3% were using sertraline, and 18.7% were using citalopram. The CGI-I and CDI scores were significantly different between the three groups. There was no significance in terms of treatment compliance. Stomach ache and aggressionirritability were meaningfully different between the groups. Conclusions: Citalopram appears to be less effective than fluoxetine and sertraline. Fluoxetine and sertraline seem to show similar effectiveness. {"title":"The impact of a redesigned clinical course curriculum on the development of clinical reasoning skills among medical students: a quantitative study","authors":"Rhys E. Taylor, S. It is best described as the ability to analyze, interpret, and investigate a patient appropriately given their constellation of symptoms and signs. Clinical reasoning has come under scrutiny as an area for professional development, with many questionings whether it can be formally taught or if it is simply an organic process which improves with experience. This study aims to investigate whether formalized clinical reasoning teaching during medical school has an impact on the clinical reasoning abilities of its students. Methods: Data was collected across three cohorts at the University of Nottingham Medical School. Each cohort had had varying degrees of clinical reasoning teaching, with Cohort 1 receiving no formalized clinical reasoning teaching, Cohort 2 receiving some teaching, and Cohort 3 having an embedded clinical reasoning component to their clinical education. The end of year exam results for each cohort were analyzed both in their first and final year of clinical exposure, to determine how well each cohort performed in the clinical reasoning component of the written tests. Results: On analysis of the results, the clinical reasoning scores were consistently higher in the cohort who received full clinical reasoning teaching during their clinical phases. There was one exception to this, but the data suggests this may itself be due to an anomaly in the overall abilities of that cohort during this time. With these results, the null hypothesis that clinical reasoning teaching has no significant effect on the clinical reasoning of students as measured by the clinical reasoning score in the summative exams was rejected. Conclusions: The main finding of this research was that the implementation of a clinical reasoning teaching curriculum can have a significant impact on the clinical reasoning of its students. Method: The research had a cross-sectional design. The sample of the study consisted of 356 women. The data of the study were obtained using a questionnaire that consists of 41 questions by face to face interview technique. Results: The mean age of the women who participated in the research was 36.49±7.07, the mean first gestational age was 25.20±3.75, the mean number of pregnancies was 1.85±0.91, and the mean parity was 1.74±0.84. The first labors of 70.5% of the women were normal vaginal delivery, 52.0% used effective family planning method, and 95.5% could easily reach the methods. In the study, it was determined that there was a negative, moderate, significant correlation between the first marriage age and the mean number of pregnancies, that the number of pregnancies and parity increased significantly as the educational level of women decreased, that the number of pregnancies and parity increased significantly as the educational level of spouses of women decreased and that the number of pregnancies and parity decreased significantly as the level of income of family increased. Conclusion and Recommendations: In the research, the fertility of women was found to be associated with some sociodemographic characteristics. For this reason, improvement of sociodemographic characteristics that may negatively affect the fertility should be supported. It is a rare entity in developed countries, however, increases due to immigration. Most cases are due to vitamin D deficiency and they may be diagnosed by an exhaustive anamnesis, typical biochemical changes and radiological signs. Its suspicion and early diagnosis are essential for an early start of treatment. We report a case of rickets in a young child with vitamin D deficiency, to remark the importance of this problem, which is currently reemerging in our environment. The child presented legs deformity and growth retardation. All the examinations performed, including blood tests, radiological studies and clinical history oriented to rickets. {"title":"The potency of intraoperative execution on immediate postoperative outcome in patients with chronic hepatitis C viral infection undergoing mitral valve replacement","authors":"A. Thus, recommendations, surgical indications, and maneuvers were not conclusive. This work aims at illustrating the impact of intraoperative performance of mitral valve replacement (MVR) on the immediate postoperative outcome parameters, recovery, and survival to deduce beneficial recommendations to improve the operative and postoperative results of this high-risk group of patients. Methods: This study included 144 chronic HCV infection patients presenting with rheumatic mitral valve disease necessitating MVR between April 2012 and March 2019. Group (I) included 108 patients with <45 minutes ischemic time and group (II) included 36 patients with >45 minutes ischemia. Results: Group (I) who was subjected to statistically significant lower ischemic time showed statistically significant operative and immediate postoperative outcome parameters: total cardiopulmonary bypass (CPB) time, total operative time, smooth weaning off bypass, intraoperative hemodynamic parameters, duration of mechanical ventilation, duration of inotropic support medications in intensive care unit (ICU), postoperative platelets transfusion, postoperative values of transaminases, alkaline phosphatase, total bilirubin and creatinine, incidence of postoperative acute renal failure, time needed to reach target INR, total ICU stay, and total duration of hospital admission. The overall postoperative mortality was 6 (4.16%); 3 (2.77%) deaths in group (I) and 3 (8.33%) deaths in group (II) (p>0.05) and the overall hospital complication rate was 32.41% and 38.88% for group (I) and (II) respectively (p>0.05).
