Rupawala, Naiya Shah, Zenish Bhatti, Kruna Bhimani","doi":"10.5005/jp-journals-10001-1442","DOIUrl":"https://doi.org/10.5005/jp-journals-10001-1442","url":null,"abstract":"Aim and objective: To scrutinize and conduct a review of the scientific literature describing the visualization of elongated styloid process (ESP) using an orthopantomogram in general population as well as in patients having oral submucous fibrosis (OSMF). Background: The styloid process, arising from the temporal bone and extending up to 25–30 mm in length, can be a cause of chronic pain and suffering. Similarly, OSMF is rapidly emerging as a widespread, debilitating collagen metabolic disorder affecting the younger population, owing to addiction to tobacco and areca nut chewing. The surgical protocol for the treatment of submucous fibrosis demands thorough postoperative physiotherapy, failure of which will lead to reoccurrence of trismus. The physiotherapeutic exercises become particularly challenging if the styloid process exhibits abnormal elongation, due to chronic inflammation and irritation. An ESP, thus, may become an impediment for patients being surgically treated for OSMF. Review results: The review was designed based on the 58 relevant articles obtained after inserting the terms “Elongated Styloid Process and Oral submucous Fibrosis” and “Elongated Styloid Process and Orthopantomogram” in the databases of PubMed and ScienceDirect, respectively during the period of September 2017 till September 2020. Conclusion: Orthopantomogram was widely used as a primary imaging investigation to evaluate the styloid process. An ESP, measuring more than 30 mm, was commonly observed in the older population and in men. The use of cone beam computed tomography and three-dimensional computed tomography was highly limited yet universally advocated in long-term studies for a more detailed scientific explanation of the occurrence and angulation of an ESP both in general population and in patients exhibiting OSMF. Clinical significance: The co-incidental finding of an ESP and OSMF must be carefully diagnosed before planning the surgical intervention in patients having submucous fibrosis. EDH results from traumatic head injury, usually with an associated skull fracture and arte rial laceration. The incidence of EDH is highest among adolescents and young adults and reported mortality rates range from 5-43%. Therefore, we aim to look into the EDH for both medical students and new physicians face in the recognition, diagnosis and management of these conditions. Targeted population: EDH patients who are requiring urgent management in the ED, with Emergency Physicians for teaching approach protocol. Aim of the study: Appropriate identification of EDH patients and management by training protocol to Emergency Physi -cians. Based on the child’s age, site, severity and types of injuries. Methods: Collection of all possible available data about the EDH at the emergency department. By many research ques tions to achieve these aims so a midline literature search was performed with the keywords “critical care”, “emergency medicine”, “principal’s Epidural hematoma in different emergencies”, “cardiac arrest with Epidural hematoma”. Litera ture search included an overview of recent definition, causes and recent therapeutic interventions strategies. Results: All studies introduced that initial diagnosis of different emergencies situations for EDH and their interventions therapy are serious conditions that face patients at the emergency departments. Conclusion: EDH is caused by bleeding in the potential space between the dura and the skull, usually because of traumat ic injury. The Clinical manifestations of EDH are highly variable, and include altered consciousness, headache, vomiting, drowsiness, confusion, aphasia, seizures. This improves overall functional and cosmetic outcomes. Conservative mandibular resections including the classical rim, reverse, sagittal, and oblique types have been described in contemporary literature. Materials and methods: The study used a retrospective analysis of adult patients managed in a University Hospital who underwent PCTR and anastomosis from 2007 to 2017. Results: During the 10 years, 53 patients were diagnosed with cricotracheal or tracheal stenosis. Prolonged orotracheal intubation and blunt trauma were the leading causes of upper LTS. The stenosis were classified as per Myer-Cotton classification. Thirty-seven were chosen for surgical intervention. Of the patients who underwent surgical intervention, PCTR was done in 33, resection and anastomosis in 3 and 1 underwent balloon dilatation. Tube displacement and surgical emphysema were the most common complication seen postoperatively. Ninety-seven percent of the patients who underwent surgery were successfully decannulated. Conclusion: Partial cricotracheal resection is an established surgical procedure with low morbidity and mortality. Risk factors for increased morbidity include diabetes mellitus (DM), lengthy resection, and children. Rib autograft was found to be ideal for reconstruction. Aims and objectives: Describe and discuss: (1) The consequences of unilateral deafness. (2) The gradual recovery of failing neuronal circuits when stimulated with a cochlear implant. (3) The case of a blind patient with sudden unilateral deafness who required cochlear implantation that is used as a common thread for the subject. Materials and methods: Forty-five-year-old blind woman with sudden unilateral deafness. With unilateral deafness, she could not localize the sound source in terms of side nor if the sound came from above/below, near/far, or from front/back. Her hearing in groups and with ambient noise deteriorated. As a result, she lost her autonomy and required and underwent cochlear implantation. Results: It took her 2 years to recover full sound localization, to be able to discriminate in groups, and to recover binaural fusion. Recovery was gradual. Her abilities to localize sound source in terms of side, of being near or far, or coming from above or below recovered separately, that is to say, at different periods of time. Conclusion: After losing functional neuronal circuits, early stimulation with a cochlear implant helped to fully recover these circuits. Neuronal circuits for sound localization for side, coming from above or below, near or far are seemingly different since they recovered at different times. Due to different mechanism, the upper airway (UA) collapses during sleep. The ENT is the specialty that plays an important role in the exploration of the UA and also in the treatment of OSA, because the UA is our domain and in our hands is the ability to explore and to treat the disease. Nevertheless, the exploration of the UA in an obstructive sleep apnea syndrome (OSAS) patient is not the same as the exploration of other pathologies, one has to pay attention to small details that are not important in everyday exploration but in OSAS can change the treatment plan.
