As a side effect of the therapy can occur an acute interstitial nephritis (AIN) or an acute tubular necrosis (ATN). In acute kidney injury, a major challenge is to distinguish between underlying etiologies. For instance, there are no disease-specific therapies for acute tubular necrosis, whereas acute interstitial nephritis requires immunosuppressive therapy. The differentiation of the two diseases is very important, but unfortunately, they present clinically and laboratory similarly. Good biomarker for differential diagnosis is lacking. Often renal biopsy is required for diagnosis. Renal biopsy is often associated with side effects and cannot be performed in all patients. Therefore, it is very important to know the small differences of the two diseases. In girls the cause can be congenital or post traumatic. Female genital mutilation may be associated with urethrovaginal fistula and the predominant presentation is that of continuous urinary incontinence. Presentation with bladder outlet obstruction and inability to pass urine is rare. We report a 12-year old girl with traumatic urethro-vaginal fistula post female genital mutilation who presented with inability to pass urine and bladder outlet obstruction. Case report This is a 12-year old girl who presented with urethral bleeding and inability to pass urine following female genital mutilation. She had suprapubic cystostomy due to failed urethral catheterization. Micturating cystourethrogram revealed huge bladder with tight bladder neck. The haemogram, urine microscopy, electrolyte urea and creatinine were within normal limits. She had examination under anaesthesia which revealed urethrovaginal fistula and obliteration of the bladder neck. She had multilayer repair of the fistula via abdominoperineal approach. Urethral catheter was removed after 6 weeks. She developed nocturnal enuresis post operatively which was managed with tolterodine. Conclusion Urethrovaginal fistula from female genital mutilation may present with bladder outlet obstruction. Micturating cystogram may not be diagnostic. Examination under anaesthesia may be necessary for proper diagnosis. Albeit uncommon, it and other congenital nephropathies (e.g., Frasier syndrome) may warrant a clinical workup of cardiovascular disease in symptomatic patients with cardiopulmonary deficits, as the latter is associated with renal dysfunction given the multifarious physiologic roles and nodes of action concerning the kidneys. These include not only serving as a site of neural and hormonal stimuli that affect systemic equilibrium, but the regulation of blood pressure and osmolality as well. The cardiorenal demise classically seen in later stages of chronic kidney disease (CKD) in relatively older adults may arise earlier in these patients. We report the case of an 18-year-old 46, XY phenotypic female with DDS who complained of increasingly nettlesome dyspnea. Initially, she presented with a low index of suspicion for diminished cardiopulmonary fitness. We investigated factors associated with End Stage Renal Disease (ESRD) patients' knowledge about KT benefits.\u0000\u0000\u0000Methods\u0000We randomly invited 1,400 dialysis patients to complete a survey about benefits of KT. Using multivariate analysis, we calculated odds ratios for the probability of choosing the correct responses.\u0000\u0000\u0000Results\u0000Of 673 participants, 17.6% agreed with benefit of KT for older patients, 36.5% agreed with benefit of KT for diabetic patients, and 31.5% agreed with benefit of pre-emptive KT. Non-white (OR: 0.68) and older (OR: 0.65) participants were less likely to agree with the survival benefit of KT. Older participants were less likely to agree with benefit of KT for older (OR: 0.64), and diabetic patients (OR: 0.54). Participants with less than high school education were less likely to agree with benefit of pre-emptive KT (OR: 0.58). Participants with a previous KT were more likely to agree with benefit of KT for older (OR: 2.32), and diabetic patients (OR: 2.50), and with the benefit of pre-emptive KT (OR: 2.34). Participants who had received 3 or more modes of education about KT were more likely to agree with benefit of KT for diabetic patients (OR: 2.04), and with benefit of pre-emptive KT (OR:1.67).\u0000\u0000\u0000Conclusions\u0000Dialysis patients have limited knowledge about benefits of KT. It is quite prevalent in all races and has no gender predisposition. However, white adults are at more risk of MN. It is a relatively less common entity in pediatric population. We report a case of 12 years old female with idiopathic MN. She had no symptoms suggestive of nephrotic syndrome and was normotensive.
