Shaw","doi":"10.21037/fomm-21-4","DOIUrl":"https://doi.org/10.21037/fomm-21-4","url":null,"abstract":"The COVID-19 pandemic is having a significant impact on the provision of non-COVID-19 related clinical services. Early recommendations for head and neck reconstructive surgery were based on guidance from expert groups, advocating de-intensification of surgery. Since then, patient outcomes derived data has suggested that the continued practice of head and neck reconstructive surgery based upon pre-pandemic standard of care is safe if appropriate measures are in place for appropriate screening and segregation of care pathways for patients. In addition, adequate levels of personal protective equipment (PPE) are vital for both patients and the healthcare team. We present the current practice guidance within the UK National Health Service (NHS) for head and neck reconstructive surgery in the COVID-19 pandemic era in the following areas: COVID-19 testing/screening, care pathways for patients, the potential future role of immunisation against SARS-CoV-2, airway management, selection of the type of reconstruction, postoperative care and rehabilitation. The guidance produced reflect the evolving nature of the response of NHS to the COVID-19 pandemic, some of the suggested practice protocols could differ from local policies in various parts of the world however the principles which underlie these standards are the results of regular review of the needs of the patients and health service, balanced against the background of the ebb and flow of the prevalence of COVID-19 infection within the community and healthcare settings. © Frontiers of Oral and Maxillofacial Medicine. The literature has consistently demonstrated the successful utilization of alloplastic temporomandibular joint reconstruction devices (TMJR) in the management of end-stage TMJ disease. In light of the safety and efficacy demonstrated by TMJR in such cases over the years, this paper will discuss the rationale, advantages and disadvantages related to the use of TMJR devices for the management of mandibular condylar resorption. Methods: A review of the pertinent English language orthopedic and TMJ literature on PubMed (pubmed. gov) between the years 1996 and 2021 was conducted using the terms “bone adaptive remodeling” and “TMJ condylar resorption”. Key Content and Findings: Based on the effects of increased or extreme joint overload resulting in a maladaptive remodeling of the TMJ condyle, potential comorbid factors, the severity and activity of the process, as well as patients’ desires, the following management options were found and can be considered: no treatment, orthodontics, medical management with orthognathic surgery, disc repositioning with orthognathic surgery and alloplastic TMJ replacement. Based on the relevant literature reviewed, TMJR appears to be a safe and effective surgical option for the management of patients with end-stage condylar resorption. Conclusions: Based on the literature reviewed for this paper, TMJR is a safe and effective surgical option for the management for both the skeletally mature and skeletally immature patient with end-stage condylar resorption. Therefore, it also appears appropriate for surgeons to consider a management option for condylar resorption such as TMJR that does not depend on a biomechanically compromised, degenerated condyle with documented biological maladaptive capacity under loading functions. The need for antimicrobial coatings for devices placed inside of the body, particularly penetrating devices that emerge inside the oral cavity is of profound interests to dentistry. Titanium dental implant surfaces, including the abutment-implant junction are associated with a relatively high incidence of chronic infection termed peri-implant disease. In fact, peri-implant disease is the most frequent complication of dental implant treatment. Therefore, a narrative review of the need for technological development to prevent or curtail peri-implant disease is presented. Methods: A review of current English language literature published between 2011 and December 1 st , 2021 was done in PubMed using the MeSH subject of anti-bacterial treatment of titanium dental implants. The Search librarian conducted the search at the American Dental Association Library in Chicago, Illinois. A further criterion was searched for sustained in vivo anti-microbial activity. There were no stringent selection criteria, such as removing fluoride treatment from the search, due to the limitation of articles in this nascent field. Key Content and Findings: This narrative review summarizes the lack of publications that fulfill the search criteria with no surface treatment being shown to have a sustained antimicrobial effect. Therefore, there remains a need for extraordinary oral hygiene measures to delay gingival marginal instability, circumferential bone loss, papillary height instability and extension of peri-implant disease around the surface of the implant. Conclusions: Antimicrobial surface treatments or coatings were reviewed, none of which have proven to satisfy present clinical needs including the use of antibiotic, silver or nitride coatings.
