{"title":"Managing many patients with a urethral stricture: a cost-benefit analysis of treatment options.","authors":"B C Ogbonna","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To report a management method in a community where there are many patients with urethral stricture and where the short-term goal of providing some treatment to most may override the sometimes conflicting long-term aim of minimizing recurrence rates.</p><p><strong>Patients and methods: </strong>Over a 3-year period, using optical urethrotomy in 76 patients followed by intermittent self-dilatation (ISD) in 29, urethroplasty in 28 and dilatation in three, 92 of 134 patients with a urethral stricture were treated and the outcome compared.</p><p><strong>Results: </strong>The overall recurrence rate was 22%; a combination of urethrotomy plus ISD had a recurrence rate of 17% and gave a mean duration of follow-up without recurrence similar to that after urethroplasty. ISD significantly increased both the time before recurrence and the duration of follow-up without recurrence after urethrotomy. Johansson","doi":"10.1097/00005392-199805000-00140","DOIUrl":"https://doi.org/10.1097/00005392-199805000-00140","url":null,"abstract":"OBJECTIVE\u0000To evaluate the efficacy and safety of ureteric stone treatment by ureteroscopy and laser lithotripsy during pregnancy.\u0000\u0000\u0000PATIENTS AND METHODS\u0000Four pregnant women (mean age 29.5 years, range 27-35) with five episodes of ureteric stones were treated by ureteroscopy and laser lithotripsy when the fetus was at 26-35 weeks of gestation. The stones (between 5 and 16 mm in diameter) were located in the proximal (one) or distal ureter (four).\u0000\u0000\u0000RESULTS\u0000All five stones were removed successfully by ureteroscopy and laser lithotripsy. The operating time varied between 15 and 70 min. In two of the five cases, topical anaesthesia was adequate and in no case was fluoroscopy necessary. No complications occurred that could be related to the procedure.\u0000\u0000\u0000CONCLUSIONS\u0000Ureteroscopy and laser lithotripsy seem, in experienced hands, to be a safe and reliable method in the treatment of ureteric calculi during pregnancy.
