It may be tempting to send patients to the nearest eye emergency service when hearing the patient's presenting complaint. This is understandable given limited access to ophthalmic equipment in primary care, including, for example, a slit lamp or even a working ophthalmoscope. However, taking a relevant and accurate history with a subsequent focussed examination to assess the extent of damage is both useful and vital to ensure the patient is seen by the ophthalmologist within the appropriate timeframe. This condition was named after Francois Gigot de la Peyronie in 1749. This condition causes symptoms of curvature of the penis on erection, painful erection, or pain during intercourse, and can lead to emotional or psychological turmoil for the patient. People suffering from the disease can feel a lump or plaque in the shaft of the penis, which can lead to deformity or shortening of the penis. An estimated 84% of couples will conceive naturally within a year if they have regular unprotected sex; but those who have been trying to conceive for greater than 3 years without success are less than 25% likely to conceive naturally within the course of the following year. Investigation of infertility in primary care is often focused on the female patient and neglectful of their male partner. However, male factors alone are responsible for 20–30% of couples’ infertility, and in an additional 30–40% of cases a male factor is present in combination with other female factors. An estimated 7% of men are affected by infertility, and the incidence of infertility appears to be increasing in the developed world. This article will address potential causes and how to investigate for them.
