{"title":"Comparison of Mean Post-Operative Pain Score with Pre-Incisional Versus Postoperative Injection of Bupivacaine in Patients Undergoing Elective Surgery.","authors":"Muhammad Bilal Akbar, Babar Hameed","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The most important concern after the surgical intervention is pain which needs to be addressed for better compliance and early mobilization to avoid other complications. The need for a better analgesic and time of administration is the key to success.</p><p><strong>Objective: </strong>To compare the mean post-operative pain score with pre-incisional versus post-operative injection of bupivacaine in patients undergoing elective surgery.</p><p><strong>Study design: </strong>This Randomized controlled trial was conducted in the General surgery department, National Hospital & Medical Centre, Lahore from 24-02-20 to 24-11-20. Data was collected through Non-probability consecutive sampling. The cases of both genders with an age range of 18-60 years undergoing elective surgeries were selected. They were divided into two groups. The cases in group A were given pre-incision bupivacaine and group B with post-operative bupivacaine and were assessed 1 hour after surgery regarding pain on VAS.</p><p><strong>Results: </strong>In the present study there were a total of 64 cases with 32 in each group. The mean age in group A was 47.56±7.51 years while in group B was 49.13±8.03 years. There were 19 (59.37%) males in group A vs 18 (56.25%) in group B. The mean post-operative pain in group A was 3.07±0.67 vs 3.59±0.91 in group B on VAS with p= 0.03. Mean post-operative pain in males was 3.12±0.65 vs 3.47±0.86 and in females, it was 3.21±0.66 vs 3.53±0.90 in groups A and B with p values of 0.21 and 0.23 respectively. Mean Post-operative pain was 3.11±0.66 vs 3.44±0.83 in the age group 18-39 years and 3.08±0.68 vs 3.47±0.88 in age group 40 to 60 years in group A and B with p values of 0.25 and 0.20 respectively. There was a significant difference in terms of postoperative pain in cases undergoing open cholecystectomy where pain was 3.21±0.78 vs 3.86±1.02 in group A and B respectively with p= 0.01. This study aimed to comprehensively evaluate the memory-enhancing activity of an ethanol extract from Ficus benghalensis and its impact on acetylcholine levels in a scopolamine-induced amnesia model. Albino Wister rats were used for the experiments. The ethanol extract from Ficus benghalensis was prepared and administered to the rats at a dose of 100, 200 mg/kg. The scopolamine-induced amnesia model was established by injecting scopolamine. The memory and cognitive function of the rat were assessed using appropriate behavioral tests, such as the elevated plus maze, and passive avoidance test. Acetylcholinesterase activity was measured using specific biochemical assays. The ethanol extract from Ficus benghalensis at a dose of 200 mg/kg exhibited significant memory enhancement in the scopolamine-induced amnesia model. The treated group showed improved performance compared to the control group. Furthermore, the extract demonstrated inhibitory effects on acetylcholineesterase activity, leading to increased levels of acetylcholine. Panchakarma is one of the vital branches of Ayurveda, which deals mainly elimination of vitiated doshas from the body. Basti is considered as superior, because of its specific indications and action. Though shodhana is contraindicated in Bala and Vrudha, Basti is indicated. Ageing is an inescapable reality of the human existence, plays a crucial role in the global demographic transition. According to UN Population Division, there will be 2elderly persons for every child by 2050. This implies the aged60 and above, which currently constitute less than 20% of the population will account for32% of the population by2050. In kalajajara as vata prakopa occurs naturally, and results in mamsa bala kshaya that is degenerative condition. Basti is the first line of treatment for the vatadosha. Yapana is a type of basti which sustains supports and maintains the life and provides longevity. It can be given in all season, any time and in all age groups. Yapana basti enhances status of shukra, mamsa and gives bala to the body and checks the process of aging enhances the tissues, alleviating the disease, support and maintains the life. Rasayana is a specialized type of treatment influencing the fundamental aspect of body like dhatu, Agni, srotas and ojas etc. and boosts tejas and immune system. Most of the time patients admitted to the hospital with any serious condition are first given an intra venous route of administration. Some of patient will continue with IV where as some will be converted to oral administration. The shift from iv to oral depends on disease severity, co-morbidity, and drug bioavailability. Body weight-based conversion of intravenous to oral should be practiced for effective and tolerable treatment and for good therapeutic outcome. But this conversion from IV to oral is not applicable in all patients whose GI absorption is altered. IV administration is mostly effective in highly infected patient and in septicemic for their early recovery. But long-term iv administration leads to iv abuse that includes phlebitis, infiltration, hematoma. Prolonged iv administration of psychotropic drugs like Carbamazepine leads to sleep abnormalities, decreased gait, anxiety disorders. Nowadays irrational use of IV antibiotics is increasing, hence to avoid irrational use of antibiotics IV should be converted to oral at proper time and guidelines.
