With its increased utilisation, anaesthesiologists will more frequently encounter patients with an indwelling vagal nerve stimulator (VNS), both in the elective and in the emergency surgical settings. We encountered a case of a suction catheter that was stuck in an ETT and split by forcible pulling. The patient was intubated with a 6.5 mm oral-preformed ETT. At the end of surgery, a 12-French suction catheter became lodged in the ETT. The catheter was pulled forcibly, and it split into two parts. Normal saline was injected into the ETT for lubrication and, using forceps, the catheter was pulled in small increments and successfully removed. To prevent this complication, physicians should carefully select the appropriate catheter size, and the catheter should be lubricated. The procedure was performed under epidural anesthesia, intravenous sedation with dexmedetomidine and spontaneous ventilation without tracheal intubation. The combination provided high quality intraoperative analgesia and a moderate level of sedation without causing respiratory depression or excessive hemodynamic instability. The goal of the Centers for Disease Control and Prevention (CDC) is to better understand the pathogenesis, signs and symptoms, and prevention of this delayed anaphylactic reaction compared to the rapid onset of most food allergies, especially immunoglobulin E (IgE) mediated allergy.
