{"title":"The Role of Cardiac MRI in the Postsurgical Follow Up of Aortic Coarctation-Our Experience","authors":"S. Stuppner","doi":"10.23880/CRIJ-16000144","DOIUrl":"https://doi.org/10.23880/CRIJ-16000144","url":null,"abstract":"Congenital cardiac disease represents the most frequent malformation pathology, with an incidence variable from 0.5% to 1.2% of live births. These values are significantly higher if we also consider mild congenital defects that do not require treatment or are not recognized at birth, but which can become clinically relevant in adulthood, such as some abnormalities of the aortic arch, the bicuspid aortic valve, an abnormal venous return partial, coronary anomalies and fistulas. In our South Tyrol region (Italy), the birth rate is of about 5500 births / year, equal to a prevalence of CCD of 50 60 / year. About 40% of the CCDs requires surgical treatment in the neonatal period (<30 days) and about 90% of patients operated reach adulthood. The rate of growth of the adult population affected by CCD is equal to 5% per annum. {"title":"Conventional Transarterial Chemoembolization (TACE) of a Hepatocellular Carcinoma with Aberrant Extrahepatic Blood Supply using Balloon Microcatheter for Flow Redirection","authors":"K. A","doi":"10.23880/crij-16000152","DOIUrl":"https://doi.org/10.23880/crij-16000152","url":null,"abstract":"Hepatocellular carcinoma (HCC) is a malignant tumor that commonly affects cirrhotic patients. For tumors with a Barcelona Clinic Liver Cancer (BCLC) stage B, locoregional therapy is the mainstay of treatment. Transarterial chemoembolization (TACE) utilizes the hepatic arterial network in localizing chemotherapy to the HCC. 80% of HCC cases are primarily supplied by the hepatic artery and most have extrahepatic collaterals from the inferior phrenic artery. This is the first reported case where the gastroduodenal artery (GDA) was the primary and sole blood supply to the HCC. The patient’s levels for AST, ALT, hemoglobin, bilirubin, albumin, PT/INR, and platelets were all within normal range. The initial MRI showed a single exophytic tumor located on the caudate lobe. According to the BCLC staging system, the tumor was determined to be in the intermediate stage and TACE was considered to be the appropriate treatment. The right gastroepiploic artery was first occluded with detachable coils. Subsequently, a 150 cm straight 2.9FrSniper balloon microcatheter was introduced into the proximal GDA. TACE was performed using balloon occlusion to redirect blood flow away from the feeder arteries to the duodenum and into the blood supply to the tumor. The diagnostic imaging should be performed with contrast tomography looking for findings of grouping of handles; grouping, elongation and regurgitation of the mesenteric vessels; displacement of the mesenteric main trunk to the right, among others.
