Despite this condition having adverse effects on maternal and child health, its prevalence and associated risk factors are still significant, especially in developing countries including Ghana. This study aimed to assess the prevalence and demographic distributions associated with pre-eclampsia among pregnant women at the Ho Teaching Hospital. Methods A facility-based retrospective study was conducted by reviewing available data or hospital records of pregnant mothers who were admitted to the labour and maternity wards from January 2018 to December 2020. The data were collected using a structured checklist. Results A total of 5,609 deliveries were recorded from 2018 to 2020. Out of the 5,609 deliveries, 314 pre-eclampsia cases were recorded giving an overall prevalence of 5.6%. The yearly prevalence for 2018, 2019, and 2020 were 4.6%, 5.6%, and 6.6%, respectively. The most recorded pre-eclampsia cases were seen among women within the age group of 18-24 years. The data showed that 112 (35.7%) of the mothers who had pre-eclampsia were nulliparous. Pre-eclampsia associated maternal and fetal complications were; preterm delivery 221 (70.4%), intrauterine fetal death 62 (19.7%), eclampsia 9 (2.9%), HELLP syndrome 5 (1.6%) and maternal death 17 (5.4%). Associated factors of pre-eclampsia were parity, level of education, and occupation (p=<0.05). Conclusion The findings of this study showed a rising trend in the incidence of pre-eclampsia over the years at the Ho Teaching Hospital. Parity, level of education, and occupation were found to be associated with developing pre-eclampsia.

Beyond managing\u0000the infection risk, positive Covid-19 surgical patients add additional challenges to their perioperative care, where perioperative risk is\u0000superimposed onto an already baseline anesthesia risk in real-time in an unknown and unpredictable fashion. Recent evidence suggests\u0000that surgical procedures for both current and recovered Covid-19 patients are associated with elevated morbidity and mortality. These\u0000risks must be well managed. However, the anesthetic risk aside from transmission is yet to be fully understood; consequently, effective\u0000and speedy dissemination of practice changes is vital to effective health care delivery. However, the traditional “knowledge to action\u0000gap” of 17 years or more in healthcare is a hinderance. We herein designed and implemented a new but simple perioperative criterion\u0000for previous Covid-19 patients based on the principle of the perioperative surgical home (PSH) within the Consolidated Framework\u0000for Implementation Research (CFIR). Given these tools, implementation and/or implementation components of the pathways based on\u0000recommendations which are fast evolving can readily be done, nearly live

In that eventuality, the\u0000assessment, diagnosis and treatment of this cohort can be challenging. The rapidly changing pathophysiology of the peri-operative\u0000patient may cognitively overload the attending clinicians. Furthermore, until stabilised, a comprehensive assessment of the patient is\u0000unlikely to be feasible or appropriate. So, vital data may be overlooked. In emergency situations, suboptimal management decisions\u0000may cause significant morbidity and mortality. Fortunately, standardisation and careful design of documentation (i.e. proformas and\u0000checklists) can enhance patient safety. Indeed, the World Health Organization’s Safer Surgery Checklist has been proven to improve\u0000post-operative outcomes. So, extrapolating this concept, I have developed a checklist proforma to guide the assessment of acutely\u0000unwell patients who may require emergency surgery. This proforma also promotes the systematic recording and presentation of\u0000patient data. This can facilitate the retrieval of the specific information required for the management for these patients. Moreover, this\u0000framework for clinical reasoning can identify the need for organ support and thereby guide decisions about escalation of therapy..

Many GT patients experience recurrent bleeding and require frequent platelet transfusion. Therefore,\u0000they are at risk for alloimmunization against platelet glycoprotein or Human leucocyte antigen (HLA) epitopes resulting in platelet\u0000refractoriness and therapeutic failure. We report a unique case of GT in pregnancy at risk for severe postpartum hemorrhage due to\u0000anti-GP IIb/IIIa and anti-HLA antibodies and the use of pro-hemostatic agents to optimize outcomes. Questions remain on whether activation of the pituitary influences the threshold and development time-course of postoperative pain. To address these questions, we evaluated postoperative hypersensitivity in female and male rats with ablated pituitary and gonadal hormone productions via hypophysectomy, ovariectomy and gonadectomy, respectively. Plantar incision, a model of acute postoperative pain, or sham operation was performed on rat hind paws. Hypophysectomy, ovariectomy and gonadectomy were achieved by surgical disconnection of pituitary, ovaries and testicles, respectively. Postoperative thermal and mechanical hypersensitivity were monitored for 7 days post incision. Hypophysectomy on female and male rats produced statistically similar thermal and mechanical postoperative hypersensitivity thresholds and time-courses as compared to intact estrous female and male rats. Moreover, ovariectomy and gonadectomy did not significantly change postoperative hypersensitivity observed in control female and male animals. Our experiments demonstrate that hypophysectomy, ovariectomy and gonadectomy do not significantly impact postoperative hypersensitivity observed in normal female and male animals. These data suggest that surgery-induced changes in the endocrine system via activation of pituitary and subsequently gonadal tissues have little impact on the threshold and development of postoperative pain in female and male rats.
