Patients with dental anxiety tend to avoid professional dental care which poses a problem for dentists and patients alike.There are various treatment options for reducing dental anxiety. Music has been used in different medical fields to meet physiological, psychological, and spiritual needs of patients. Music interventions can either be passive (music listening) or active music (therapy).There was a significant gap in the literature on the effectiveness of music therapy and music listening in reducing dental anxiety in patients undergoing various dental treatments/procedures. This systematic review specifically examined the effectiveness of music interventions in reducing dental anxiety in both paediatric and adult patients. OBJECTIVE The objective of this systematic review was to critically appraise, synthesise and present the best available evidence related to the effectiveness of music interventions on dental anxiety in paediatric and adult patients. DATA SOURCES A comprehensive search was undertaken on major electronic databases from their inception to October 2010. The search was restricted to English language and other languages where a translation was available from colleagues (for e.g. Chinese, Japanese, and Italian). REVIEW METHODS Randomised controlled trials, quasi-randomised controlled trials and quasi-experimental studies were included in the review. Critical appraisal and data extraction were undertaken using the Joanna Briggs Institute critical appraisal instrument and the standard data extraction form for evidence of effectiveness. RESULTS Seven studies with a combined total of 598 patients were included. Two out of the seven studies were RCTs, four quasi-randomised and one quasi-experimental study. There were 513 adult patients as participants from five studies and 85 paediatric patients from two studies. The results were classified into two categories.Two studies had paediatric patients as population group. One study found that music listening did not result in any reduction of anxiety during dental procedures on young patients; therefore the authors concluded that music alone is ineffective as a distraction during dental procedures. However, another study found that music reduced anxiety to some extent, although it was not significant.Five studies included in this review had adult patients as population group. Two studies concluded that relaxation when compared to music is an effective method of reducing patient's anxiety. One study concluded that music listening significantly lowered levels of anxiety and stress of females during dental procedures. However, in humans this has not been systematically investigated. If quinine has antiepileptic properties in humans, it may reduce the neurological sequelae associated with acute seizures in severe malaria and promote its choice over other antimalarial drugs in treating severe falciparum malaria. OBJECTIVE The review objective was to examine available research evidence on the effects of quinine on seizures in adults or children who present with seizures or who develop seizures in the course of treatment. INCLUSION CRITERIA Participants: This review considered adult and child patients who were prescribed using quinine for malaria, arthritis, nocturnal leg cramps, arrhythmia and systemic lupus erythematosus. INTERVENTION This review evaluated the use of quinine in comparison to other drugs used for malaria, arthritis, nocturnal leg cramps, arrhythmia and systemic lupus erythematosus. OUTCOMES The primary outcome of interest for this review was the proportion of participants who had seizures after the administration of quinine, compared with those who were not given quinine.Types of Studies: This review considered randomised controlled trials. SEARCH STRATEGY We searched online databases for published and unpublished studies written in English and identified articles using predefined criteria.Methodological Quality: Papers selected for retrieval were assessed by two independent reviewers for methodological validity prior to inclusion in the review using standardized critical appraisal instruments from the Joanna Briggs Institute Meta Analysis of Statistics Assessment and Review Instrument. DATA EXTRACTION/SYNTHESIS The data extracted included specific details about the interventions, populations, study methods and outcomes of significance to the review question and specific objectives. A standardized data extraction tool was used. A random effects model was used to statistically pool data in meta-analysis, in order to determine the effect of quinine on prevalence of seizures in comparison to other drugs. RESULTS We identified six randomized controlled trials on severe malaria. Quinine was compared to the artemisinin derivatives in all trials. A total of 8,244 patients were included. In the meta-analysis, there was no significant effect of quinine on the prevalence of seizures (Odds ratio=0.90 95% Confidence Interval=0.63-1.30). There was significant heterogeneity (Chi-squared=17.44, p=0.008). DISCUSSION This is the first review on the antiepileptic effect of quinine in humans. However, this effect is not demonstrated in patients with malaria. A dose-response effect may be responsible for the absence of antiepileptic properties of quinine in humans. Typically, patients less than eight years of age do not necessarily get emerged from general anesthesia in the operating room and transferred to the post‐anesthesia care unit in the same fashion as patients older than age 8. Therefore, patients less than 8 years of age will be excluded from this review. Patients not transported directly to the post‐anesthesia care unit from the operating room (e.g. direct intensive care unit admission or death in the operating room) will also be excluded from this review. Questions 1. What are the potential safety issues for seniors who engage in self‐care in their home community setting? 2. What are the potential safety issues related to activities of daily living? 3. Is there a distinction between adverse events resulting from therapeutic self‐care and selfcare related to activities of daily living in this population? 4. What is the experience of seniors living at home in dealing with adverse events resulting from self‐care behaviours? Inclusion Criteria Types of participants This systematic review will consider studies that address community‐dwelling seniors (65 years of age and older) who live alone or with others, but who have the capacity to and do engage in self‐care. Although studies may include younger participants, the average age of participants should be 65 years and older. Data from studies may be collected from seniors themselves, family members, formal and informal caregivers, including healthcare practitioners or from community organizations. Includes seniors who may have ongoing or occasional assistance with therapeutic or personal care, but who are able to direct such care. Data generated from studies of institutional settings or from those who are unable to direct their care because of impaired cognition will be excluded. Types of intervention This systematic review will focus on safety issues for seniors who engage in self‐care behaviours that are related to therapeutic regimes or daily living activities. The phenomena addressed in qualitative studies will be the experience of seniors living at home in dealing with adverse events resulting from self‐care behaviours. Inclusion Criteria Types of participants The review will consider publications that include the postoperative management of children aged between four to 18 years old,55 who have undergone surgery in the hospital and stayed in surgical, day surgery wards or ambulatory care units. The age group of four to 18 years was chosen as children younger than four have different linguistic abilities and have postoperative needs, which are beyond the scope of this systematic review. Phenomena of interest The review will consider studies that include the following phenomena of interest: children’s experiences in the management of their own postoperative pain. Context Context may include, but is not limited to, paediatric surgical wards in acute care hospital settings or in home settings within half a month after the surgery. Review Purpose/Objectives: The purpose of this systematic review is to systematically review and synthesise research literature reporting nurses’ experiences of ethical preparedness for dealing with catastrophic public health emergencies and health care disasters and the ethical quandaries that may arise during such events. INCLUSION CRITERIA: Types of Participants: The review will consider publications that include nurses registered or authorised under a given country’s state of emergency provisions to practice in jurisdictions in which a public health emergency (e.g. pandemic influenza) or sudden‐onset mass casualty health care disaster (e.g. flood, hurricane, earthquake, tsunami, volcanic eruption, terrorist attack) have occurred, or may occur. Phenomena of interest: This review will examine the phenomenon of nurses’ experiences of preparing for and/or managing ethical issues arising during a public health emergency or health care disaster. This systematic review aims to identify the efficacy of complementary therapy interventions in reducing the frequency and severity of agitated behaviour among older people in RACF. Specific questions to be addressed include: What type of complementary therapies are been implemented for the symptom management of agitation in older people residing in a RACF? Which complementary therapies are identified as being effective in reducing the frequency and severity of agitation in older people in a RACF? Inclusion Criteria: Types of Participants: Older people living in RACFs will be included regardless of cognitive ability, gender or ethnicity and existing co-morbities. Trials of RCTs investigating symptom management of agitation in older people over the age of sixty five will be examined. Trials undertaken in settings other than RACFs will be excluded. Types of Interventions: The types of interventions to be explored in this review are complementary therapies include: aromatherapy exercise massage music therapy therapeutic touch These interventions are of interest as they are non invasive, have minimal interactions with current medications and require minimal time, resources and training by nurses. Types of Outcome Measures: The outcome measures to be explored include severity and frequency of agitated behaviours. Outcomes will be measured with a variety of standardised and non standardised measurement tools using observation, with long term and short term measures.
