Little is known about gestational diabetes in KONKAN region of Maharashtra where women are undernourished across all stages of life. We estimated the prevalence of gestational diabetes among pregnant women visiting tertiary care hospital in KONKAN region. Methods: Between March 2020 and November 2020, 514 pregnant women registered in the antenatal clinic of BKL Walawalkar Hospital in KONKAN region. They underwent oral glucose tolerance test using the protocol set by Maternal & Child Health Division of Government of India. Results: Five hundred and six women completed oral glucose tolerance test. Mean age, gestation, weight and BMI at registration were 26.7 years, 23.1 weeks, 48.3 kg and 20.5 kg/m2 respectively. Prevalence of GDM was 20.3% and more than 50% of them were diagnosed before 24 weeks of gestation. Conclusion: Our results have shown that presence of GDM despite absence of overweight (high BMI). This is the first demonstration of hyperglycemia in pregnancy among undernourished women of KOKAN region. Offspring’s exposed to intrauterine hyperglycemia are at increased risk of obesity, type 2 diabetes in postnatal life hence long term implication of GDM in KONKAN can only be understood by systematic, community based study. Generally, diabetes is more prevalent in middle-aged men than in age-matched women. Similarly, in most rodent models of diabetes, male rodents exhibit a more severe diabetes phenotype and a greater incidence of diabetes than their female counterparts. However, the underlying mechanisms by which sex modulates disease progression and outcome are poorly understood. Thus, rodent models of diabetes that exhibit sex differences, like those observed in humans are valuable tools for investigating the interaction between sex and biological processes. This review summarizes the pathogenesis of diabetes in several rodent models of type 1 and type 2 diabetes and the sex differences observed in the incidence and severity of diabetes. In addition, we discuss the possible biological mechanisms through which these differences arise. We describe the protective effect of estrogens in modulating the endoplasmic stress response that appears to contribute to sexual dimorphism in various rodent models of diabetes. In addition, we compare the differences observed in the regulation of the immune system response and nutrient metabolism in autoimmune and obese model of diabetes, respectively. We conclude that a better understanding of the biological processes that underpin sex differences in diabetes is needed to improve personalized approaches in diabetes prevention and treatment strategies.","PeriodicalId":91536,"journal":{"name":"Journal of diabetes research and therapy","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67392741","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Research design and methods: We carried out a prospective and descriptive study. The study involved 150 patients with type 2 diabetes who were followed on a regular basis. The different epidemiological, clinical and biological data were collected patient’s medical file. Each patient was investigated for cardiovascular risk factors. A thorough physical examination was performed in addition to biological assessment (fasting plasma glucose, HbA1c, lipid profile). After consent had been obtained, all patients underwent the ankle-brachial systolic pressure index (ABI) test. Results: The mean age of our patients was 57.46 ± 8.04 of the 150 patients, 24.7% had abnormal ABI suggestive of lower limb ischemia. Among them, 8.7% had an ABI of 1, 3, a presumed marker of vascular stiffness. The mean age of the LEOPAD population was 58 years. LEOPAD was predominantly asymptomatic in our population (88.1%). Only 5 patients (11.9%) had intermittent claudication. The comparative study of patients with LEOPAD showed a statistically conversely significant correlation between LEOPAD and lower levels of HDL. However, there is no statistically significant correlation with the following factors: smoking, sex, hypertension, dyslipidemia, obesity or overweight android pattern, the presence of microalbuminuria, renal failure and sedentary lifestyle. In terms of glycemic control, it did not show a statistically significant difference between the two groups. Moreover, it was noted that the patients with LEOPAD had had more micro vascular complications compared to patients without LEOPAD. Conclusion: Lower extremity occlusive peripheral artery disease occurred more commonly than it was expected in the diabetic population, associated with a high cardiovascular comorbidity. The sensitivity, specificity and simplicity of the ABI measurement promote its routine use and integration into the clinical approach of high-risk LEOPAD patients, especially the diabetics.","PeriodicalId":91536,"journal":{"name":"Journal of diabetes research and therapy","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67392708","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The primary factor preventing widespread uptake of CGM is cost. Currently there is little provincial coverage for CGM, and many patients still must pay for the technology. The cost of CGM is here evaluated in comparison to standard care (self monitoring of blood glucose), including analysis of cost-savings and improvements in quality of life that could be achieved with the use of CGM. When costs of comorbidities and absenteeism that could be reduced by the use of CGM are included in the analysis, it is determined that CGM is cost-neutral relative to standard care.","PeriodicalId":91536,"journal":{"name":"Journal of diabetes research and therapy","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67392665","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives: We decided to produce a detailed review of the literature using a systematic evidenced-based approach and assess the benefits and limitations of their clinical efficacy and usability of mHealth applications (including smart phones) in older individuals (≥ 55 years) with type 2 diabetes in randomised Controlled Clinical Trials (RCTs) and to identify future research directions in this area. Methods: We identified 15 RCTs which matched our selection and inclusion criteria and using a highly descriptive approach, we reviewed changes in areas such as changes in HbA1c, blood pressure, lipid measures, physical activity levels, user satisfaction and self-behaviour changes. Results: Improvements in metabolic measures including HbA1c, body weight, physical activity, blood pressure and lipid profile were observed. Some improvements were noted in diabetes knowledge, patient satisfaction and personal behaviour and mHealth applications were noted to be useful in some cases of self-titration of hypoglycaemic medications but this was based on limited evidence only. Unfamiliarity, skills and technique acquisition with the mobile app were the main factors for dropouts. Conclusions: This review revealed that there was a lack of longer-term studies (>1 year), and few studies in people aged 70 years and over! In addition, few studies had a nurse or therapist support mechanism as part of the intervention. The review suggests the need for longer term intervention studies that involve personalised diabetes management programmes and an assessment of their cost-effectiveness.","PeriodicalId":91536,"journal":{"name":"Journal of diabetes research and therapy","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67392574","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
