Severe claudication is a frequent symptom for which angioplasty represents a possible therapeutic avenue. We report the outcomes of four patients with PXE treated by angioplasty and stenting of the superficial femoral artery in two centers. These patients exhibited an abnormal failure rate for angioplasty and stenting of the superficial femoral artery, suggesting an as yet unknown susceptibility in such patients. In the absence of further evidence, we do not recommend arterial angioplasty with stenting as a primary surgical approach in PXE patients with femoral artery lesions.</p></div>","PeriodicalId":91348,"journal":{"name":"Journal of vascular surgery cases","volume":"1 4","pages":"Pages 276-278"},"PeriodicalIF":0.0,"publicationDate":"2015-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.jvsc.2015.10.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"54824734","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report a patient successfully treated in an endovascular manner, thereby applying minimally invasive techniques to avoid the morbidity of open thoracotomy. Decreased short-term morbidity with the endovascular approach offers significant advantage over open repair, thereby omitting aortic cross-clamping, thoracotomy, and increased risk from the patient's comorbidities. An endovascular approach to this potentially devastating complication of thoracic spinal instrumentation can be both safe and effective in selected patients and not exclusively performed in cases of hemorrhage, hematoma, or pseudoaneurysm.</p></div>","PeriodicalId":91348,"journal":{"name":"Journal of vascular surgery cases","volume":"1 4","pages":"Pages 264-267"},"PeriodicalIF":0.0,"publicationDate":"2015-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.jvsc.2015.09.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"54824683","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Preceding cardiac procedures often compound the high rate of morbidity and mortality associated with open repair. A case is described of an iatrogenic pseudoaneurysm in a patient with a recently placed prosthetic aortic valve and a clinical course precluding repeat open operative procedure. An endovascular approach was used, with placement of a thoracic aorta endograft with temporary cardiac pacing and a double-curved Lunderquist wire to avoid instrumenting the prosthetic aortic valve. At 9 months of follow-up, the patient returned to his baseline activity status, and at 24 months, had no symptoms or signs of infection, and a computed tomography angiogram demonstrated pseudoaneurysm exclusion with no graft migration.</p></div>","PeriodicalId":91348,"journal":{"name":"Journal of vascular surgery cases","volume":"1 4","pages":"Pages 283-286"},"PeriodicalIF":0.0,"publicationDate":"2015-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.jvsc.2015.09.002","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"54824694","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
