Coulibaly, Kadidiatou Singaré, Mohamed Keita","doi":"10.11648/j.ijo.20241001.16","DOIUrl":"https://doi.org/10.11648/j.ijo.20241001.16","url":null,"abstract":"<i>Aim</i>: Study the epidemiological, clinical and therapeutic aspects of laryngo-tracheo-bronchial foreign bodies. <i>Materials and method</i>: This was a retrospective cross-sectional study which focused on 41 cases of laryngo-tracheo-bronchial foreign bodies collected at the Mother-Child University Hospital Center “Le Luxembourg”, from March 31, 2013 to March 31, 2023. <i>Results</i>: Forty-one cases of laryngo-tracheobronchial foreign bodies were collected. The male gender represented 65.9%. Children under 2 years old accounted for 56.1%. The reasons for consultation were dyspnea in 73.2% and cough in 26.8%. The consultation time was 51.2% in the first 48 hours with extremes of 6 hours to 11 months. The history revealed a penetration syndrome in 78% of cases. The complementary examination of first resort was the front and lateral chest radiography. He did not visualize a foreign body in 75.6% of cases. Endoscopy was performed in all cases except for one case who died during transfer to the emergency room. The location was the right main bronchus in 63.4% of cases. The foreign bodies were organic in nature in 70.7% and 21 cases of peanut seed was found (51.21%). The postoperative course was simple in all cases under antibiotic-corticotherapy treatment. <i>Conclusion</i>: Laryngo-tracheo-bronchial foreign bodies predominate in young children. The most suggestive sign is penetration syndrome. It can be life threatening. It is a sepsis difficult to recognize and most often due to Fusobacterium necrophorum. The purpose of this case report was to discuss the diagnosis and treatment of Lemierre syndrome in children. Observation: A three-year-old child consulted for rhinopharyngitis complicated by an abscessed cervical adenophlegmon. The symptomatology had been evolving for 7 days with a notion of self-medication. The blood cell count showed hyperleukocytosis at 14.06.10<sup>3</sup> elements/mm<sup>3</sup>, anemia at 8.2.10<sup>6</sup> elements/mm<sup>3</sup>, and platelets at 102.10<sup>3</sup> elements/mm<sup>3</sup>. CRP was 258.5 mg/dL. HIV serology was negative. Bacteriological culture of the biological fluids collected (abscess pus, blood and joint fluid) was sterile. The CT scan revealed a collection of abscesses in the right sub-angulo-mandibular region. It was associated with thrombophlebitis of the internal jugular vein, as well as pulmonary and joint infections. The diagnosis of Lemierre syndrome was done. Cervicotomy allowed drainage of the abscess. A triple parenteral antibiotic therapy (Ceftriaxone, Metronidazole, Gentamycin) with per-os relay by Amoxicillin + Clavulanic Acid allowed to obtain the cure in one month of treatement without complications. Conclusion: LS is difficult to diagnose due to its non-specific clinical presentation and low incidence. Generally benign, but can have osteolytic powers and the possibility of transformation into malignancy during its evolution. Several hypotheses emerge in the literature, however the most frequent remains viral origin with the involvement of human papillomavirus (HPV). This is a case of bilateral inverted papilloma in a 60-year-old male patient at the Reference Health Center of the VI commune of the Bamako-Mali district. The symptoms appeared about 14 months ago, marked by a bilateral anterior rhinorrhea, of gradual appearance, associated with nasal obstruction, epistaxis, headaches and anosmia. Thus the diagnosis was made using physical examination and additional examinations such as the scanner of the facial massif, the anatomopathological examination of the operating room, and the test of the human papillomavirus, which turned out to be positive for HPV16 and HPV18. The management consisted of endonasal surgery, with the complete macroscopic removal of the mass and an average meatotomy accompanied by two years of follow-up after the surgical procedure. Patients and methods: A prospective, observational study including 85 patients with unilateral COM (with or without cholesteatoma), the study was conducted at the Otorhinolaryngology Department, Damietta Faculty of Medicine, Al-Azhar University, Egypt between August 2020 and December 2021. <i>Results</i>: In the present study (30.59%) of cases were having a unilateral squamous disease and (69.41 %) were mucosal CSOM, (70.6%) of patients having a diseased contralateral ear, of which (76.9%) having squamous disease and (67.8%) having mucosal CSOM. Pars tensa retraction was the most common abnormality (60%) seen in cases with squamous OM while thin TM was the most common abnormality (50%) of cases with mucosal CSOM. Pars tensa retraction was more severe in CLE of patients with squamous disease, (41.7%) were grade 4, while in mucosal type (50%) were grade 1. HRCT of mastoid reveled abnormalities in (24.7%) of all CLEs, more prevalent (62%) in the CLEs of cases with squamous disease than cases with mucosal COM. Audiologic examination of the CLE revealed (47%) having conductive hearing loss CHL of average 32.1 dB {36.5 dB average hearing loss in (70%) of cases with squamous type and 27.7 dB average hearing loss in (30%) of cases with mucosal CSOM}. Tympanometric evaluation of the CLEs revealed (50.6%) having abnormal curve {(73%) of cases with squamous type where type C curve was found in (84.2%) and type B curve in (15.8%)}, and in (40.7%) of cases with mucosal CSOM. where type C curve was found in (83.33%) and type B curve in (16.7%). <i>Conclusions</i>: The present study has confirmed the presence of high incidence of abnormalities in the CLEs of unilateral squamous and mucosal COM which is more in squamous type, it emphasizes the importance of considering both ears as a pair. Disease in one ear especially in squamous type needs a close follow up of other ear, as in most cases there a hidden pathology which needs attention for complete cure. The study aimed to investigate the epidemiological-clinical profile, treatment and determinants of Ear-Nose-Throat disorders in children attending the paediatric department of the KIMPESE Evangelical Medical Institute (EMI) hospital. This was a prospective cross-sectional study with a descriptive and analytical approach, which took place between October 2022 and March 2023. The KIMPESE Evangelical Medical Institute (KIMPESE/EMI/) was the setting for this study. The study included all children aged 0-16 years who had been consulted or admitted for any sign of Ear-Nose-Throat disease, whose parents had given their consent. The data collected was encoded an Excel 2016 database. They were transferred to SPSS for Windows version 21 for processing and analysis. Out of a total of 205 children cared for at the EMI, 139 had presented with at least one ENT pathology, i.e. a frequency of 67.8%. The median age of the children was 5 (2-10) years, and more than half were under 5 years of age. They were predominantly male, with a sex ratio of 1,13. The ENT pathologies presented by these children were distributed as follows: 28.8% suffered from otological pathologies, 25.4% from rhinological pathologies and 17.1% from pharyngological pathologies. The most common otological pathologies were cerumen impaction and congestive AOM, while rhinitis was the most common rhinological pathology and rhinopharyngitis was the most common pharyngological pathology. The factors associated with ENT pathologies in children, in multivariate analysis, were determined by passive smoking, a history of rhinopharyngitis, household size ≥6 people, and living in a polluted environment. This study has shown that oral diseases in children are also common in rural areas, the case of KIMPESE Evangelical Medical Institute, especially in our African countries where the population is very young. Many factors are known to cause tinnitus but sensorineural hearing loss (SNHL) was found to be the most common. The objective of this study is to determine the actual frequency of idiopathic tinnitus and frequency of hidden SNHL in patients presenting with idiopathic tinnitus without hearing loss. This cross sectional study was performed at PAF Hospital Islamabad between December 2018 to Dec 2022. All the patients that presented with tinnitus without any subjective complaint of hearing loss were included in the study. All the possible causes of tinnitus were ruled out before labelling it idiopathic tinnitus. PTA was performed in all patients. The number of patients was calculated who had tinnitus without hearing loss on PTA. These were the patients with idiopathic tinnitus (G Hi ). Then these patients were tested for hearing loss on PTA in frequencies higher than 8000 Hz to find out hidden hearing loss. In these patients, the number of patients having hearing loss in frequencies higher than 8000 Hz (G H ) was calculated. These patients (G H ) were removed from the list of idiopathic tinnitus and were the patients with hidden SNHL. In this way the actual frequency of idiopathic tinnitus (G i ) was calculated. The results were tested biostatistically using SPSS 20. Chi-square, simple t test and Fisher’s exact test were applied. The significance of results was calculated. Out of 100 patients, 65 were female and 35 were male. The mean age was 28.9±11.7 years. On audiometry only 10% patients had hearing loss in frequencies usually tested during audiometry (500Hz-4000Hz), Gs. All of these were males (100%). Routine audiometry was normal in the remaining 90% of patients (G Hi ). When these patients were tested in frequencies higher than 8000 Hz, out of these 90% patients, 55% of the patients had high frequency hearing loss (>8000 Hz), (G H ); this is the hidden hearing loss. P values were < 0.001and 0.75 on Fisher’s exact and Chi-square tests, the results were found to be significant. It is often seen that patients with idiopathic tinnitus develop SNHL later in life. Therefore, tinnitus can be considered as an early symptom of hearing loss. Hemoptysis is one of the common presentations of these tumors. Carotid blowout syndrome (CBS) due to bleeding from the pseudo-aneurysm (PA) is a dreaded complication of head and neck tumors. Sometimes it can be massive and difficult to control and maybe fatal. Surgical bypass or ligation of external carotid was the only option available, but it had high mortality and morbidity associated with it. Endovascular management has given a new hope for these patients, in the form of covered stent implantation. These endovascular stents works by reconstructing the arterial wall, maintain its patency and decrease the ischemic complication rate which was higher with surgical option due to vessel deconstruction strategies. Recurrent bleeding can occur due to infection of stent which is a fearful complication but it occurs late and its reported rate in the previous studies ranges from 33-86%. We present a case of such tumor complicated with massive bleeding, which was managed by an endovascular approach.
