These patients are a high-risk subgroup who present addi- tional challenges to the surgeon when performing laparoscopic cholecystectomy. It is important that all surgeons who perform this procedure have a safe strategy they can revert to. This article outlines our approach. After obtaining pneumoperitoneum via a supra-umbilical incision, we advise placing a fascial suture before proceeding with the operation. This allows for high-quality closure, reduces the incidence of incisional hernia, and reduces the risk of inadvertent bowel injury. We also advise the repositioning of the patient on the operating table prior to port placement such that an ergonomic set-up can be achieved. In addition to standard ports, we use an additional twelve-millimetre port in the left upper quadrant. A fan retractor can be inserted via this port and used to gently retract the duodenum inferiorly. This provides adequate exposure for Calot's dissection and arguably reduces the risk of injury to a fatty liver. {"title":"Video game experience affects performance, cognitive load, and brain activity in laparoscopic surgery training","authors":"H. O. Individuals who interact or play video games, tend to have a better visuospatial ability when compared to non-gamers. Numerous studies suggest that video game experience is associated with faster acquisition, greater sharpening, and longer retention of laparoscopic skills. Given the neurocognitive complexity of surgery skill, multimodal approaches are required to understand how video game playing enhances laparoscopy skill.\u0000\u0000METHODS\u0000\u0000Data from thirteen surgeons with varying levels of laparoscopy experience and no video game experience, and 27 students with no laparoscopy experience and varying levels of video game experience performed standard laparoscopic training tasks. Their performance, subjective cognitive loading, and prefrontal cortical activity were recorded and analyzed.\u0000\u0000RESULTS\u0000\u0000The surgeons performed the tasks significantly faster, had significantly lower cognitive load as well as less left prefrontal activation, relative to the non-gamer novices. In gamer novices, the task completion time decreased significantly with increasing game experience. The cognitive load decreased significantly with increasing gaming experience in gamer novices and with laparoscopy experience in surgeons.\u0000\u0000CONCLUSION\u0000\u0000Our results suggest that along the dimensions of performance, cognitive load, and brain activity, the effects of video gaming experience on novice laparoscopy trainees are similar to those of real-world laparoscopy experience on surgeons. Existing evidence shows that the work Relative Value Unit (wRVU) does not accurately represent surgeon's work. Therefore, the current study attempted to develop a local model to measure surgeons' work.\u0000\u0000\u0000Material and Methods\u0000This study was conducted in two main phases of determining the metrics and model development using quantitative and qualitative approaches from December 2019 to April 2021. Literature review, focused group discussions, and interviews were used to collect data. Con- tent analysis and Exploratory Data Analysis techniques were applied to analyze data.\u0000\u0000\u0000Results\u0000The findings demonstrated that patient's conditions (age, severity of disease at referring time, and comorbidities), disease specifications (time, complexity, physical effort, and risk), and provider characteristic (surgeon's willingness, imposed stress, and surgeon's skill) were important by 17, 51, and 32%, respectively, in determining surgeons' work.\u0000\u0000\u0000Conclusion\u0000Determining a fixed value for each procedure does not accurately estimate the amount of required surgeon's work for any procedure. Many factors, such as the patient's condition, surgeon's characteristics, and disease specification affect surgeons' work in the operation room.
