In the respiratory system, the hormone has implications for a variety of processes, including lung development, alveolar type II cell function and differentiation, cell proliferation, apoptosis, motility, and matrix interactions. PTHrP regulates branching morphogenesis and type II cell maturation in fetal lung through bidirectional interactions with lung fibroblasts. Consequently, it appears to have a role in the pathophysiology of respiratory distress syndrome and bronchopulmonary dysplasia. In adult lung, the molecule has autocrine actions on type II pneumocytes that stimulate phosphatidylcholine production, inhibit cell proliferation, and sensitize to apoptotic stimuli. Type II cell PTHrP levels fall after lung injury, inversely coordinated in time with their proliferation, suggesting that reduction in its growth inhibitory effect may contribute to the increase in type II cell number...","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"130-134"},"PeriodicalIF":0.0,"publicationDate":"2012-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612443067","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65895933","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Useful for diagnostic and therapeutic purposes, prior studies have documented the inherent safety of this procedure. In proper hands, modern techniques such as the use of safety (Turkel) tipped needles and ultrasound localization have reduced the risk of major complications to less than 1%. Nevertheless, major bleeding remains a dreaded complication of this procedure, especially in patients with liver failure. Our historical experience has been that this complication is unpredictable, likely because of procedural injury to an abdominal wall artery or peritoneal varix. The authors report a case where the risk of vascular injury was mitigated using portable ultrasound and suggest a simple procedure for future risk reduction.","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"58 1","pages":"128-129"},"PeriodicalIF":0.0,"publicationDate":"2012-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612442381","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65896126","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Septic cardiomyopathy is well described in the literature but is both underrecognized and undertreated clinically. Direct cardiac assessment with transesophageal echocardiography (TEE) has documented advantages, but is limited by single evaluations and the need for advanced training. Recently, a disposable, monoplane TEE probe that provides for serial echocardiographic assessments in unstable patients to guide medical management, or hemodynamic TEE (hTEE), has been introduced. This article reviews a case of a 48-year-old gentleman with a history of smoking, chronic obstructive pulmonary disease, and Child’s A cirrhosis who underwent an uneventful off-pump coronary artery bypass grafting and 2 days later developed septic shock from pneumonia complicated by right ventricular dysfunction. hTEE provide...","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"135-138"},"PeriodicalIF":0.0,"publicationDate":"2012-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612442686","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65896190","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As a result, emergency management is essential for government agencies to private businesses, including hospitals. The events of 9-11 have drastically changed the way individuals think and operate in our health care environment. This report describes an emergency incident that occurred in the author’s medium-sized urban hospital because of loss of power from the power grid due to theft. The emergency that ensued lasted for more than 24 hours and confronted the hospital with unique challenges. This article describes the basic information about the general structure of hospital emergency preparedness and the specific aspect of preparedness that affect the intensive care unit. During the course of the emergency situation great insight into the function of the emergency preparedness plan was gained. In the end, many valuable lessons were ...","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"144-148"},"PeriodicalIF":0.0,"publicationDate":"2012-04-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612443381","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65896660","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The authors evaluated factors associated with prolonged hospital stay in a large single center series. Prolonged hospital stay was defined as more than 30 days. A total of 578 adult cadaveric liver transplants were included, and of these, 160 (27.7%) had a prolonged hospital stay. These patients had shorter waitlist time, higher preoperative MELD (model for end-stage liver disease) scores and received organs from donors with lower donor risk indices. In multivariate analysis, only preoperative MELD score remained significant. Postoperatively, there was no difference in the incidence of acute kidney injury; however, patients with prolonged hospital stay were more likely to have early allograft dysfunction and a higher 90-day mortality.","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"75-79"},"PeriodicalIF":0.0,"publicationDate":"2012-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612439315","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65895826","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

After each transplant, she required extended periods of circulatory support via extracorporeal membrane oxygenation (ECMO). The patient was never able to be weaned from ECMO. In the end she developed extensive multisystem organ failure and expired 45 days after the initial attempt at transplantation after spending the entire postoperative time on ECMO. The family, ethical, and sociopolitical issues raised by the extended course of care for this patient are discussed.","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"91-96"},"PeriodicalIF":0.0,"publicationDate":"2012-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612436971","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65896154","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Telemedicine involves real-time, live interactive high-definition video and audio communication that allows critical care physicians to have a virtual presence at the bedside of any critically ill patient. There is increasing data to support new care models that incorporate telemedicine in caring for the critically ill, resulting in higher care quality; more efficient resource use with improved cost-effectiveness; and higher patient, family, and remote provider satisfaction. As further research is conducted, the best use of telemedicine will be better defined and will result in increased access to critical care expertise to a larger population of patients requiring ICU services.","PeriodicalId":89772,"journal":{"name":"ICU director","volume":"3 1","pages":"70-74"},"PeriodicalIF":0.0,"publicationDate":"2012-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1177/1944451612439207","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65896267","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, DKA can trigger a systemic inflammatory response with the potential for multiple complications. This 30-year-old woman with type 1 diabetes mellitus improved within 48 hours of admission with a standard DKA treatment protocol. However, subsequent chest X-rays revealed bilateral infiltrates consistent with pulmonary edema; she then developed fever, hypotension, and acute-on-chronic renal failure. An altered level of consciousness prompted evaluation with neuroimaging, which revealed ischemic infarcts in the right frontal, parieto-occipital, and temporal cortices and cerebellum correlating with a left-sided hemiparesis. This case illustrates the potential complexity in the pathogenesis and management of patients with DKA. This patient likely had a systemic inflammatory response syndrome with multiorgan failure during this episode of DKA.
