Grodzicki","doi":"10.31139/CHNRIOP.2019.84.2.08","DOIUrl":"https://doi.org/10.31139/CHNRIOP.2019.84.2.08","url":null,"abstract":"The goal of his study was to show that oxygen saturation measurement in the distal part of injured upper extremity is one of the best method to assess post-traumatic blood flow disturbances and the necessity for exploration of brachial artery in supracondylar fractures. 32 children, age 4-16, with Gartland’s displaced extension type III injuries and distal disruption of blood flow were treated between 2014 and 2018. Blood disturbances were measured by pulse oximeter. Very good or good results were achieved in 22 non-operatively treated patients where pulseless pink hand symptoms occurred and, after reduction, disappeared. Very good or good results were achieved in 9 patients with pulseless pale and cold hand symptoms. In one patient with pulseless hand (verified by pulse oximeter) but without symptoms of cold hand we decided to explore brachial artery after angio CT examination (Angio-CT showed complete entrapment of the brachial artery). In conclusion we think that oxygen saturation measurement in dislocated supracondylar fractures in children is the best method. It allows us determine whether to avoid or to perform exploration of brachial artery. {"title":"Infection cure rate and functional results after 40 two-stage and 28 one-stage cemented reimplatations for knee prosthesis infected by methicillin resistant staphylococcus aureus","authors":"I. The study compares infection cure rate and functional results after oneand two-stage revision of total knee prosthesis due to MRSA infection. Material and methods. Results of 28 one-stage cemented revisions with addition of 5% of vancomycin and 40 two-stage revisions with hand-made, articulating cement spacer containing 5% of vancomycin were compared. The average follow up was 4.5 years ( 3 to 7.5 years ). Functional results were assessed in HSS, HKASS and WOMAC scores. As good result the lack of clinical, radiological and biochemical signs of infection, more than 61 points in HSS score and more than 50 points in HKASS score were assumed. Results were statistically analysed. Results. The infection cure rate was 95% after two-stage reimplantation with articulating spacer and 89,3% after one-stage revision. Functional results in HSS score were significantly better after two-stage reimplantations than after one-stage reimplantations (average 78.9 points vs 65.8 points, p-value 0.05; CI 95%: 72.4-95.6%). The satisfaction with treatment on HKASS scale was higher after two-stage than after one-stage revision ( average 78.5 points vs 68.3 points), but not statistically significant. Conclusions. They are most often associated with indirect trauma – fall on outstretched hand with elbow in extension and forearm in pronation [6,18]. Some of these fractures, more precisely the nondisplaced or minimally displaced ones, can be treated nonoperatively [19,20,22], but most require surgery. When treating patient with radial head fracture, surgeon should choose the most effective way of treatment that can provide quick stabilization of the elbow and allow early rehabilitation in the form of passive and active movement [21]. Formerly the operative armamentarium consisted of only open reduction with internal fixation of the fracture and radial head resection [2]. Many complications after radial head resection forced searching for a way of alternative treatment of severely displaced comminuted fractures [3,23,24]. Hence the idea of hemiarthroplasty of radiohumeral joint was born. There were thirteen radial head arthroplasties performed in our department. Only patients with Mason type III and IV fractures qualified for this type of treatment. Despite minimal invasive surgical techniques, the procedure is accompanied by the postoperative pain in various intensification. In this article we focus on the multimodal anlagesia, particulary taking local infiltration analgesia (LIA) in consideration. Review of current literature allows us to state that LIA decreases the level of postoperative pain, lower opioids usage, improves the rahabilitation and shortens hospital length stay. The study included the femoral heads of nine patients with AVN, aged between 30 and 68 years. The surgical specimens of femoral heads were obtained during the procedure of hip replacement. The measurements of bone mineral density (DXA method) included in vivo examination of patients with AVN, conducted prior to the hip replacement procedure, as well as the densitometry of surgical specimens of the whole femoral heads and their 1.0-1.5 cm thick slices. The surgical specimens were also examined under digital, polarization, and scanning electron microscope, and analyzed chemically by means of electron dispersive spectroscopy (EDS). Moreover, the cartilage isolated from surgical specimens was subjected to X-ray diffractometry. Our study revealed that densitometry of the proximal femoral epiphysis is not useful in the evaluation of AVN. We observed that AVN is associated with severe osteoporosis of subchondral bone at a periphery of the femoral heads, leading to complete bone loss (“holes”) in extreme cases. {"title":"Intramedullary nails coated with Antibiotic Loaded Acrylic Cement (ALAC) containing 5% of culture specific antibiotic in therapy for infected long bone fractures and nonunions","authors":"I. Treatment of fractures or non-union of long bones with infection requires debridement of the medullary cavity, antibiotics in high concentra- tions in the bone and biofilm, elimination of dead space in the place of debridement and mechanical stability at the site of infection. External stabilization is inconvenient for the patient and makes skin plastics difficult. Material and methods. There were 13 patients (6 women and 7 men), aged 18-82 years, treated with debridement and stabilization with intramedullary nail covered with Antibiotic Loaded Acrylic Cement (ALAC). Indications for nail covered with ALAC (ACIIN): infected fractures or infected nonunion of the femur (6), tibia (6) and humerus (1) diaphysis. Follow-up: 1-13 years (average 6.5 years). Etiology of infection: S. aureus (11), Staph. Epidermidis (1), mixed infection: MRSA and Pseudomonas aeruginosa (1). Up to 40 g of ALAC with gentamycin in a dose below 1 g were added: 2 g vancomycin (11), 1 g vancomycin and 1 g meropenem (1 – mixed infection), 1 g vancomycin and 1 g ceftriaxone (1– MSSA) – based on preoperative culture . In the absence of a conclusive antibiogram, vancomycin was a pre-emptive therapy, focused on the most common pathogen. Nail locking applied: static (5), dynamic (2); without locking (6). Results. Healing of the infection was achieved in 11, bone union in 9 of 13 patients. Non-union in 4 cases: large segmental defects of the femoral shaft (2); tibial nonunion (2). Conclusions. ACIIN nails with the addition of 5% targeted antibiotic can be an effective treatment for infected fractures or infected nonunions of the femoral, tibial and humeral diaphysis.
