Casas","doi":"10.1016/S1575-0973(09)73470-1","DOIUrl":"10.1016/S1575-0973(09)73470-1","url":null,"abstract":"<div><h3>Background</h3><p>The purpose of this study was to analyze the need for an epidemiological study of opiate-dependent patients aiming to improve the knowledge about their treatment and medical and psychiatric comorbidities.</p></div><div><h3>Matherial and methods</h3><p>Data on clinical management, treatment type, and comorbidities in Spain were examined. There are no comprehensive data, representative of the Spanish patients. Therefore, a national representative sample was selected from opiate-dependent patients ongoing replacement therapy programs, attending care centers for opiatedependent patients.</p></div><div><h3>Results</h3><p>The representative sample included 624 opiate-dependent patients ongoing opiate replacement therapy programs from 74 drug-dependent rehabilitation centers in Spain.</p></div><div><h3>Conclusions</h3><p>Data on therapeutic management and psychiatric and medical comorbidities in opiate-dependent patients are not well known. Perception of the treatment influences its acceptability and therefore, whether it is followed and adhered to, thus modulating its effectiveness. The purpose of this study is to find out patient perception and satisfaction with a pilot buprenorphine treatment study in therapeutic communities and one outpatient centre.</p></div><div><h3>Material and methods</h3><p>The study was carried out with a qualitative methodology. The participants in the study were 18 patients that received treatment in therapeutic communities and one outpatient centre who were given a semi-structured interview. Results were analysed using the Constant Comparison Method, proposed by the Grounded Theory.</p></div><div><h3>Results</h3><p>The results obtained show that the patients perceive they are more comfortable during withdrawal treatment with buprenorphine than with methadone. The patients further perceive that the buprenorphine treatment is easier to adapt to their daily lives, to which the possibility of dispensing it in pharmacies would contribute. This research tries to evaluate the relationship between those nosologic entities in patients who assist for SUD's treatment.</p></div><div><h3>Material and methods</h3><p>Cross-sectional study, with descriptive and analytic aspects. The sample was 249 patients obtained by consecutive sampling in an Outpatient SUD's Unit.</p></div><div><h3>Results</h3><p>The 28.9% of patients had a probable ADHD. Although ADHD were not related to cocaine's or other stimulants’ addiction, heroine's addiction predicted more risk of ADHD (OR: 1.12-2.15).</p></div><div><h3>Conclusions</h3><p>The relationship between ADHD and SUD can’t be explained only by self-medication hypothesis. The clinical management of alcohol abuse in these patients is complex and treatment of alcoholism and substance abuse treatment is a key issue for reducing morbidity and mortality.</p></div><div><h3>Material and methods</h3><p>The case of a 44-year-old male with alcohol abuse and HCV/HIV coinfection is examined. A detailed toxicological and clinical history is described and a review made on the alcohol abuse treatment and general management of HCV/HIV coinfected patients.</p></div><div><h3>Results</h3><p>Priority is given to the treatment of alcohol abuse and better control adherence to HIV treatment in order to improve the immunological and virological condition. Álvarez","doi":"10.1016/S1575-0973(09)72410-9","DOIUrl":"10.1016/S1575-0973(09)72410-9","url":null,"abstract":"<div><h3>Aim</h3><p>To analyse the clinical case of a male with episodes of binge drinking, dependency on cannabis and cocaine consumption related to the binge drinking.</p></div><div><h3>Materials and methods</h3><p>This clinical case corresponds to a 34-year-old male attending an outpatient centre for treatment of binge drinking episodes. A systematic diagnosis is carried out, registering the patterns and periodicity of the consumption of alcohol, an evaluation of the diagnostic abuse criteria and DSM-IV and CIE-10 dependency, the use of alcoholism detection questionnaires (CAGE, AUDIT, MALT), instruments to evaluate problem drinking (CIWA-AR, Multidimensional Scale of Craving for Alcohol [EMCA]), physical and biochemical examination and an evaluation of the psychosocial consequences of substance abuse.</p></div><div><h3>Results</h3><p>After carrying out a systematic diagnosis, the patient was shown to be alcohol and cannabis dependent with a sporadic consumption of cocaine. The centre will carry out both pharmacological and psychosocial treatment, supported by cognitive-behavioural and group self-help therapies.</p></div><div><h3>Conclusions</h3><p>The patterns of alcohol consumption in our society have changed significantly. The pattern of an intense consumption of alcohol concentrated within a short space of time, the so-called “binge drinking” which has been adopted by adolescents and young adults, has spread beyond these groups to many adults. The percentage of the population that also associates the consumption of other substances with that of alcohol is on the increase. The medical and psychosocial resources will have to widen the basis of their treatments in order to adapt to these new forms of consumption, which can lead to addiction and have consequences of varying degrees of seriousness. Actually cocaine and cannabis use are increasing and opiates comsumption is declining. Everywhere relation between drug abuse and delinquency is stablished. The aims of the study were: a) to determine sociodemographic data related to drug comsumption in a jail detenees sample at court department; b) to obtain healthy data referring to VIH, VHB and VHC infections; c) to obtain cocaine, hachis, heroine, benzodiazepines and synthesis drugs consume data and d) to evaluate the relation between the adicción to drugs with the delictology.</p></div><div><h3>Matherial and methods</h3><p>Study realized in a population of 150 subject consumers of illegal drugs in the courts of policeman of the city of Barcelona, during a year and half.</p><p>Administration of a questionnaire with items related to sociodemografic, sanitary and drugs consumption. We obtained sample of urine to be analysed by enzimoinmunoassay in the analyzador (AsXym, Abbott). Results were interpreted like positive or negative acording to the cut-off established in the methods specifications.</p></div><div><h3>Results</h3><p>The profile of the sample is a single man, with primary studies and without a defined profession. The illegal drug more consumed is the cocaine (77,5 %) followed of the opiates (62,9 %), benzodiazepines in 61,6 % (self-administered 40,9 %) and cannabis (60,3 %). The clinical prevalence of VIH was 16,7 %, VHB 19,1 %, and VHC 37,9 %. The most related crime with the consumption of drugs was the robbatory.</p></div><div><h3>Conclusions</h3><p>A high consumption of cocaine is detected as well as a decrease of heroine. The consumption of cannabis and benzodiacepines follows being overhead. The third part of the studied population had consumed three different drugs. The clinical prevalence of infections by the VIH, VHB and VHC is overhead.
