{"title":"Intensification of antiviral therapy for chronic hepatitis B with autoleukocyte immunization.","authors":"Oleksandr B Herasun","doi":"10.2174/1872214812666180507095457","DOIUrl":"10.2174/1872214812666180507095457","url":null,"abstract":"<p><strong>Introduction: </strong>The article presents a new method of intensification of antiviral therapy for chronic hepatitis B with intradermal autoleukocyte immunization during treatment with a drug of nucleotide analogues - tenofovir (patent UA 113873 U, 2017); other curative vaccines for chronic hepatitis B are also regarded.</p><p><strong>Objective: </strong>The research involves patients with chronic hepatitis B (30), whose replication of HBV DNA decreased to a certain level after long-lasting (over 2 years) antiviral therapy, its further reduction ceased. These patients underwent intradermal autoleukocyte immunization for intensification of antiviral therapy.</p><p><strong>Method: </strong>Leukocytes were isolated from plasma of a patient's heparinized venous blood by centrifuging at 400 g for 8 minutes, after that the precipitate was resuspended in 1-1.5 ml of own blood serum and injected intradermally into the back in the dose 0.1 ml. Immunization was conducted at least 3 times with 30-40-day interval.</p><p><strong>Results: </strong>Due to autoleukocyte immunization, the process of further decrease in virus reproduction resumed in 24 patients (out of 30; 80%). It is prevalent in Latin American, mainly in Brazil. Therefore, PCM has fundamental impact on the Brazilian global economy, especially in public health system, since it is affecting economical active population in different country regions.\u0000\u0000\u0000OBJECTIVE\u0000The present study aimed to standardize the Real Time-Polymerase Chain Reaction (rt-PCR) for an efficient and safe PCM diagnosis amplifying the recombinant protein PB27 gene, only expressed by specimens of Paracoccidioides genus.\u0000\u0000\u0000METHODS\u0000To standardize a methodology of rt-PCR using species-specific primers and probe designed for annealing in this specific region of the fungi´s genome, amplifying the recombinant protein PB27 gene, only expressed by specimens of Paracoccidioides genus. Followed by design in silico, experiments were performed in vitro to determine rt-PCR specificity, efficiency and genome detection limit.\u0000\u0000\u0000RESULTS\u0000The primers and probe sequences were deposited in Brazilian Coordination of Technological Innovation and Transfer (CTIT), under patent reference number BR1020160078830. The present study demonstrated the rt-PCR applicability for support on diagnosis of paracoccidioidomycosis, presenting low cost, which makes it affordable for public health services in developing countries as Brazil. It is noteworthy that it is necessary to validate this methodology using clinical samples before to use as a safe method of diagnosis. A review of all patents related to this topic was performed and it was shown that, to date, there are no records of patent on kits for paracoccidioidomycosis´s diagnostic. Indeed, there is still a lot to go to reach this goal.\u0000\u0000\u0000CONCLUSION\u0000The reaction developed was standardized and patented, opening perspectives to molecular diagnosis development for paracoccidioidomycosis, since rt-PCR can be applied to a broad spectrum of infectious diseases. {"title":"Iodine Prophylaxis in the Case of Nuclear Accident.","authors":"Szybinski Zbigniew","doi":"10.2174/1872214811666170530075117","DOIUrl":"https://doi.org/10.2174/1872214811666170530075117","url":null,"abstract":"<p><strong>Background: </strong>On 26th April, 1986 the greatest accident of nuclear plant in Czernobyl occured and isotopes with high percentage of release were erupted: 33-Xe, 131-I, 132-Te, 134-Cs and 137-Cs. The radioactivity of the isotopes was very high - for instance: 33-Xe 6500 PBq, 131-I 1760 PBq. Rest of the 15 isotopes represented similar radioactivity with shorter percentage of release. The most exposed group of people were 237 liquidators, and 11600 people living around had to be evacuated when the limit dose for a person (5mSv) was crossed. Ionizing radiation on the molecular level produces high energy radicals, water radiolysis and ionization of the atoms leading to damage of the enzymes activity centers and receptors, cell membranes DNA, intracellular lysosomes, and especially important for ATP synthesis - mitochondria. These destructions lead to tissue and organs damage.</p><p><strong>Objective: </strong>The aim of this article is the presentation of the protective property of iodine application in the case of nuclear accident.</p><p><strong>Methods: </strong>In Poland at that time, effective iodine prophylaxis did not exist. In the face of such exposition, a special Governement Commission was appointed. When permissioned maximal dose for children and adolescents 50mSv appeared in some areas of the country, the Commission made a decision about obligatory administration of a single pharmacological dose of potassium iodide for all children and adolescents till age 16. No relevant recent patents were available for this WHO report.</p><p><strong>Results: </strong>In this way, the dose of 131-I to thyroid for inhabitants in high, moderated, and low contaminated regions were reduced by about 45%. However, from 1987 to 1997 increase in prevalence of the differentiated thyroid cancer in adults in Polish population especially in women over 40 years old in the southern part of Poland was observed.</p><p><strong>Conclusion: </strong>Actually in European countries work 185 nuclear power plants and to 2045 another 100 is planned. It may also develop in association with various pathologic states. The challenge for the physician is to distinguish physiological gynecomastia from those with an underlying pathology.</p><p><strong>Objective: </strong>To review in depth the pathophysiology, clinical manifestations, and treatment of gynecomastia.</p><p><strong>Method: </strong>A PubMed search was completed in Clinical Queries using the key term \"gynecomastia\". Patents were searched using the key term \"gynecomastia\" from www.google.com/patents, www.uspto.gov, and www.freepatentsonline.com.</p><p><strong>Results: </strong>Gynecomastia is caused by an imbalance between the stimulatory effect of estrogen and the inhibitory effect of androgen at the breast tissue level. Clinically, gynecomastia is characterized by the presence of a firm or rubbery, discrete, subareolar ridge of glandular tissue that is symmetrical in shape, freely movable, and nonadherent to skin or underlying tissue. Since most cases of physiological gynecomastia regress spontaneously with time, reassurance is all that is necessary. For pathological gynecomastia, treatment should be directed at the underlying cause, if possible. If gynecomastia persists in spite of the above measures, pharmacologic therapy and reduction mammoplasty may be considered. Recent patents related to the management of gynecomastia are discussed.</p><p><strong>Conclusion: </strong>The majority of cases are physiological and do not require treatment other than reassurance. For pathological cases, the underlying cause should be treated if possible. If gynecomastia persists in spite of the above measures and treatment becomes necessary, tamoxifen is the treatment of choice. {"title":"Role of Iodine in Metabolism.","authors":"S. Over 1/3 of the global population is exposed to iodine deficiency, especially in the mountains. Iodine is a component of the thyroid hormones and a strong antioxidant with electric potential of -0.54 V. Iodine as iodide is taken up by the thyroid gland, gastric mucosa, salivary glands and mammary glands in pregnant and breast-feeding women. The transport of iodine to the thyroid gland is regulated by specific enzymes: Natrium Iodine Symporter, Na+/K+ATPase, Pendrin and Apical Iodine Transporter. In the thyrocytes, the thyroid hormones biosynthesis is carried out by iodization of tyrosine to T3 and T4. Secretion of T3 and T4 is regulated by TSH. On the tissue level, the active hormones are T3 and its derivative thyroamine. The most important target is the cardiac muscle, where T3 action results in an increase in the inotropic action and thyroamines exert a negative inotropic and chronotropic effects. The consequences of iodine deficiency are endemic goiter, irreversible brain damage, cretinism, impaired thyroid function and thyroid, gastric and mammary glands cancers. In 1994, the World Health Organization proposed the main strategy for iodine prophylaxis, i.e. Universal Salt Iodization. In Poland, a new model of iodine prophylaxis was introduced in 1996; the model includes obligatory iodization of household salt and neonate formulas and recommendation to take an additional daily dose of iodine for pregnant and breast-feeding women. This model allowed for eliminating endemic goiter in schoolchildren, eradicating cretinism in mountainous areas, and markedly diminished thyroid and gastric cancer incidence. In 2002, WHO classified Poland as a country with a proper daily iodine intake on the population level. Tiegh. (Loranthaceae) (Argentine mistletoe) is usually used in local folk medicine.</p><p><strong>Objective: </strong>We studied the effect of treatment with the Lc proanthocyanidin-enriched fraction (PLc) in rats fed with Cho-enriched diet on plasma lipids levels, the hemorheological parameters, and biliary secretion.</p><p><strong>Method: </strong>Adult male Wistar rats were fed ad libitum with a Cho-enriched diet (Cho (97% purity) 8 g/kg of diet and corn oil 280 g/kg of diet) during 28 days. Then, were separated in six experimental groups (n=5 each one), which were injected ip every 24 h with: 1) saline solution (control group, C) and 2) PLc, 3 mg/100 g body weight (treated group, C+PLc), during 3, 7 and 10 days. Group C presented an increase in plasma levels of Cho and Triglycerides (TG), and also, accumulation of hepatic lipid droplets. Also, cell shape and their corresponding morphological index (MI) were altered too.</p><p><strong>Results: </strong>The treatment with PLc at 3, 7 and 10 days produces a diminution in the plasma Cho, LDL-Cho and serum TG levels, accompanied by a diminution of the lipid accumulation in the liver. The rates of bile acid output in bile can explain the diminution of plasma Cho, evidencing that some of the enzymes involved in the cholesterol conversion into bile acids could be up regulated by the treatment with PLc, leading to the observed increase bile flow. PLc treatment leads to a diminution of plasma levels of Cho and TG.</p><p><strong>Conclusion: </strong>Essentially, the treatment with PLc, despite the duration produces a modification in hemorheological parameters approaching the values of the experimental group with standard diet. Plasma levels of Cho, LDL-Cho and TG represent selected markers to evaluate the effect of enriched extract from Ligaria cuneifolia.
