This article illustrates the use of a crimpable tube with a 90-degree bend on the base wire to secure a T-loop in segmental retraction. {"title":"A comparison between landmark and surface shape measurements in a sample of cleft lip and palate patients after secondary alveolar bone grafting.","authors":"Chung H Kau,&nbsp;Liana Medina,&nbsp;Jeryl D English,&nbsp;James Xia,&nbsp;Jamie Gateno,&nbsp;John Teichgraber","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>To compare landmark vs surface-shape measurements in a sample of patients with cleft lips and palates following secondary alveolar bone grafting.</p><p><strong>Methods: </strong>The faces of 10 patients (4 males and 6 females) with an unilateral cleft lip and palate were captured using a 3D surface camera system before and 6 weeks after alveolar bone grafting. In each face, six coordinates were registered. The pre- and postoperative images were superimposed on areas that were not affected by the surgery. Using 3D modeling software landmarks, nasal symmetry, and surface-to-surface deviation, analysis was performed. All data were subjected to standard statistical analyses.</p><p><strong>Results: </strong>Color map surface-to-surface comparison revealed a significant anteroposterior elevation in the nasal region of the cleft side after surgery.</p><p><strong>Conclusion: </strong>The ala, alar base, and paranasal areas are increased anteroposteriorly after secondary bone grafting. This surgery tends to diminish the asymmetry in nasal morphology typically seen in patients with unilateral cleft lip and palate. {"title":"Perception of discomfort during orthodontic treatment with tongue spurs.","authors":"Eustáquio Afonso Araújo,&nbsp;Ildeu Andrade,&nbsp;Gisele de Mattos Brito,&nbsp;Leonor Guerra,&nbsp;Martinho Campolina Rebelo Horta","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>To evaluate patients' and parents' reactions to the orthodontic treatment of open bite patients with a mandibular lingual arch and spurs (MASP) and to compare any changes in anxiety and discomfort during treatment in different age groups and sexes.</p><p><strong>Methods: </strong>Seventy-two patients completed a baseline questionnaire to assess their levels of discomfort and reaction to this appliance. The patients were divided into two groups: Group 1 consisted of 33 patients up to 14 years of age, and group 2 comprised 39 patients 15 years of age and older. A random group of 72 parents composed a special group. Parents' reactions were assessed through analysis of data gathered from an additional questionnaire with specific questions on their children's reactions.</p><p><strong>Results: </strong>Ninety-eight percent of the patients accepted treatment with an MASP. The discomfort time was up to 10 days in the majority of patients. The younger group and the female group seemed to show higher tolerance to an MASP. Speech and chewing impairments were the most common functional problems associated with an MASP. Seventy-six percent of subjects did not feel a lack of confidence when wearing an MASP.</p><p><strong>Conclusion: </strong>This treatment was well accepted by patients and parents. This positive result seems to be closely related to how informed they were. Following leveling, alignment, and endodontic therapy, the maxillary fused left central incisor was sectioned and the mesial portion extracted as planned. Fixed orthodontic treatment commenced after 10 days to align the maxillary anterior segment. After completion, the midline diastema was closed with veneers. Material property data were represented, boundary conditions were defined, and force was applied. Different situations were simulated in which a power arm of varying vertical lengths were attached at different locations on the tooth--namely, the incisal, middle, and cervical thirds.</p><p><strong>Results: </strong>The amount of bodily movement is maximum when the force is delivered directly at the cervical third. It decreases at the middle third and is least when attached at incisal third. The varying lengths of the power arm for a particular site of attachment does not bring about any change in the movement.</p><p><strong>Conclusion: </strong>The attachment of the power arm at the cervical third brought about maximum bodily movement, followed by the middle and incisal thirds. Variations in length of the power arm at different sites of attachment did not bring any change in the outcome.
