These are how service users are to be enabled to access more person-centred support, based on the values of 'independent living' and how this is to be possible for the diverse range of long-term health and social care service users. It draws on the experience of a national, service-user organisation and network, the work of the National Development Team and recent developments and findings in personalisation and 'self-directed support'. It highlights the importance of effective user involvement to take such developments forward effectively and inclusively and the key role that user-controlled organisations will need to play in making this possible.","PeriodicalId":89406,"journal":{"name":"Journal of care services management","volume":"4 1","pages":"24-31"},"PeriodicalIF":0.0,"publicationDate":"2009-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1179/CSM.2009.4.1.24","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65492292","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It demonstrates that the current centralised approach to providing services is designed to place control in the hands of the local authority, both at the assessment and care management stage and through the application of strategic commissioning, and as such asserts that this model will never be able to achieve true self-directed care. It argues that, although significant progress has been made in assessment and care management, equally radical change is required in relation to commissioning practice if the aims are to be achieved. The paper proposes a new model — the service gateway — that takes both operational and strategic decision making related to the provision of care against assessed need out of the hands of the local authority and puts power in the hands of individua...","PeriodicalId":89406,"journal":{"name":"Journal of care services management","volume":"3 1","pages":"334-356"},"PeriodicalIF":0.0,"publicationDate":"2009-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1179/CSM.2009.3.4.334","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65492498","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While Australia's minimum management and care standards expected are clearly described in the Aged Care Act (1997) Commonwealth, and Quality Principles (as amended), for many managers the pursuit of quality outcomes is a goal that extends beyond mandated requirements. Data voluntarily submitted to a quality performance database by 14 per cent of Australia's aged care service providers provide evidence of management focus on improving performance and their commitment to excellence in residential aged care through industry benchmarking and internal analysis of performance on key indicators. As healthcare budgets come under increasing pressure and demands for care to be delivered in a manner more convenient to patients increase, so interest in this method of care delivery has grown. In recent years, people have come to accept that not all patients need to receive their care in the acute hospital setting. Home healthcare services have grown, offering patients an alternative to acute hospital care. This paper briefly reviews the evidence base for home healthcare. While the discussion focuses largely on its contribution in the area of cancer, this model of care is also relevant to patients with other chronic conditions, including multiple sclerosis, rheumatoid arthritis and HIV.","PeriodicalId":89406,"journal":{"name":"Journal of care services management","volume":"3 1","pages":"357-363"},"PeriodicalIF":0.0,"publicationDate":"2009-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1179/CSM.2009.3.4.357","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65492552","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Historically, the separate care needs of older people have not been recognised and they have been treated as part of the whole prison population. Until recently, the Prison Service in the UK could claim Crown immunity from health and social care legislation that would apply to the general population. This has had to change as the over 60s have become the fastest-growing age group in the prison population. EU treaty obligations and High Court actions on human rights issues have led the UK Government to remove prison immunity. An example of this change may be found in the Disability Discrimination Act 2005 that now applies to prisons. However, despite this dramatic rise in the older prisoner population and new legislation, there is currently no mandatory requirement or national strategy for the care of older prisoners, although this is under consideration. With the transfer of prison healthcare...","PeriodicalId":89406,"journal":{"name":"Journal of care services management","volume":"3 1","pages":"364-374"},"PeriodicalIF":0.0,"publicationDate":"2009-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1179/CSM.2009.3.4.364","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"65492619","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These people participated in a sequence of up to eight narrative interviews over a six-month period. The findings revealed that the lives of these individuals differed markedly. Some felt that their lives had been enriched when they entered a place where they had increased opportunities for meaning, purpose and satisfaction. They described this experience as 'living with care'. Others felt that they were restricted — they had little choice in what they did and who they interacted with. Their views were not taken into consideration, and their daily life revolved around the routines and the regimes of the institution. These people described their experience as little more than 'existing in care'. Six dimensions of care home life influenced the individual's experience.
