Whether smoking habits of transplant recipients affect death-censored renal allograft survival is currently unclear. Methods: All (N=1197) recipients of a renal allograft or a combined kidney-pancreas graft, transplanted at our institution, between 1991 and 2001, were included in this study. Smoking habits were prospectively interrogated and noted in the central database. All clinical data were collected prospectively. Full data were available for 1066 patients. Statistical analysis was performed using log-rank testing and multivariate Cox proportional hazards analysis. Minimum follow-up is 10 years posttransplant. Results: Active smokers had significantly decreased overall graft survival compared to patients who never smoked (adjusted OR 1.60 [CI 1.2-2.1]; p=0.001), adjusted for other baseline demographics (donor and recipient age, gender, repeat transplantation). Also patients who quit smoking prior to transplantation had significantly decreased overall graft survival in comparison to patients who never smoked (adjusted OR 1.4 [CI 1.2-1.7]; p=0.0002). Patients who quit smoking did not do better than active smokers in terms of overall graft survival (adjusted OR 1.1 [CI 0.9-1.5]; P=0.39). Importantly, also death-censored graft survival was significantly worse in active smokers compared to patients who never smoked (adjusted OR 1.9 [CI 1.3-2.7]; p=0.0005), and compared to patients who quit smoking prior to transplantation (adjusted OR 1.4 [CI 1.0-2.0]; p=0.04). Interestingly, patients who quit smoking prior to transplantation had significantly worse death-censored graft survival compared to patients who never smoked (adjusted OR 1.3 [CI 1.0-1.7]; p=0.04). Conclusion: Smoking after renal transplantation is deleterious for the graft. Renal transplant recipients should be strongly encouraged to quit smoking, in order to maximally preserve graft function. However, ex-smokers never overcome the deleterious effects of smoking, and quitting smoking even before transplantation remains a significant risk factor for graft failure long after transplantation.","PeriodicalId":89378,"journal":{"name":"Graft (Georgetown, Tex.)","volume":"326 1","pages":"145-150"},"PeriodicalIF":0.0,"publicationDate":"2003-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86052543","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
