The organism is a member of the oral and gastrointestinal microflora of humans. The disease actinomycosis most commonly occurs in 3 body regions: cervicofacial (55% of patients), abdominopelvic (20%), and pulmonothoracic (15%). Involvement of other parts of the body is uncommon and usually secondary to a lesion in one or the other of the above sites. Extremity disease can occur by secondary involvement through direct extension or hematogenous spread. However, primary actinomycosis of an extremity is very rare. Barack Obama's presidency has opened the door for stem cell research by revoking statements and orders made during the former President Bush's administration. This provisional period will allow the National Institute of Health to rewrite policies governing how federal funds are distributed for stem cell research. Simanonok MD","doi":"10.1016/j.jcws.2009.06.001","DOIUrl":"10.1016/j.jcws.2009.06.001","url":null,"abstract":"<div><h3>Purpose</h3><p>Sirolimus is an immunosuppressive drug used as part of the drug regimen after kidney, liver, and heart transplantation. There have been numerous reports of transplant surgical wound healing complications secondary to sirolimus. The authors present a case of impaired wound healing in the lower extremity after kidney transplantation for a patient on sirolimus. This is one of the few reported cases that we are aware of that demonstrates the effects of sirolimus on wound healing at a nontransplant site.</p></div><div><h3>Methods</h3><p>A case highlighting aggressive limb-salvage modalities for a kidney transplant patient on sirolimus is presented. The subject was informed that data concerning the case would be submitted for publication. The etiology and comorbidities associated with chronic wounds can also complicate the assessment of the quantity and quality of the pain perceived by the patient. It is critical to adequately assess pain when establishing a comprehensive plan of care.
