{"title":"Perioperative Management of Obstructive Sleep Apnea in Patients With Syndromic Craniosynostosis Undergoing LeFort III Osteotomy With Distraction: A Case Series.","authors":"Kathryn W Brown, Shelley R Edwards, Ian C Hoppe","doi":"10.1177/19433875221142676","DOIUrl":"10.1177/19433875221142676","url":null,"abstract":"<p><strong>Study design: </strong>Retrospective observational study.</p><p><strong>Objective: </strong>The purpose of this publication is to address the absence of literature detailing respiratory management in patients with syndromic craniosynostosis and obstructive sleep apnea during the immediate postoperative interval following LeFort III advancement with placement of distraction hardware but prior to sufficient midface advancement.</p><p><strong>Methods: </strong>After IRB approval, the investigators retrospectively selected candidates for inclusion in this case series. The sample was composed of four patients ranging from 10 to 19 years of age undergoing LeFort III midface advancement during a one-year span at a single tertiary care center. All operations were performed by a single surgeon. Three of the selected patients suffered significant obstructive sleep apnea necessitating the operation, as determined by polysomnography. One patient experienced persistent apnea postoperatively requiring prolonged ICU level care.</p><p><strong>Results: </strong>Three of the four patients had severe OSA diagnosed by polysomnography with a median AHI of 28.3. Two of the three patients with preoperative OSA experienced no untoward respiratory compromise in the immediate postoperative period; one required nightly oxygen tent and the other required no supplemental oxygen. Patient 1 experienced significant postoperative respiratory distress with nightly apneic episodes and desaturations requiring supplemental oxygen and frequent stimulation.</p><p><strong>Conclusions: </strong>The present study suggests that early involvement of sleep medicine and management of patient expectations is vital. Extremely close postoperative monitoring in the ICU is necessary. Additionally, nutritional level of dams during pregnancy has the potential to improve offspring growth and fat development. The purpose of this study is to evaluate the intramuscular adipocyte development and expression level of related genes in bovine fetus, and the effect of increased crude protein (CP) intake during pregnancy on the growth performance and carcass characteristics of male offspring. Eighty six pregnant Hanwoo cows (average body weight, 551.5 ± 51.3 kg, age 5.29 ± 0.61 y) were used. Fetuses were collected at 90, 180 and 270 d of gestation from 18 pregnant Hanwoo cows. The remaining 68 pregnant cows were randomly assigned to 2 feeding groups. The control (CON) group was provided the standard protein diet (n = 34), and treatment (TRT) group was provided a diet with a 5% increase in CP intake (n = 34). Male offspring were divided into two groups according to protein treatment of the pregnant cows: CON male offspring (CON-O) and TRT male offspring (TRT-O). Intramuscular adipocytes were found in the fetal skeletal muscle after 180 days of gestation. Male calf's birth weight increased in the TRT group compared to that in the CON group (<i>p</i> < 0.002). The final body weight (<i>p</i> < 0.003) and average daily gain (<i>p</i> < 0.019) of male offspring were significantly higher in TRT-O than in CON-O. The feed conversion ratio was also improved by 10.5% in TRT-O compared to that in CON-O (<i>p</i> < 0.026). Carcass weight was significantly higher in the TRT-O group than that in the CON-O group (<i>p</i> < 0.003), and back fat was thicker in the TRT-O group (<i>p</i> = 0.07). The gross receipts and net income were higher in TRT-O than in CON-O (<i>p</i> < 0.04). The patients were categorized as 1. those receiving dual antiplatelet therapy containing a daily dose of 75 mg clopidogrel plus 81 mg aspirin (DAPT group), and 2. those not receiving DAPT (control group). Areas of GA, based on red-filtered fundus autofluorescence, were measured at baseline, and at 3, 6, and 12 months. The primary outcome was absolute 12-month changes in the square root (SQRT) area.</p><p><strong>Results: </strong>One eye in each group developed neovascular AMD and was excluded from the analysis. The DAPT (24 eyes) and control (22 eyes) groups had comparable age and baseline SQRT area (1.2 ± 0.27 and 1.8 ± 0.41 mm, respectively; <i>p</i> adjusted for age = 0.23). At 12 months, after controlling for age and the presence of soft drusen or reticular pseudodrusen, patients receiving DAPT had fewer changes in the SQRT area than that of the control group (0.097 vs. 0.17 mm; <i>p</i> = 0.02). The presence of drusen significantly predicted increased GA growth and choroidal thickness reduction.</p><p><strong>Conclusions: </strong>Routine uses of dual antiplatelets were associated with decelerating GA growth. In the spring of 2020, as the coronavirus swept across the globe, millions of people were required to make drastic changes to their lives to help contain the impact of the virus. Among those changes, scientific conferences of every type and size were forced to cancel or postpone in order to protect public health. Included in these was the European Geosciences Union (EGU) 2020 General Assembly, an annual conference for Earth, planetary and space scientists, scheduled to be held in Vienna, Austria, in May 2020. After a six-week pivot to an online alternative, attendees of the newly designed EGU20: Sharing Geoscience Online took part in the first geoscience conference of its size to go fully online. This paper explores the feedback provided by participants following this experimental conference and identifies four key themes that emerged from analysis of the questions: what did people miss from a regular meeting; and to what extent did going online impact the event itself, both in terms of challenges and opportunities? The themes identified are: connection, engagement, environment, and accessibility; and include discussions of the value of informal connections and spontaneous scientific discovery during conferences, the necessity of considering the environmental cost of in-person meetings, and the opportunities for widening participation in science by investing in accessibility. Myeloid-derived suppressor cells (MDSC) are tumor-induced cells mediating immune tolerance that are detectable in the peripheral blood of cancer patients. The authors previously developed a novel clinical assay to detect the phenotypes of two human MDSC subsets in peripheral blood, and hypothesize that higher MDSC levels measured by this assay correlate positively with both malignancy and worse patient outcomes.</p><p><strong>Methods: </strong>A prospective observational pilot study was performed of patients undergoing thyroidectomy for a solitary thyroid nodule. The presence of a thyroid nodule >1 cm was confirmed sonographically, and fine-needle aspiration biopsy performed prior to surgery in all cases. Peripheral blood collected preoperatively was analyzed using a novel flow cytometry-based immunoassay to detect and quantify two subsets of human MDSC. Circulating MDSC levels were compared by histopathologic diagnosis, stage, and presence of persistent disease after treatment.</p><p><strong>Results: </strong>Of 50 patients included in this study, MDSC measurement was successful in 47 (94%). One patient was found to have a concurrent cancer, leaving 46 patients for primary analysis. The cytologic diagnoses were benign in five (10.8%), atypia or follicular lesion of undetermined significance in five (10.8%), suspicious for follicular neoplasm in five (10.8%), suspicious for malignant in three (6.5%), and malignant in 28 (60.1%) of the 46 nodules. Final histopathology was benign in 11 (24%) and DTC in 35 (76%), encompassing 34 PTC cases and one follicular thyroid carcinoma. Mean percentages of CD11b(+)HLA-DR(low)HIF1a(+) MDSC (CD11b(+)MDSC) were 14.0 ± 6.2% and 7.9 ± 3.6% in DTC versus benign nodules, respectively (p < 0.005). A cutoff of 12% yielded a specificity of 0.91, a sensitivity of 0.72, and a likelihood ratio of 7.9. Mean CD11b(+)MDSC levels increased linearly with higher TNM stage (p < 0.01), and were 19.4 ± 5.4 in patients with persistent cancer after surgery compared with 13.2 ± 6.8 in those without evidence of disease (p < 0.05).</p><p><strong>Conclusion: </strong>MDSC measurement using this flow cytometry-based assay represents a novel approach for preoperatively assessing malignancy risk and cancer extent in patients with thyroid nodules. The Society has also recently added a blog on its website and the option of smaller workshop conferences to be hosted by various institutes and universities during the years when the biennial conference does not take place. The 2012 conference at the University of Texas–Austin was organized by Matt Cohen and Coleman Hutchison. Focusing on Oman's history after becoming a British colony, the second section of the book entails an in depth analysis of the splitting of the Omani-African state and the separation of Zanzibar from Oman in 1861. The slave trade brought about much heated debate between the Omanis and British, particularly as it provided an important source of revenue for the sultan. For the next few decades, Oman would be plagued with various uprisings and revolutions. It was the discovery of oil in the region that brought a shift to Omani politics and freed it from isolation. This eventually led Oman to play a regional leadership role in economic, social, and political development within the Persian Gulf area. The author provides a comprehensive study of the history of Oman. In addition, he offers the reader with some predictions for the future of this strategically-located state. In terms of political development, it seems the transition to democracy will continue to be relatively successful for Oman, especially in comparison to the rest of the region. Oman has a stable economy that is rooted in its local democratic development. Moreover, the country also enjoys diverse natural resources, which are more significant than those in some neighboring states. Ghubash argues that it is the current democratic foundation that gives hope to Oman's potential for full democracy in the future. The author concludes by saying that Oman's recent \"renaissance\" has helped to bring this country into the 2 l century. From a modernization theory perspective, one may argue that Oman possesses the necessary criteria (including education, wealth, and democratic tradition) to allow democracy to flourish. Oman: The Islamic Democratic Tradition is thorough in its historical analysis of the rise and evolution of the Omani regime. On the whole, the author has done a fine job of describing the past actions, illustrating the present trends, and even making a few useful predictions about the future political and economic development of this significant state. The late winter harvest season in Yemen is qiyddb and noljiydd (p. 606). I suspect someone familiar in Aden with the j>g shift failed to recognized the q>g dialectical shift in Sanaa, the dialect recorded in the original source cited by Ettore Rossi. A second significant value are the photographs, some 352 in total, although these are for the most part poorly reproduced in the reprint version. The maps come across as xeroxes and are not easy to read, an unfortunate result given the high price for the volume. For these visuals try to access the original text. The loss of the mother introduces Yasminc into a literate world that might have been closed off to her otherwise. That world of language comes at a price; she loses her link to the maternal and feminine and is unable to speak the language of women and tradition. Liberation ironically results in alienation and exile. In the novel's suspenseful conclusion, Yasmine finally comes into her own voice as she sings a lament on behalf of her lost mother. Yasmine insists upon the nomadic life, wears men's clothing, and writes stories and memoirs. Literate, rebellious, and of mixed racial origin, she is no ordinary figuration of the feminine, but instead a contradictory interstitial form that embodies the dialectic resolution of the violent past with a tentative hopefulness for the future. It is in mediating the internal conflicts between black Africa and Arab, and male and female, the novel seems to suggest, that Algeria will emerge as its own agent, speaking with a distinct voice. The implications of the novel's argument concerning gender and liberation are unclear. El-Majnoun states that: \"If I wanted to start a mass uprising, it would rather be to women that I'd turn\" (p. 77). The novel holds Yasmine up as its figure of agency and hope, and yet the politicized consciousness of Yasminc is enabled by sexual violence against the mother. The novel draws connections between violence towards (black African) women and agency and liberation that may be read politically, as a commentary on the victimization of women and racial minorities during nationalist liberation movements or psychoanalytically as the precondition for a uniquely \"feminine language.\" The novel's most disappointing moments are a result of the writer's expository writing style that deploys awkward metaphors. For example, locusts find their way into the title of the novel and clearly function as a crucial metaphor for destruction and rebirth in Mahmoud's story, but they are nearly absent from Yasmine's story, making the metaphor seem inconsequential. Century oflocusts will be a welcome addition to literature or cultural history courses. The novel is pedagogically useful for it probes the often-neglected intersection of race and gender during Algeria's colonial era.
