Perhaps less well known are the distortions to temporal judgement that are exhibited by patients with Parkinson's disease or schizophrenia. Both these classes of patient also suffer from hallucinations, which of course can be induced in hypnosis. It is argued that these are not chance parallels; they can all be accommodated by Gray's (1995) account of the neurophysiology underpinning consciousness. Recognizing that patients with hallucinations have their consciousness altered, it is concluded that hypnosis too represents an altered state of consciousness. The degree of alteration is shown to be correlated with the magnitude of time distortion experienced. In contrast with many studies, it is also reported that distortion is significantly greater in those who are highly susceptible to hypnosis. It is also increasingly employed as a tool in research, particularly in association with modern neuroimaging techniques. This paper reviews some of these ‘instrumental’ uses of hypnosis in relation to conversion disorder (paralysis), malingering, chronic pain and disorders of volition and motor control. In addition to their relevance to models of normal psychological functioning, the findings have significant implications for the understanding and treatment of a range of clinical conditions. They also serve to underline the subjective ‘reality’ of the experiences that accompany hypnotic phenomena. It is concluded that hypnosis has a significant role to play in mainstream psychological, neuropsychological and clinically related research. Gruzelier","doi":"10.1002/ch.35","DOIUrl":"10.1002/ch.35","url":null,"abstract":"<p>An update is provided of an earlier review (Gruzelier, 1998) of the range of evidence for neurophysiological changes in frontal and lateralized functions with hypnosis, changes which have differentiated high from low hypnotically susceptible subjects, and which led to a working model and neuropsychological translation of the hypnotic induction process. New evidence is outlined from an fMRI/EEG study. This study also disclosed the importance of neural efficiency in left lateral frontal and anterior cingulate structures, and their connectivity, for distinguishing high from low hypnotic susceptibility both in hypnosis and in the everyday state. This amplifies earlier constructs such as cognitive flexibility. Though the focus will be largely on the alteration of connections with the anterior brain and its corresponding alterations of function, interhemispheric, posterior and subcortical connectivity is also considered. The practical implications for the interaction between the hypnotherapist and subject are considered, including stage hypnosis. This paper gives a brief overview of the contemporary uses of hypnosis in paediatric and adult oncology and shows how hypnosis can be integrated into a total therapeutic process based on the needs and goals of the patient and the health care team treating them. The first section describes studies that have evaluated hypnosis in adult oncology. The second half consists of a review of the hypnosis literature in paediatric oncology. The paper concludes with suggestions for future research, and implications for clinical practice, which at the moment lags behind. One hundred and sixty-seven participants previously assessed for hypnotic and imaginative suggestibility were randomly assigned to distraction, cognitive-behavioural package (i.e., Stress Inoculation Training), hypnotic cognitive-behavioural package, hypnotic analgesia suggestion, placebo control, or no-treatment control conditions. The four ‘active’ treatments reduced pain more than the no-treatment control condition. There was no statistical difference in effectiveness between these four treatments, but only the cognitive-behavioural package reduced pain more than the placebo control condition. Response expectancies partially mediated the effects of treatment on pain. Imaginative suggestibility, defined as a generalized tendency to respond to imaginative suggestions delivered outside of hypnosis, moderated the effects of the cognitive-behavioural package. Contrary to prediction, neither hypnotic suggestibility, nor <i>hypnotizability</i> (operationalized as hypnotic suggestibility with imaginative suggestibility statistically controlled) moderated the effects of the hypnotic treatments. She had sought a number of different therapies, but not really benefited from any of them. She felt confused about herself and did not have a sense of her identity. She had recognized there were parts of herself that seemed separate from each other. Ego-state therapy with hypnosis helped her to understand the cause of her compulsive bingeing. This was followed by the hypnotic affect bridge which enabled her to access another child part of herself concerned with a fear of starvation and abandonment, and wanting to remain ‘solid’ but not ‘fat’. These phases of therapy produced great improvement in her eating behaviours, which she had not experienced before. By accessing the ego-states, she was able to start an inner communication, and make cognitive and emotional changes. This was reinforced later with cognitive therapy, from which she had not gained much benefit previously. The importance of combining these therapeutic approaches is discussed.
