The ability to age-in-place is affected by physical health status, along with access to services and social support. Few previous studies have included the experiences of lesbian, gay, and bisexual (LGB) midlife and older adults living in rural communities and their concerns about aging-in-place. This study used a qualitative approach to explore health care service utilization and support networks of 20 midlife and older LGB participants over the age of 40 who live in rural communities. Themes emerging from this study included transportation difficulties, lack of choices for care, affordability of care, and the lack of connection and sense of belonging to a community. Unique issues faced by LGB adults, such as isolation and the lack of informal support, make obstacles to aging-in-place for LGB adults living in rural communities particularly more difficult to overcome. The majority of MSM were 25 years or older, completed secondary school or higher, never married, and currently single. Just over a third of MSM reported not using a condom at last penetrative sex with a man. In a multivariate regression analysis controlling for several factors MSM had significantly higher odds of using a condom at last penetrative sex with another man if they were younger, had occasional partners in the past 6 months, ever had an HIV test and perceived themselves to be at high risk for HIV and significantly lower odds if they had sexual contact with a main male or with any female partner in the past 6 months and used alcohol in the past month. Accordingly, we present a qualitative process evaluation of an innovative PSE outreach service in England, UK. Through interviewing volunteers (n = 5), cruisers (n = 14), and all salaried staff (n = 5), we provide an insider's perspective highlighting both the challenges of PSE outreach provision and the unique solutions developed locally. Outreach success is achieved through respecting the local sexual culture within which cruising activities occur. Although a few research studies have assessed higher risk behaviors among this population, studies have not typically focused on safer sex practices. This article presents findings from an original research study designed to assess both high-risk and safer-sex requests among MSM within a sample from Florida (n = 483) who use a popular Internet social networking site to initiate sexual relationships. Data come from Wave III of the National Longitudinal Study of Adolescent Health, and include 11,619 participants (mean age = 22) who provided complete data on sexual orientation and behavior. Chi-square and multiple logistic regression analysis were used to examine associations between sexual orientation measures and sexual behaviors. Results indicate that males with a same-sex orientation (using an attraction, behavior or identity measure) were more likely than those with an opposite-sex or both-sex orientation to report engaging in sexual activity after a very brief acquaintance, and females with a both-sex orientation (using any measure) were more likely than those with an opposite- or same-sex orientation to report having multiple sexual partners. Predictors of sexual risk behavior were assessed with chi2 tests and multiple logistic regression. One-third reported unprotected receptive anal sex (URAS) last time and 36% inconsistent condom use in the past month. URAS was associated with younger age, less than high school education, low income, and low HIV transmission knowledge (adjusted odds ratio [AOR] = 2.1, 2.5, 3.7 and 2.5, respectively). Inconsistent condom use was associated with less than high school education (AOR = 3.2) and low HIV transmission knowledge (AOR = 3.5). We show that over one-fifth of MTSW have sex with IDU clients. Combined with high levels of risk behavior and very low levels of risk reduction and knowledge, the extent of sexual networking with men who inject drugs contributes further to the sex workers' health risks. Our findings suggest that isolated interventions with single-risk groups are unlikely to be sufficient to control the spread of the epidemic in Pakistan.
